This bill is not an ambulance bill.  Tristate Emergency Physicians are the physicians who treat patients in the Emergency Department, and their bill is separate from the hospital’s charges.  Charity Care only applies to the hospital charges, however these physicians offer a 25% discount if the balance is paid in 1 payment.   This would bring your bill down to $492.

We have on several occasions tried to mail this consumer the requested information.  When we mail correspondence to the address on this complaint, the address gets updated  by NCOA to different address.  The mail sent to the NCOA provided address then gets returned to us...

as “undeliverable as addressed”. There is no way for us to bypass the NCOA, so the consumer would need to update his information with the postal service or provide another way for us to communicate with him.   If the consumer would like, we can fax or email (a personal, not work email address) the requested information.

After pulling my credit report I found a collection by this company for a bill from [redacted] Cable. This was for renters that were in my property but the cable was in my name. When I called to disconnect the service I paid the outstanding balance (nothing was past due because it had always been on automatic withdrawal). The renters were supposed to return the cable box but apparently never did and apparently [redacted] failed to disconnect when I called in the order. The property is in [redacted] and I live in [redacted] so I had no way of knowing this had not been done. [redacted] never informed me that a balance was still due or that the equipment had not been returned. If they mailed anything at all it was to the [redacted] property even though the billing was supposed to be sent to [redacted] address. When I found the collection I offered to pay this provided it was removed from my credit report but CBHV refuses to remove this item and will only show a 0 balance. I was NEVER informed of the original debt by anyone and have never had any notification from anyone that the debt was outstanding. This is the only negative item on my credit report and it is ridiculous that these people can get away with it.

Review: CBHV is trying to collect a debt for a medical group out of network on my insurance for a check I never received,failure to provide documentation and forwarded said debt while still trying to collect debt to another collection agency.CBHV never verified initial debt or said check to validate actual debt. We are in receipt of the above referenced complaint. We have obtained a copy of the billing statement from our client, Sleep Anesthesia Associates, and it is being forwarded it to [redacted]. Our client placed the account with us on 7/**/13 to collect the balance owed for services provided to [redacted]. Since they are an out of network provider with [redacted]'s

insurance, insurance payments made on the claim were sent directly to the policyholder ([redacted]). Sleep Anesthesia Associates would have no copies of cashed insurance checks. [redacted] would have to request that directly

from his insurance company, since he is the policyholder.

Our office has had no contact with [redacted], and has never received a request from him for verification of this debt.

As previously stated, an invoice from our client is being sent to [redacted] today. We trust the information provided in the invoice will provide all the information necessary for [redacted] to contact his insurance company

with any questions he has regarding how the claim was processed.

Review: I have been sick and visited doctor, the insurance was suppose to cover the bill, but some how it fell through cracks. I moved and found out later. I have paid the bill in full but the collection agency is not fulfilling it's commitment. They have failed to completely remove the collections from the three Credit Reporting Agnecy (Experian, Transunion, and Equifax). Because of that the account continuously showed up on my credit history and drops my credit score significantly from otherwise excellent history. I have asked them to please make a request to completely delete from the three agencies, but they have been indifferent to my legitimate requests. It is also a violation of my HIPPA compliant act. Review: While going through my credit reports I noticed this company had a collection in my report. It is [redacted] the amount of $1391 to [redacted]. I have disputed this many times and no response. I have mail out a validation letter to the company as well that they should have just received. This company needs to delete this account from my report as it is not mine. Review: I contacted collection bureau of Hudson valley inc.. when I tried to explain that the account filed under my credit bureau was incorrect. they would not give me information on the acct. [redacted] login was very rude and un cooperative when it came to giving me information. my concern was that this may be fraud and I needed the companies name and the person on file information so that I could file a police report and the company in dispute. all of a sudden she was tight lipped with no information to give me. she would only say that I should contact the credit bureaus. my main concern was identity theft. In this matter they were no help to help me resolve the fraud. they seem to be a scare to give me information as if they had committed an error on their part and wanted to just get rid of me without giving me any information at this point. when I would ask for the address on file she said she didn't have it. Which was false because no collection company takes on as account without all the persons information so that they can collect. how dumb can she be to insult my intelligence. this company is a fraud its amazing how this company put out false information ruining my credit and then would not take responsibility to help the innocent in this case me.Desired Settlement: I need them to take me off the credit bureau so that my credit score is not affected by their error. I would like some kind of recourse. these companies are allowed to ruin your credit without any type of penalty and that is just not fair to me and any other person caught in this mess.

We were unable to give to [redacted] the information we had because we would have been disclosing a consumer's information to a third party. We advised [redacted] to dispute the item via the credit bureau. The credit bureau sends us an electronic inquiry and we can then inform the credit bureau that [redacted]' s information does not match that of the consumer we have on file, and that the item should be deleted. As of July [redacted], we have advised the credit bureaus that the limited information provided to us by [redacted] does not match the information for the consumer we have in our records. He should find that the account has been deleted from his credit report.

Review: Collection showed up on my credit report experian ,transunion and equifax for a medical bill from tristate emergency Physicians .I contacted them yesterday they would not give me contact information for the original creditor

they do not have any information except my name not even correct social security number ,not even correct birth date or address I know factually that I have never been contractually obligated to pay a debt with this company because I have never had an account or received services from them. We do not have an account for this consumer. We believe the credit bureaus have taken information we sent them for a different consumer and placed it on [redacted]'s credit report in error. We are unable to delete this item, because as [redacted] stated in his/her complaint, we don't have his/her full name, date of birth, or social security number. We cannot send the credit bureaus information we do not have to delete an account for a consumer we don't have an account for. The consumer must dispute the item with the Credit Bureaus. They will contact us electronically and ask us to confirm whether or not our information matches this consumer's information. We then advise the credit bureaus that it is not, in fact, the same consumer. The credit bureaus will then delete the account from this consumer's credit report. Review: this company keeps calling and asking for a person that does not live here,I don't even know the person.they say that they will stop calling,but still call and ask for the same person over and over again.Desired Settlement: I am reporting them because they have not stopped calling after they have been told several times that they have the wrong number.

Review: I have tried on multiple occasions over the past six months to make a payment to a medical bill that this agency has on me (account number [redacted]) and have been told (rudely, and on most occasions involved the person screaming unprofessionally) multiple times I the amount I wish to pay is "unreasonable" and that "they expect me to pay more." I have sent checks in via mail with my account number and a note enclosed that I wish to make a payment, and none of them have been cashed or returned.Desired Settlement: If this company is unwilling to take payments from me in an amount that I can reasonably pay, I would like the company to kick the bill back to the hospital so I may set up payments with them as I have done on my other medical bill through this hospital. I also request this be taken off my credit reports as I have tried, again, on multiple occasions to pay this before it was put on my report however was denied from being able to pay my bill.

I have tried to mail checks to this company as stated before, and they have not been cashed nor returned. I have again tried to make a payment via phone yesterday, 6/*/14, and was again screamed at and told the amount I am trying to pay is "unreasonable." I again ask if this company is unwilling to take my payment, no matter how "pretty" their response to my complaint is, that they kick the bill back to the hospital and take this off my credit report. I am not going to continue to attempt to make my payment only to be screamed at or have my checks held on to and then told "they were never received" even though I received the certified return letter stating they were signed for. These are terrible business practices, even for a collections agency.

Perhaps [redacted] sent his correspondence and had recent conversations with the other agency representing [redacted]. We have noted [redacted]'s account that we will accept his $10 payments. I went through all my records and last time I was in the hospital was in 2012 and everything was covered. I did not go to emergency room in 2013 at all.I had Unitedhealthace,still do and my emergency is $200. Review: I authorized a one time payment on a debt collection that this company was in charge of. We have refunded [redacted]' card for the initial payment authorized in the amount of $112.07. We have not received any documents to show how his declined transaction of 2/[redacted]/14 caused any overdraft fees to [redacted]' account.

Review: I have paid this company in full on behalf of the [redacted]. I got a letter via mail stating that balance is paid in full but the [redacted] is still showing a balance & has reported to all 3 credit bureaus that the balance still exist. My credit score dropped 57 pts on yesterday because the collection company has failed to notify the [redacted]. I talked an [redacted] for the [redacted] and discovered that the account has been SOLD to Hudson Valley. It's Hudson's Valley job to report account has been paid in full on behalf of the [redacted] to ALL 3 major credit bureaus. I even submitted the letter from Hudson to all 3 bureaus & the [redacted] reported back to the Credit bureaus that I STILL OWE them the amount. I don't know what's going on but I have been done in & I did like they asked me too.Desired Settlement: I am asking for removal of the [redacted] from all 3 credit bureaus since I have paid in FULL to Credit Bureau of Hudson Valley. They bought the account. It's their job to report my payments.

Review: I have contacted this company for approximately the last six months telling them that the account #[redacted] does not belong to me, I have further stated to them that this account is not mine and that I have requested a debt validation in accordance with the Federal Rules of Civil Procedure #1.130. To no avail, this company has reported this fraudulent account on my credit file for the past six months and have not validated this fraudulent debt. I have tried to resolve this matter with less confusion as possible and the company has blatantly ignored me and continued to practice unethical tactics in order to get me to pay accounts that are not mine. Review: We were recently contacted by Jeff B[redacted] reportedly a collection agent from Collection Bureau of Hudson Valley . Mr B[redacted] stated he was trying to find [redacted], My wife. He called in the late evening of November the [redacted], 2015. My wife answered his questions as well as she could, but Mr B[redacted] was very ill and abusive towards my wife. He got her to state not only her SSN but mine as well before my wife had realized that we had never done business with the company who Mr B[redacted] had stated he was trying to collect for. I was at work at the time of the call, so I called Mr B[redacted] the very next morning. Mr B[redacted] at first talked calmly to me but as I tried to get information about what he supposedly was collecting for he became evasive. I got an account Number for the claimant, [redacted], [redacted], and called [redacted] and asked them what it was for. They do not know what the account number is and had never done business with me or my wife before. The Claimant has not reported any credit history information on equifax nor transunion either. I fear this was a scam or a misdirection attempt.Desired Settlement: I would like to be cleared of any accusation by the Credit Collection agency involved, reassurance that my or my wife's personal information and SSN will not be used in any actions by this Collection agency and Mr B[redacted] needs to be negatively cited for his actions.

[redacted] returned our call on 11/**/15. She told the representative she was speaking to, Bill Jordan, that she thought we were calling about her husband’s hospital bill. Our representative used Caller ID to locate the account in question. He advised [redacted] that we were calling to speak with her. He explained who the original creditor was and asked her to confirm the last 2 digits of her social security number. [redacted] then gave her full social security number. This information matched the information provided to us by our client, [redacted]. [redacted] then advised our representative that she did not recognize the creditor.

Any information provided to CBHV by the consumer is used solely for the purpose of debt collection.

We have requested a copy of the itemized invoice from [redacted], and will forward that to [redacted] as soon as we receive it.

CBHV does not report accounts to any credit bureaus for [redacted], since [redacted] does it’s own credit reporting. We believe they did not recognize their account number when Mr. Wallace contacted them after his phone call to our office with Jeff B[redacted] on 11/**/15 due to confusion regarding the account number ending in the letter O and not the number zero.

We will make no additional attempts to contact [redacted] other than to provide the invoice detailing the items purchased and payments made towards the purchase.
