courtesyAfter the initial appointment is scheduled we send the patient a letter that includes the following paragraph: “If you have dental insurance please bring your insurance card so we may bill your insurance company correctlyPlease call your insurance to verify that your child has coverage with our providerIn addition, please inquire from your insurance company if you need prior authorization to be seen in our officePlease be prepared to pay your copay and/or deductible at the time of servicePlease be advised that if your insurance company does not accept assignment of benefits, we will ask for payment in full at time of serviceWe will gladly bill your insurance on your behalfYour insurance can then reimburse you for your services.” This patient’s mother was sent this letter in addition to patient forms to be filled out prior to the initial appointmentThe patient’s mother brought these patient forms to the initial appointment. The patient’s mother also signed a treatment estimate for two fillings on primary teeth for her childBecause we are never familiar with a patient’s insurance when they are new, we tell the patient this is only an estimate and underline the word estimate as emphasis to this policy(See signed document, attached) The patient’s mother’s insurance company called our office as a courtesy to this patient and stated that they have had a lot of problems with this employer, that employees were dissatisfied with this plan and they spent much time dealing with the complaints of these employees because of their dissatisfaction with the insurance provided by their employerBecause of these complaints toward the employer, this cell phone company (our patient’s employer) began to offer two plans to their employeesAt one time the employees only had one plan to choose from. When the patient’s mother was in our office, she stated that she had no idea that her child had no coverage in our office We asked her if she had received her EOB’s (Explanation of Benefits) in the mailShe stated no. During our phone conversation with the insurance company, the insurance company employee stated that yes, she had indeed been sent EOB’sThe patient’s mother asked why our office did not notify her that her insurance had not paidWe told her because of the volume of insurance claims we submit as a courtesy for our patients, we are not able to contact every patient to see if they received and read their EOB’s. Because of this unique situation we told the patient’s mother that she could make payments, whatever amount is suitable for her, for as long as she needs. Dr*** and Youthful Smiles have maintained an A+ rating by Better Business for over years
