We regret that Ms [redacted] has not accepted our explanationWe have addressed this issue with her, including a letter dated December 13, A copy of the policy has been provided to herThe nonforfeiture option of extended term insurance is contractual and does not require owner consent to take effectThe policy contract states: If a premium is not paid by the end of its grace period, the policy will lapse as of the premium due date If the policy has a net cash value when it lapses, you may surrender it for its cash or you may continue it as extended insuranceThe net cash value, which is the value remaining after the loan is repaid from the cash value, plus any divided value, will be used as a net single premium to buy extended insurance Extended term insurance is a level amount of term life insurance that begins on the date of lapseIt continues in force for a limited timeIt is payable if the insured dies before the time runs out Ms [redacted] has the option of keeping the term coverage until it ceases on November 29, 2026, or surrendering the policy for the cash surrender valueThis amount decreases over time

Because I was not notified of this loan untill after signing papers to take on my policyI was never notified that my insurance had lapsed and that it was set to a term life policy

Since business last responded consumer called Indianapolis office and asked what is needed to cancel a policy, they said you need to call the representative to and and the representative will submit a written form of cancellation for you.Per consumer, "I know that's what I did and the representative at the time remembers me doing that, his name is Everet F*I told Mary *** to call him I have his cell phone number, and she goes I'll call him well obviously she didn'tI told the lady you are saying if I would have died premium would have gotten paid lets go back months and you pay me that back it went from $to $whatever it was ridiculous when I found out, if they want to pay me months of high premium, so they said one month of the high premiumI would be satisfied if they would just give me back of when it went to the high premium, roughly about or months."

[redacted] requested documentation showing where her Dad ( [redacted] ) canceled and renewed the policies [redacted] did not submit a request to cancel the policies; the premiums simply were not paidAs outlined in the policy contract, when the premium is not paid, the policy will end when the cash value is less than the monthly chargeBoth policies automatically canceled at the end of the grace period when the due premiums were not paid [redacted] began the reinstatement process for one of the policiesHowever, when advised we would need to interview [redacted] , she withdrew her requestAs such, we do not have any documentation of [redacted] renewing his policies

There representative was told to cancel the policyWhich he says he did, Mary *** said because he no longer worked there he was not a credible source to make a statement, this is just an excuse not to make good on the full refundFurthermore she stated that had I died they would have paid out benefits, this could not have been done, had I died, no one would have known to contact them for premiums, because as far as we all knew the policy was cancelled

We apologize if it was not communicated to [redacted] that any quote provided for the rating change was based upon approval of the policy change request by our underwritersOnly underwriters can determine the rating a client’s current health qualifies for and approve a rating adjustmentUnfortunately, [redacted] ’s current health does not qualify the policy for a rating adjustment

We regret [redacted] ’ concern about mailings she received She has been contacted by one of our representatives requesting she send us a copy of one of the mailings to review Once we receive the copy, we will address the issue with [redacted]

We would like to apologize to [redacted] for the difficulty she experienced when she requested to withdraw funds from the annuity for her daughterThe funds were wired to her bank on January 20, 2016, as requested
