Spoke to our Rebate Department and the Rebate was processed today. The customer will see the Check in 2 weeks. We did not ever see the rebate form come in. Spoke to the customer and let her know the outcome.

I am rejecting this response because: First and foremost I am a "her" and not a "him". Second, the doctor did sign off on the prescription and amendments; if they had bothered to read it (see attached) they would have seen this however they were very quick charging my credit card before this alleged problem with the Rx. And refunding my money within 3-10 days is also unsatisfactory. This matter can be considered closed because I don't believe anything more can be done but please be sure to show it as CLOSED/UNSATISFACTORY. Thank you for your efforts on my behalf.
