Please clarify.Tell us why here...The category Mammogram Screening / Pap Smear would not pertain to me as I am a male individualI have looked over my copy of the form that I filled out and submitted to them and no where on that paperwork does it say anything about a Mammogram Screening / Pap SmearIt clearly states that that I submitted their form that they sent me that I had a heart attack condition which resulted in quadruple bypass surgery.On the check for $dollars it stated there that I was only receiving that amount because my mammogram screening / pap smear only qualified for a 25% payoutAgain I am a male individual so that would not pertain to me and also it was clearly stated on the paperwork that was sent to them that it was for a hart attack which resulted in bypass surgery.Again I am not sure who they came up with mammogram screening / pap smearThanks, [redacted] ***

We regret the form sent to the owner has not been received Another form has been sent for the owner to authorize a withdrawal request As soon as the signed form is returned, it will have our prompt attention

We regret that Mr. [redacted] *** is not satisfied. As stated in his policy contract, the Disability Premium Credit Rider Benefit that Mr. [redacted] has in his policy provides a monthly benefit equal to one-twelfth of the annual selected premium. Therefore, as long as he remains permanently... disabled, a total of $27.75 ($30 minus the premium charge of $2.25) will be added to his cash value each month by the Disability Premium Credit Rider Benefit. Mr. [redacted] is responsible for the monthly insurance cost beyond that.

Revdex.com: I have reviewed the response made by the business in reference to complaint ID [redacted] , and have determined that this does not resolve my complaint For your reference, details of the offer I reviewed appear below I do not accept because they may say that they have mailed these documents but I was never informed of the policy lapsing and I was also not informed of the loan until after taking this policy over and I'm sure that when I assumed responsibility for this policy I was and had I know about the loan I would have not taken over the policy and I thought that when I called in to have the policy canceled because I could not afford the loan that was on the policy that it had took all the money that was in the policy that is why I never received anything I had no idea or notice of it becoming a term life policy and feel that I should have then been notified or had to consent to this being done and if that is not the case then the company has lost all the money that this policy occurred and my policy was a dollar policy and was in affect for years and the only cash value it has occurred is dollars worth that is almost Impossible [To assist us in bringing this matter to a close, we would like to know your view on the matter.] Regards, [redacted]

An insurance company that cannot show documentation? I expect my all premiums paid from 2012-to be refunded.If legal action is taken, I doubt "no documentation" would help in this dispute.*** ***

We regret *** *** * *** did not accept our explanationThe premiums paid provided coverage on his life throughout the years and cannot be refunded. Additionally, in June 2016, we sent him a letter to informing him of the scheduled withdrawal dates for the upcoming year. That was yet another notification sent to *** *** to bring to his attention there were monthly deductions being withdrawn*** *** has mentioned this issue with regulators, and we will address this matter with them as well

Ms*** *** automatically became owner of this policy when she turned 21. As the new owner, she assumed all rights to the policy, along with the cash values and the outstanding loanThe loan was $580.42, including interest, when Ms*** chose to allow the policy to lapse in
When the policy lapsed, the loan was deducted from the cash value and the remaining amount was used as a single premium to purchase paid up term death benefit coverage of $9,until November 29, Ms*** may choose to keep the extended term death benefit coverage of $9,or cash in the policy for the remaining cash value, which is approximately $172.20. The cash value will continue to decrease, as it provides the coverage and will deplete in November

We are sorry about the death of one of our insured, Mr*** ***, and offer our condolences to Ms*** *** and her family. We regret the delay in paying the claim and understand the difficult time the family is going through. As soon as the requested documents are received, we
will pay the claim. Mr*** was owner of his insurance, and the named beneficiary pre-deceased him. Therefore, our obligation is to the estate of Mr***. We have reached out to his family, in the person of his aunt Ms***, advising she should contact the probate court to determine and obtain the proper documentation to pay this claimWe are waiting for the documents directing our company as to who should be paid the death benefit. Although Ms*** believed the ownership and beneficiary change forms were signed in 2013, we did not receive any paperwork for Mr***’s or for his sibling’s policies requesting these changesTo make the changes at this time for his brother, we sent him a form, as he owns his policy. His signature is required to process the changes. As soon as he returns the signed documents, we can make the changes

We received the necessary documents, and the death settlement benefit was paid to MsAnn *** on April 6,

We have been notified by *** *** *** that she and her sister received the death claim checks
