in regards to complaint [redacted], the...

patient was charged on a HSA card for a deductible amount from date
of service 12/03/2013. The insurance processed claim on 12/07/2013, no payment was made from insurance as patient
has a deductible that had not been met. We are under contact with [redacted] so adjustments were made, leaving a
balance of 80.22 the patient has advised did not receive statement from us or an explanation of benefits from insurance
and apparently was not familiar with her insurance deductible. The patients mother called us on 05/02/2014
and advised us of the situation, she was told at that time we would contact her insurance ([redacted]) as we
could not put the 80.22 back on her card due to the time period it was charged. We advised would call
and see what the best way to make sure they received the money back so that her card could be unfrozen. On 05/05
/2014 we contacted [redacted] and was told we could refund the patient and the patient could send the 80.22 back to the
Insurance. At this time we issued a refund check in the amount of 80.22 payable to the patient, [redacted]. Patient was
called to see if check should be mailed or if patient wanted to pick up the check at the clinic. Patient picked up check
from clinic on 05/07/2014. As a courtesy we adjusted off the balance of 80.22 since patients mother advised she
did not get a statement from us or an explanation of benefits from her insurance, as her insurance did not pay we
accepted $25.00 for the entire office visit.
