Review: On 4/**/14, my husband, [redacted] and I went to the dentist for a cleaning. Before we left, I asked the front desk about my 2 daughter's getting their teeth cleaned, and coming by themselves, as my husband and I work during the day. I was given paperwork to fill out and told that I would need to send a letter of authorization, for them to perform the cleaning. I offered them the girl’s dental cards and was told to have the girls bring them, with them, at the time of their cleaning.

On 5/*/14, the girls went to the appointment, and handed the front desk their insurance cards, authorization letter, and completed information forms. The authorization form stated the following: I, [redacted] give you permission to give [redacted] and [redacted], a dental check up on 5/*/14. It shall include a basic dental cleaning and x-rays that their United Healthcare Insurance covers. Please call ###-###-#### with dental information of their checkup.

Please note, that on their dental information page, I was the only parent listed, as they were adopted before I was married. The first bill I received showed that the insurance did not pay. I called and spoke with [redacted]) and was told that MY HUSBAND'S insurance had been billed and the girls were not covered. I asked why my husband's insurance was billed, especially since his information was not on their information forms. Remember, we had only been there once and the girls were not with us????? She could not answer that question. I asked what happened with the cards the girls gave the receptionist....she could not answer. I was just told it was my responsibility to $596.00 for a basic cleaning and x-rays and that they don't take state United Healthcare. Then called back and asked that she fax me the information forms,that I filled out for both girls, and the authorization form. She then stated she had billed the state and I asked, how could she, if I didn't put any of that information on the forms? She said, after reviewing, I had put partial information on [redacted]'s forms. I then asked again for her to fax the information forms and authorization forms. She faxed me only the information forms...and there was ALL the insurance information, filled out on both forms. I then had to call her back to ask, again for the authorization letter I sent. She finally sent it 4 hours later. I originally asked to speak with someone above her and she told me she was the only one I could speak with. I asked for contact information on the [redacted]'s and was told that I would have to send it to her and she would give it to them....it took 4 phone calls to get their names.

Then I received a bill that never showed what they received from the UHC State insurance and 2 ppo discounts of 149.00 for each girl. On 7/**/14, I emailed her the following: Good Morning [redacted],

I received a bill in the mail yesterday and would like to know the status of it? Please explain what the PPO Discount is for [redacted] and [redacted]. As you know, per my authorization letter, I gave permission for “a basic dental cleaning and x rays that THEIR United Healthcare Insurance covers”. Please advise.

Her response email: Hi [redacted],

The discount was a courtesy that we gave to cut the service we provided in half. Unfortunately, the plan that your children have are a plan that we do not participate with. With the new healthcare plans in effect all children have dental insurance with their medical. We do participate with United Healthcare as well as other plans but do not participate with any state plans. [redacted]

I then responded: That is all fine and well, but because your dental office staff didn’t do their job of looking at the authorization form or the forms I filled out putting their insurance information on, I am not responsible for paying anything. Your staff had their cards and made copies and all the information to determine that their insurance would not work. There was even a phone number to contact me or my husband to discuss this before services were rendered. This is an error on your office’s part. Had my husband or I been contacted in regards to this issue, we would of said no. But, no one contacted us and apparently didn’t even look at the forms that I filled out. Your office just billed my husband’s insurance. [redacted], it is my hope, that you will take responsibility as the manager of the office and send me a bill showing that we have a zero balance. Your office made the mistake and I have the authorization form proving you were not authorized to render service, unless it was covered by THEIR insurance. I will pursue this until this matter is resolved.

Her response: I will express this to the [redacted]s and will be in touch next week. Thank you,

I never heard from her again and just received another bill for 298.00 on 8/*/11.Desired Settlement: I want the [redacted]'s to be sent this complaint. Their names are as follows: [redacted], and [redacted]. I want a final bill showing that I owe nothing.
