Hello-
I have worked with our payroll department to get a new W2 sent out to Ms. [redacted].  A new W2 was sent to [redacted]. Reynoldsburg, Oh  43068 on March 2, 2015.  Additionally, the payroll manager called the phone number listed and received a message stating that the voicemail...

was full.  My hope is that Ms. [redacted] has received her W2 at this time.  She can also contact the payroll manager, [redacted], directly at ###-###-####.

I have a letter from the Chimney Sweep LLC Menasha Wi who put a liner in the chimney and tuck pointed it April of 2014.  They came out again 10/10/16 looked inside  the attic and outside the chimney to make sure it was NOT the chimney, and I have a letter from them stating so. see attached. The roof also leaks into the ceiling in one bedroom upstairs. see my attempts                                    ... I have taken pictures of the wood around the entire chimney INSIDE my ATTIC to show you that, only after a snow, the ROOF leaks around the chimney IN MY ATTIC.(no snow around OUTSIDE of the chimney, no leaking). PLEASE, you got paid and I want the leaking to stop. I have also taken pictures of the chimney outside to show you there is no visible flashing around the chimney. I need it fixed within the next week. We have at least 2-3 more months of snow and more damage is being done to the wooden support beams,  my attic floor, and the chimney bricks in the attic.   Sincerely, [redacted] [redacted]

Upon review, I have found that the manager of this location did send a request to our corporate office to replace the cost of a family meal $20.00.  Our Guest Relations team did mail out these gift certificates with an apology from the restaurant on 5/20/2015.  As the guest is requesting a...

full refund of the family meal we will be mailing an additional $30 in gift certificates that can be used at any of our restaurants.  We will also call the guest personally to let them know of our resolution.
