On 09/01/2013 a General Liability policy was issued for [redacted] by BOLT Insurance Agency. Coverage was contingent on the insured passing an inspection by the insurance company. On 01/06/2014 the insured received the inspection report which cited a...

request for more information on the insured location, as well as for State licensing to be provided. On 02/04/2014 Mrs. [redacted] received a second request to comply with the inspection requirements in order to maintain active coverage. The first and second notice for underwriting information were never answered. On 03/14/2014 coverage was cancelled, notice was sent to the insured citing “failure to provide underwriting information” as the reason for cancellation. On 04/08/2014 a service representative from BOLT Insurance Agency spoke with Mr. [redacted] and discussed the cancellation. Mr. [redacted] was asked to provide the requested underwriting information in order to have the policy reviewed for reinstatement. On 04/14/2014 Mr. [redacted] advised BOLT Insurance Agency they would be unable to comply with the inspection requirements. On 04/16/2014 BOLT Insurance Agency received notice from the carrier that the reinstatement request was denied, as inspection requirements were not met. A manager from BOLT Insurance Agency reached out to Mr. and Mrs. [redacted] to explain the decision and offer a rewrite of coverage once the business can comply with carrier requirements. At this time Mr. [redacted] was charged for coverage from 09/01/2013-03/14/2014, a refund check in the amount of $54.00 was issued to the insured on 03/08/2014.
