To Whom It May Concern,
This letter is in regards to case ID [redacted]. I have looked over [redacted]’s account and it does appear he is in Collections with CBS collections our collection agency for $ 59.32. The charges are stemming from dates of service 1/25/2016 for 13.20, 4/05/2016 for...
25.00 4/19/2016 For 52.50 for total of 90.73. These charges then were sent to collections on various dates, where cbs collections charges a 35% collection fee on charges. His total owed to the collection company is 122.76. I see we received payment from the collect company on 08/04/2016 for 13.60, and then we received another payment from Collection Company on 09/08/2016 for 24.74. For a total payment of 38.34. If you minus that from the original amount owed of 90.73 the total amount he still owed to our office not including the 35% he owes to collection agency is 52.39.
