For whom we are a provider. Unfortunately [redacted] farms out their vision to many different companies, one which is [redacted] , that company we are exam only providers. When the patient came in we even called [redacted] and were given benefits. I even sent the original claim straight to [redacted] . It was not until that claim was denied and I called them again that we learned that her vision insurance was really [redacted] . Our office would intentionally mislead someone about insurance coverage, which I tried to explain to the patient more than once. I feel the fault if there has to be any is with [redacted] for not telling us the first time we called that the billing went through [redacted] . Had we know we would have advised the patient immediately before she had her eye exam. I am enclosing a copy of the billing showing the different time and companies billed. Complaint: [redacted] I am rejecting this response because: I did not have any issues with my eyes as the subsequent tests confirmedThe initial test was in errorI had to return for three more tests, which have shown no anomalyI am retired, and I have paid into Medicare all my working life, and still payingMedicare is my primary provider, as it it for most all people who are retired [redacted] insurance is my supplementary to my Medicare insurance I have asked the provider to make a copy of my medicare card, which she didMedicare rejected my claim, stating that with this claim I've met my deductibleThe only reason I've decided to file a Medicare claim, because during the exam, the examining doctor (DrDavid MB [redacted] ) told me that they would pay most all of itThe initial cost was around $90, and because I've turned it into Medicare as advised by the examining doctor they have charged me $In the past 8-years I have spent over a $for glasses and examination with this provider, and I am sorry that I did Regards, [redacted]

provided to the business that I am covered by ***, all of my information only shows *** ***My insurance ID card states 'It is the providers responsibility to verify eligibility before providing treatment'I personally had no way of knowing that *** was involved at all, therefore I could not have let the provider know*** *** has informed me that when then provider calls to confirm my coverage, they would have been informed that Spectrera was involvedI do not think the business fulfilled their responsibility in confirming that I was a covered patientI think the business do not confirm my coverage properly PRIOR to providing coverage, as is their responsibility, not mine, as the consumerThis case is similar to going to a doctor and being told your are covered under you insurance and then being told after the fact that the doctor's office did not know exactly what coverage you had and then expecting the patient to cover the costs in fullThe provider, Associates in Family Eyecare would have been informed by *** *** of my coverage if they had called to confirm coverage, which I do not believe they did until after services were providedI feel I should have been given the opportunity to cancel the order for glasses once they found that I would not be covered*** *** customer service can be reached at ***

The business response indicates that [redacted] 'farms out' there clients to other providers, in this, case I was assigned to [redacted]. I have no indication on my insurance information, including my insurance...
id card that I provided to the business that I am covered by [redacted], all of my information only shows [redacted]. My insurance ID card states 'It is the providers responsibility to verify eligibility before providing treatment'. I personally had no way of knowing that [redacted] was involved at all, therefore I could not have let the provider know. [redacted] has informed me that when then provider calls to confirm my coverage, they would have been informed that Spectrera was involved. I do not think the business fulfilled their responsibility in confirming that I was a covered patient. I think the business do not confirm my coverage properly PRIOR to providing coverage, as is their responsibility, not mine, as the consumer.
This case is similar to going to a doctor and being told your are covered under you insurance and then being told after the fact that the doctor's office did not know exactly what coverage you had and then expecting the patient to cover the costs in full.
The provider, Associates in Family Eyecare would have been informed by [redacted] of my coverage if they had called to confirm coverage, which I do not believe they did until after services were provided. I feel I should have been given the opportunity to cancel the order for glasses once they found that I would not be covered.

I am rejecting this response because: I did not have any issues with my eyes as the subsequent 3 tests confirmed. The initial test was in error. I had to return for three more tests, which have shown no anomaly. I am retired, and I have paid into Medicare all my working life,...
and still paying. Medicare is my primary provider, as it it for most all people who are retired. [redacted] insurance is my supplementary to my Medicare insurance. I have asked the provider to make a copy of my medicare card, which she did. Medicare rejected my claim, stating that with this claim I've met my deductible. The only reason I've decided to file a Medicare claim, because during the exam, the examining doctor (Dr. David M. B[redacted]) told me that they would pay most all of it. The initial cost was around $90, and because I've turned it into Medicare as advised by the examining doctor they have charged me $170.00. In the past 8-10 years I have spent over a $1000 for glasses and examination with this provider, and I am sorry that I did.
