Review: I called the insurance company that my daughter was on at the time in February 2014 ,which was her dads insurance, I asked if she could go to my dental office which was not a forward dental office. I was told no and that she could go to the list of forward dental location that they read of to me. I called the forward dental office at 8801 W National Avenue , West Allis WI

To see if she could come there the reception asked for the insurance information . She then set up a appointment for my daughter over the phone. When that day came I took my daughter to this location for teeth cleaning presented the card. They informed me that she had a few cavities that needed to be filled, we then set up another appointment date that we went to for fillings to be done. A week or two later the dental office called me stating that she came to the wrong office could I call her insurance and have them back date this bill to get it paid. Mind you I went there twice and gave insurance info over the phone before going there. I called insurance company they said no to paying bill she was suppose to go to office in Glendale Wi where she was assigned to. I stated that is not what I was told per someone name Jennifer that I first spoke to in the beginning of all this. They said each patient is assigned a dental office and that forward I should no that from the code on the card. I said I don't work for you all and I would have no way of knowing that. Forward dental then said after I reported this to them that they didn't realize that she was suppose to go to other office didn't pay attention. I said that I am not responsible for this bill you all made this error and mistake not me . I stated that they had this insurance information and should have know this. Forward dental receptionist said that she was sorry and that I have to pay for all the services she had at their office , and try calling insurance company back regarding issue or my ex husband job benefits dept and see if they can get the location changed to their so bill can get paid. Now I am being sent several bills from now another collection agency . First bill was in 3/28/2014 for $881.00 now increased to $1088.75 . I don't think it is fair that this is affecting me and that I'm not the parent at fault and they want me to pay for there mistake. They also called my home a few months later saying that she was due for another cleaning at the same office that shewas not assigned to. I called forward dental a few weeks later regarding this wanting to speak with office manager they said she would return my call she never did.Desired Settlement: My daughter had insurance to cover these procedures that was done , the only thing that they said I had to pay for which I did was sedation that the dentist used to keep her calm while doing The fillings in her mouth because she was scared of the needles used to num her up. So therefore I want to not have to pay this bill forward dental and the insurance company needs to work this situation out because both are not owning up to it and it is not affecting them it's affecting me.

Review: I went in for a root canal and was given an approximate amount. When I was finished on my last appointment I asked if I owed any more the receptionist said I was all payed up. After about 2-3 weeks I received a bill for an additional $380.40. I called to ask why I received another bill so high the billing department stated that the original amount was just an estimate. I cannot believe that this business does not have set amounts and that they cannot give more accurate information. I would like to see how they could have miscalculated such a difference in amounts.Desired Settlement: I would like to see this business drop this charge and maybe give a detailed statement before services are rendered in the future.

I visited several dental practices to get a estimate on how much it would cost to get some dental work done. When I went to the front desk I was treated very poorly by the receptionist. The dentist was nice but they quoted me a outrageous fee for the work they wanted to do. They also gave me a really hard time about getting my X-rays I brought with me back. I was given a prescription for a fluoride toothpaste which I would occasionally return to purchase. The office is out of my way so when I had some extra money I returned to buy several tubes of the toothpaste. Some psycho woman comes out from the back office yelling that I can not purchase that much- it was only several tubes- and creating a big scene. She acted like I was some kind of drug addict trying to make a big score. I asked her why she was making a big deal out of a purchase of tooth paste.
Flash forward to me returning to purchase two tubes of toothpaste after I ran out from my last purchase. This woman who I had my last horrible encounter comes walking into the patient waiting room of the Franklin, Wisconsin location and gives me a death stare and turns around and goes back to the patient treating area. I bought my two 4 oz tubes of toothpaste at a cost of $58.00 and returned home.
I work 2nd shift and got to bed at 2am in the morning. I got a call at 9:00 am in the morning from Forward Dental where they just hung up. They called back about 15 minutes later and told me that I could no longer purchase the toothpaste from them. I said really, it is just toothpaste. They treat me like I am some kind of drug addict looking for a score.
These people have some serious issues. They are very toxic and sad. I am sure their poor treatment of people loses the dentist a lot of business.
