Health Republic Insurance of New York (“Health Republic”) hereby responds to complaint ID ***, received on July **, by the Revdex.com (“Revdex.com”).In review, the member’s claim was received by Health Republic in January for dates of service 7/*/2014-12/**/In February and March 2015, the documents were received but could not be printed due to email formatting errorsIn April the member was contacted and informed of this, in which she emailed againThe claim was processed in May 2015, and the member was informed a check has been sentHowever the information provided was incorrectDue to a processing error, the payment processed against the wrong account. ***, received by the Revdex.com (“Revdex.com”), which Revdex.com relayed to the Respondent on December *, Health Republic confirmed that, on November **, 2014, we received the member’s initial binder cover payment to effectuate his policy for the month of December, 2014, however, due to a data management issue, which has since been addressed, the member’s file failed to trigger the necessary effectuation mechanism which would have resulted in the generation and issuing of member ID cards; for this Health Republic offers its sincerest apologyIn view of the fact that the grievance, as detailed by the member, is legitimate we have authorized a premium credit for the month of December, Accordingly, the member will not be required to pay the premium for the month of December and the premium payments that have been allocated against his account to date have been reallocated, such that the member is now paid through to January **, The member is partially paid through for February, 2015, but will be required to remit the outstanding balance of $to satisfy that month’s premium commitment in fullWe trust this solution addresses the member’s concernsAgain, Health Republic apologizes for the inconvenience experienced, as we continue to work toward a more accurate and timely transaction process to ensure these issues do not recurRespectfully, Health Republic Insurance of New York

Freelancers Health Service Corporation, d/b/a Health Republic Insurance of New York (the “Respondent”) hereby responds to the complaint filed by *** ***-***, (the “Complainant”) received by the Revdex.com (“Revdex.com”), which Revdex.com relayed to the Respondent on April **,
COMPLAINT FACTS
According to the complaint, on February *, 2014, the Complainant and her Husband enrolled for insurance coverage with Health Republic for an effective date of March *, 2014;
On February **, 2014, having not received the first Invoice to make a payment on account, the Complainant contacted a Member Service Representative (“MSR”) of Health Republic, and was informed that the Respondent could not locate her account, but that Health Republic would look into the issue;
On March *, 2014, the Invoice for March, 2014, arrived in the mail, and the Complainant made a payment on account, thereby effectuating her policy;
On March *, 2014, the payment on account of the first month’s premium cleared Health Republic’s account, and was consequently deducted from the Complainant’s account;
The Complainant was informed by a MSR that ID Cards would not be issued until the payment was clearedHowever, notwithstanding that the payment cleared the Complainant was still not issued ID Cards
As a direct result of Health Republic’s failure to issue ID Cards, the Complainant was unable to access the full spectrum of benefits guaranteed by her coverage;
The Complainant has made several attempts to resolve her concerns with Respondent’s MSRs, all without avail; and
As a result of the matters raised in paragraphs through hereof, the Complainant filed this complaint with the Revdex.com (“Revdex.com”)
RESPONSE
The following is a timeline describing the matter and the steps Respondent took to ensure satisfactory resolution and communication thereof
On April **, Respondent received the Complaint from *** *** (***, Revdex.com)
On April **, 2014, Respondent began its internal review and escalated this Complaint with Respondent’s *** *** team, which confirmed that member enrolled in the Essential Care Platinum plan, effective March *, 2014, through to termination on March **, 2014;
On April **, 2014, Health Republic’s Billing and Enrollment Team confirmed that the Complainant’s payment had been received and allocated against the Complainant’s account on March *, 2014;
On April **, 2014, Health Republic’s vendor-partner (POMCO), confirmed that the member’s ID Cards were mailed on March **, 2014, thereby confirming the Complainant’s allegation that she was unable to access benefits for the greater part of the month of March; and
On April **, 2014, Health Republic received confirmation from the Billing Department that Complainant’s full refund for the month of March was processed
