[redacted]I accept the letter as being correct in terms of what has transpired.  I do not yet consider the situation resolved.  Total payment of the account as turned over to Audit and Adjustment will be necessary to resolve the case.  A complete resolution would include a letter to me from Audit and Adjustment that this was a result of an error on the part of the Sound Physicians, Inc. and should never have been considered delinquent in the first place. [redacted]

documentation I requested to be able to pay the bill. I did fax the completed  form to David F[redacted] at Audit  & Adjustment on 7/31 that he requested and mailing a copy to Audit & Adjustment and also a copy to you at Revdex.com.  I'm more than willing to pay the medical invoice for $130 but requiring the documentation to make the payment for our records.Sincerely,[redacted]

We mailed the requested information out to Mr [redacted] on 10/12/15 and it was returned by [redacted] due to a mail return address.  We are re mailing the documents requested. if you need further assistance please do not hesitate to contact us.

the vendor has had no problem mailing and delivering all of the collection letters I have received.
I have  copies of the September 21, 2015 and October 15, 2015 collection letters that had my correct address of  "[redacted]" 
The vendor should re send  the details I requested to prove the services and billing denials by my insurance company using my address. Since I am disabled and retired I receive my mail daily.  Feel free to use [redacted] or [redacted] or [redacted] next time. Sincerely,[redacted]
