Review: Earlier this year I made an online payment using my checking account/routing information & I received a confirmation # for my payment. A few days later I was notified by my bank that because of a security breach at Target, & for my own safety, my credit/debit card had been deactivated. Why this would have affected my insurance payment I do not know but apparently my payment became unsuccessful. I contacted Health Republic & made my payment over the phone as soon as I became aware of the situation.

When I attempted to make my next monthly payment online I received a message stating that my checking account was invalid to make payments from. I spoke to an individual at Health Republic & he took my payment info over the phone, game me a confirmation # [redacted] & told me that I should be able to go back to using my checking account to make payments online within the next month or so as it was just a glitch in your system.

Today when I went online to Health Republic to attempt to make a payment I noticed that there was no record of my previous payment & that I now owed 2 months of payments as well as late fees. I called Health Republic again & asked what the issue was & this time was told that there was nothing that can be done re: paying my bills online. I asked if I could make a payment over the phone & was told I could, so I did & was given the confirmation #[redacted].

In addition to the above issue, last months bills went out late & the monthly due date was supposed to be advanced moving forward yet there is no mention of that on my most recent statement & I was told by the woman I spoke to that my due date is still the [redacted] of each month which is what it was previously.Desired Settlement: I would like to have my checking account made valid again so that I can use it to make payments through via your website.

COMPLAINT SUMMARY

1. According to the member, he is a subscriber holding a policy of health insurance with Health Republic. He asserts that on an unspecified date during the current year, he made a payment against his policy using his checking account and received confirmation that the transaction was successful;

2. The member avers that at some point his Financial Institution (“Bank”) contacted him and informed him that his Debit/Credit card (“card”)was compromised as a result of a security breach unrelated to his transaction with Health Republic, and therefore his card was deactivated by the Bank;

3. The member asserts that as a result of the foregoing matters, Health Republic refused to accept any further payments from his checking account, though he is unclear how the foregoing detailed incident would affect his checking account status as a form of legal tender to satisfy his monthly premium obligations with Health Republic;

4. The member thereafter contacted Health Republic and made two (2) payments against his policy by telephone, which the member was assured were allocated against his policy;

5. According to the member, he was informed by Health Republic that there was a glitch in the system that was preventing it from accepting his checking account but that within a month, the situation would be remedied. Subsequent events have proven this statement misleading and/or false, as his checking account has yet to be readmitted as an acceptable form of tender by Health Republic;

6. Moreover, the member asserts that he checked his account status on-line only to find that his payments have hitherto failed to be applied against his policy; and

7. As a result of the matters raised in paragraphs 1 through 6 hereof, the member filed this complaint with the Revdex.com (“Revdex.com”).

RESPONSE

The following is a timeline describing the matter and the steps Respondent took to ensure satisfactory resolution and communication thereof.

1. On June **, 2014, after receiving this complaint from the Revdex.com, the matter was referred for investigation with the Respondent’s Billing and Enrollment Specialist team (“team”), who confirmed that on April *, 2014, the member’s premium payment against his policy was voided with an indicator notation, “account invalid”;

2. As a result of the foregoing, Health Republic’s system triggered an account restriction against the member’s account, thereby effectively preventing any further premium payments being deducted using the account number of record;

3. On June **, 2014, Health Republic removed the restriction against the member’s account, and advised him to verify his account before attempting to utilize it to satisfy his premium payments. The member was also advised that he was at liberty to use another account to satisfy his monthly obligations with Health Republic; and

4. Based on the contents of the foregoing, the Complainant’s matter has been satisfactorily addressed. However, should the member have any other concerns, it is suggested that he reach out to our Customer Service Department at our toll free number [redacted].

Review: To Whom it May Concern: At the end of November 2013 I signed up to particpate in the Obma care with Health Republic. I had called on numerous accounts to inquire as to why I had never received any documentation in the mail regarding my policy. (I have all dates and individuals written down. If I need to provide that info I can at a later date. I don't have it on me at this time) Each time I called I was told by each representive they have the correct address. Months later around the end of Feb I find out that had been a lie. Instead they had me living at one address and my mailing address was different. As a result I had sent a certified letter along wdith my 3/2014 monthly payment requesting they correct this matter immediately as well as to issue me a credit for the months of Jan and Feb as they had breached the HIPPA compliance law by making up a address of [redacted]. I do not know who lives there that has never been my living nor mailing address. Finally, on 3/**/2014 I was able to resolve the address issue. As a result of Health Republe errounous error they did comply with my request and issured me a 3 month credit of 687.36 stated on invoice number [redacted] dated 4/**/2014 which means I shouldn't owe them a dime for 3 months of service as I was issured a credit. However, on 04/**/2014 I discover a policy changed had been made to my acct without my consent suddenly I owe 458.24. It is now 04/**/2014 I have been trying since 04/**/2014 to get PROBLEM 2 resolved and of course I'v been unsucceful. On 04/**/2014 I spoke to a [redacted] who mentioned there had been a clitch in the system. On 04/**/2014 a [redacted] by the name of [redacted] spoke to me and indicated I shouldn't have to make any payment the issue is being worked on and I will receive a follow up call. I have yet to receive any calls and the problem is still lingers.Desired Settlement: I am requesting the credit that is due to me of 687.36. I shouldn't have to make any premium payments for 3 months due to the credit that had been issued. Someone needs to look into Health Repulic and perhaps put the out of business. They know how to take and run with you money (steal)but they are unalbe to resolov any issues.

COMPLAINT FACTS

1. According to the complaint, the Complainant enrolled with Health Republic for a policy of Insurance sometime in or about the end of November, 2013;

2. The Complainant states that she never received a welcome package, which prompted her to contact Health Republic for an explanation, at which point the Complainant was assured by Health Republic that her enrollment was active and that the Respondent’s record of her billing address mirrored her mailing address;

3. The Complainant alleges that sometime in or about February, 2014, she discovered that Health Republic did not in fact record her address correctly;

4. Based on the contents of paragraph 3 hereof, the Complainant registered a complaint with Health Republic and demanded redress for the breach (described by the Complainant as a HIPAA violation), by the Respondent.

5. According to the Complainant, the Respondent answered the Respondent’s complaint by issuing her a credit for three (3) months premium;

6. On April *, 2014, the Complainant discovered that an unauthorized policy change was made to her account, which gave rise to an account balance of $458.24;

7. The Complainant was informed by a CSR of the Respondent that there was a glitch in the system, which caused an error in her account balance;

8. The Complainant asserts that she has been attempting to have her billing issue resolved since April **, 2014, without success; and

9. As a result of the matters raised in paragraphs 1 through 5 hereof, the Complainant filed this complaint with the Revdex.com (“Revdex.com”).

RESPONSE

The following is a timeline with references to supporting documentation describing the matter and the steps Respondent took to ensure satisfactory resolution and communication thereof.

1. On April **, 2014 Respondent received the Complaint from [redacted]).

2. On April **, 2014, Respondent began its internal review and escalated this Complaint with the Enrollment Specialist team, which confirmed that member was enrolled for coverage effective, January *, 2014;

3. On April **, 2014, Health Republic’s Billing and Enrollment Team confirmed that the history of the Complainant’s coverage revealed that, the file received from New York State of Health (NYSOH hereinafter the “Exchange”) contained both a physical address and a billing address, the billing address appeared to be a transposition of the numbers on the numerical portion of the physical address;

4. The file that was sent by the Exchange contains information physically inputted and supplied by the Complainant at the point that she filled out the Enrollment Form on the Exchange web-site. It is this information which was used by the Exchange to create a file which was then sent to the Respondent to initiate and complete the Complainant’s enrollment;

5. A further investigation into the Complainant’s matter, confirmed that due to the error, ostensibly produced and/or authored by the Complainant, two (2) Invoices were sent by Health Republic to the said address. Health Republic can confirm that in March of 2014, it adopted a new policy of manually changing addresses that were faulty, irrespective of the origin of the fault, and which could result in bills being sent to an incorrect address upon receiving authorization to correct same from its subscriber. Consequently, the Complainant’s address was corrected on March *, 2014, pursuant to her direction;

6. As of May *, 2014, the Complainant’s physical and billing addresses correspond with the information provided to the Respondent by the Complainant;

7. On May *, 2014, the Respondent authorized its Billing and Enrollment team to process abatements for the months of: January, February and March, 2014. Accordingly, the Complainant will not be required to make any further premiums payment until sometime in or about June **, 2014, to be allocated against the month of July, 2014;

8. Let the record reflect that the abatements were not granted to the Complainant as a result of any HIPAA violation, as Health Republic’s risk assessment revealed that there was none, but rather because a commitment was made by one of Health Republic’s CSR’s, clothed with apparent and inherent authority, though no actual authority to approve the abatements. Consequently, Health Republic was bound, both legally and ethically to honor the commitment made by its representative; and

9. In view of the contents of paragraphs 3 through 8, the Complainant’s concerns have been thoroughly worked and resolved in satisfaction of the complaint.

Lets not turn the story around and make it my error. I don't appreciate that at all. Health Repulic chose to be apart of the exchange. Therefore they need to be held responsible for the incompentacey of anyone who had signed up individuals to take part in their health care package. It is not my problem that incompentant indivduals chose to change my address making one up that led to this whole mess in the first place. I'm happy with the fact that I will not have to pay any dues until the month of July. However, I want the facts states correctly if my complaint should go public. The facts are Health Repulic created two address in my favor that in no shape or form should of happened. They had a mailing address for me and a living address. I have ONLY ONE address and have been there for 9 years. In there response it was mentioned that the complaintent had provided the incorrect infomation upon signing up on the website. That is NOT SO!! First of I never signed up on the website. An individual had signed me up. Someone botched up my info. It had taken me from Nov 2013 - March * of 2014 to resolve that issure. For several months I had been told from numerous representive that they had the correct info and that was a lie. They never had my address correct. Any documentation they mailed had been going to the WRONG address due to there errounous error. That is unacceptable. If my complaint goes live the facts need to be stated correctly !!! Thasts all I want the truth to be known. Thank you

Review: We were going to cancel our insurance with this company in the fall. We contacted them and made them aware of this. A week or so later we changed our minds and contacted them again to tell them not to cancel the policy. Customer service said that we were all set and that it would not be cancelled. When I tried to go to the hospital I was told that our insurance had been dropped. Again, we contacted Health Republic and were told it would go into effect 1/*/15. We still have nothing and our account on their website shows that we are not currently under any plan. We have been in contact with Health Republic for months trying to get our ID cards and benefit booklet. We have called on numerous occasions and have been told we will get an email in 7-10 days. We have gone beyond 10 days and were told to wait again for 7 - 10 days. In the meantime, I cannot get prescriptions filled or make doctors appointments.Desired Settlement: We would like our coverage, benefit booklet and ID cards asap

Review: I had coverage from Health Republic starting May. However, starting October, I am actually under [redacted] due to income change.

Since I am no longer getting coverage starting Oct [redacted] from Health Republic, I called it to ask for my refund on Nov [redacted] 2014 since Health Republic never issued me a refund and the company has already charged me for Nov & Dec. The rep told me that she just put in a request for refund with a reference number and I will get my refund within 10-14 business days.

On [redacted] Dec [redacted] (the 13th business day), I called in again just check to see when I will get my refund. This rep told me that everything should be processed by [redacted] Dec [redacted].

Nothing happened.

Thus, I called on [redacted] Dec [redacted]. The first call happened at [redacted] and the rep told me she will need to take a deeper look into my situation and will get back to me within an hour.

Nothing happened again. I waited for 2 hours and 30 mins.

I called again at [redacted]. The rep of this time told me that the reference number I got the first time is not a request for refund. Now she gave me another reference number for my refund request. However, now I have to wait for another 30 days.

Health Republic keeps pushing back the deadline for my refund. It has been more than two months now. After this another 30 days of wait, it will be 3 months.Desired Settlement: Refund ASAP

After calling them more than 10 times. I finally got my refund. I am not happy with the way they deal with the issue. Very unprofessional? The customer reps are not well-trained. They don't even have the proper knowledge to help the customers which casued major delay for my refund.

I purchased health insurance from this company for my Dad in November 2014. By Dec 2014, he was informed that he got qualified for a Government subsided health insurance plan due to his most recent financial status. I contacted Health Republic around December **'s, 2014 every day via their online "Contact Us" service to cancel his account, but nobody got back to me. I finally spoke to a representative on Dec ** who told me to fax in a cancellation letter for my Dad and they would take care of it. I faxed them this letter that was confirmed on their end. I still received a bill for him on January 2015. Called them (waited at least half an hour) and was promised that they would take care and cancel his account. Got another bill in February. Called them again. Got a profuse apology and was promised that this problem would be take care immediately. Now, I got the March bill. Called them, but the waiting time was so long so I gave up. I have made at least 5 calls to their company and multiple emails to their Customer Service online but nothing has happened. I am extremely furious and tired about them. My Dad does not have this extra amount of money to spend every month. This was exactly what happened to me with [redacted] mobile service that I must called them every month for 6 months in a row but nothing fixed and I had to go to a [redacted] store and terminate my cell phone service with them immediately. Now, this company is illegally charging my Dad $405 every month, which I don't know when it will end.

Review: On 09/**/2015 I visited Medhattan Immediate Medical Care for health related issues. I was assured that the services were covered by my insurance company.

I have now received a bill from them in the amount of $799. They submitted a claim to Health Republic Insurance of New York four times but never received a response and are requesting me to pay the full amount. I am aware that the company winded down its operations but this happened before November [redacted], 2015. I have also called the insurance company at the beginning of December and I was not able to resolve the issue. I sent a letter and received no asnwer.

I am absolutely sure that the claim is legit, that I should not pay the full amount and that Health Republic had enough time to respond to the claim before winding down its operations.

I would like to know what to do since it seems impossible to get in touch with them now.Desired Settlement: I want to have my billing adjusted to the correct amount according to the portion that should have been paid by the insurance and the discount that should have been applied to the full service amount.

Review: I called health republic numerous times guarding the non-delivery of my insurance card. I paid for the new health insurance in November and received my insurance card a month later. I explained to the reps I am a cancer patient and that the many different doctors offices were requesting the actual card not just the "number" as quoted. Reps were not friendly nor did they provide any helpful information. The added on unnecessary stress to my current health situation! I also faxed a letter of authorization to Health Republic dated 12.**.2014 and I have a copy that the fax was successfully sent for my wife to speak on my behalf and handle any questions regarding the account including billing. The phone call made to Health Republic on 12.**.2014 no one knew anything about the authorization letter so my wife could not speak on my behalf. Also, my wife tried setting an account online as soon as we received the card and we received an error message stating the "date of birth" did not match. When I called Health Republic the rep asked a few questions to "confirm" I am the account holder and one of the questions was my DATE OF BIRTH. I told her and she "Ok yes thank you" meanwhile when I told her the issue she replies "the date of birth is wrong on your account" meanwhile I JUST CONFIRMED IT 5 MINUTES BEFORE HAND! So why respond "Ok yes thank you?" If she was doing her job she should have noted that it WAS NOT CORRECT!

I have had MANY ISSUES the past 30 DAYS with HEALTH REPUBLIC INSURANCE OF NEW YORK! I am VERY CONCERNED that this rep DID NOT actually verify the info on her screen accordingly. I would like to know what is going to be done to fix this? If these issues continue I am forced to shop with another insurance since MY HEALTH is on the line and the STRESS caused is un called for!Desired Settlement: I lost out on the month of DECEMBER in visiting doctors because HEALTH REPUBLIC FAILED to supply my insurance card within 3-5 DAYS as stated. I would like my bill to be adjusted accordingly for the stress and aggravation caused!

Health Republic confirmed that, on November **, 2014, we received the member’s initial binder cover payment to effectuate his policy for the month of December, 2014, however, due to a data management issue, which has since been addressed, the member’s file failed to trigger the necessary effectuation mechanism which would have resulted in the generation and issuing of member ID cards; for this Health Republic offers its sincerest apology. In view of the fact that the grievance, as detailed by the member, is legitimate we have authorized a premium credit for the month of December, 2014. Accordingly, the member will not be required to pay the premium for the month of December and the premium payments that have been allocated against his account to date have been reallocated, such that the member is now paid through to January **, 2015. The member is partially paid through for February, 2015, but will be required to remit the outstanding balance of $146.22 to satisfy that month’s premium commitment in full. We trust this solution addresses the member’s concerns. Review: On February [redacted], 2015 I signed up for health insurance through the [redacted] marketplace website set up to purchase private health insurance. The plan that was picked for me was Health Republic of New York and wouldn't be effective until March [redacted], 2015. They asked for a payment of $160.13 which I paid on February [redacted]. I ended up cancelling this coverage a few days later well before the March [redacted] 2015 date for the policy to begin and politely asked them for a refund since they provided no service or insurance coverage to me. To date, I have not heard back from them and am filing this complaint to expedite the process and get results.Desired Settlement: I am asking for a refund of $160.13.

3. The Complainant has since made his binder cover payment, and two (2) premiums payments to maintain his policy, for an aggregate of three (3) payments;

4. Contrary to what has been asserted by the member to date, he has accessed care under and pursuant to his policy of insurance with Health Republic a total of five (5) times, the first occurring on January **, 2014, and the last on February **, 2014;

5. On June **, 2014, The Respondent received confirmation from New York State of Health (NYSOH hereinafter the “Exchange) that the member called on several occasions complaining that he was deprived of the benefits of his policy, notwithstanding that he had made his premium payments timely. The Exchange assured the member that he presented as enrolled on the Exchanges database;

Review: I actually have problems related to customer service and billing as well.

As part of Obamacare, one must disenroll in a plan via the Health Exchange for their state by the end of the year. I disenrolled from Health Republic via the NY Health Exchange ("Exchange") on 12/**/14, to be effective on 1/*/15 (I have letters from the Exchange that evidence cancellation). Through serendipitous events, I found out on 1/**/15 that Health Republic still enrolled me in their plan and that they had no record that I cancelled their plan. I called the Exchange to confirm I enrolled in a new plan (that was not Health Republic) and received a confirmation number for my Health Republic cancellation. I called the Health Republic Billing Dept, gave them the cancellation number and was then told that they in fact did have a letter of cancellation and that it would be handled. Billing would send the information to Member Services, who would then disenroll me from the plan. The Health Republic rep did not know when the cancellation would go through because they were very busy with new enrollments.

Today (1/**/15), I called Health Republic to settle a separate billing matter and was told that I was still enrolled with them by the Exchange and there was no letter of cancellation sent to them by the Exchange. I again called the Exchange and confirmed that I was not enrolled with Health Republic. I called Health Republic again and told them that I am not enrolled and gave them the same confirmation number. I am told that the exchange sent them no letter saying I had cancelled my plan and that they will try to work on my cancellation as soon as possible. However, it was open enrollment time and that it was quite busy for them.

I have called the Health Exchange to complain as well.

This experience has troubled me deeply. Health Republic is concerned about getting more money from new enrollees than by the fact that I may be pursued for payment that I do not owe ( As of f 1/**/15, per my online Health Republic account, I was billed on 1/*/15 for $564.69). Reps have not told me the truth about my account either. I have spent too many hour thus far on the phone trying to handle this matter.

This type of business behavior is unethical and unacceptable.Desired Settlement: I would like a letter from Health Republic stating that my insurance plan has been cancelled (effective 1/*/15) and that I do owe a balance of $564.69.

Review: I have been dealing with the member services group of Health Republic since Dec 14' I have opened 3 trouble tickets, have made at least 8 phone calls and several emails in attempts to straighten coverage and billing issues for my family. Each time I speak to someone they continue to tell me that one or the other of my two children have been termed for coverage because of lack of fixing a communication problem between Health Republic and the NYS exchange. I have been told on two occasions that the problem was fixed and I would receive ID cards for my children. I have had joint conversation with both a Health Republic service member and a NYS exchange representative in the same phone conversation telling me it has been fixed. to date I have not received a card for one daughter and have now been told the other daughter is termed. Review: I enrolled for the Affordable Health Care act back in Oct/Nov of 2013 for coverage to begin under Health Republic in January 2014. Unfortunately, due to financial issues, I was unable to pay the bill on time to be covered on January [redacted] and I was ok with that. I received a letter from them in February saying that my coverage had been cancelled and would need to re-enroll if I would like future coverage. I went back onto the Affordable Care Act website and re-enrolled before the March deadline to avoid any penalties. Shortly after that I received a bill from Health Republic saying that I still owed them money from January and February even though I received a letter from them saying it was cancelled. I called them to address the problem and they said they had no record of that letter and according to their records I was still enrolled and never cancelled anything. I tried to ask how I could go about just cancelling the coverage for the months of January and February and they said that they could not help me and to call the Affordable act number and they could help with the cancellation. This did not seem right to me at all but I called them and they of course said that they could not help me and it was up to Health Republic regarding billing issues. I continued to pay by bills during all this time, minus the month of January and February. I finally sent an email to them and threatened a Revdex.com complaint if they could not resolve this issue. Review: In May of 2015, I terminated my coverage through the exchange for Medicaid. The exchange indicated that Health Republic will waive my balance and zero it out, due to the income difficulties. I spoke with Health Republic, and the representative said just that (Reference Number, [redacted]). The representative said the balance will be written off once she submits the request and they receive termination from the exchange. She also said I will receive a letter in the mail indicating this. Review: In December 2014 I called Health Republic inquiring how to cancel coverage as of 12/**/14. I was told to fax a letter with certain information and the cancellation would be processed. I asked the operator to verify that was all I had to do and I was assured it was. My January premiums had already been paid.

I followed their directions the next day. A few weeks later I received a bill for February. I called to say that I had cancelled coverage but they claimed that since I purchased insurance through the [redacted] that I would have to cancel through the state. In a conference call with the [redacted] and Health Republic the Health Republic representative kept insisting that the cancellation had to be processed through the state. The state representative insisted she had no idea what Health Republic was talking about since my coverage was through them. I had to stop payment on the February payment since they were going take the [redacted] transfer out. Finally the state issued an immediate cancellation to prevent Health Republic from containing this.

Now in February I receive a bill for the [redacted] rejection for a bill that should never have been processed.Desired Settlement: I would like an immediate refund of $271.36, my January premium, as well as a written apology from the company outlining the training plan they will use to prevent this from happening to other customers in the future.

Review: I recently became eligible and enrolled on my employer's health insurance, and needed to switch over. On February [redacted], 2014, I called Health republic Support number ###-###-####, to cancel my insurance with them. Their website has no option to do so - They would not let me cancel over the phone (if there is a reason for this I don't think it's ethical), instead they instructed me to send a signed fax to a number they provided to cancel. I did this the night of February [redacted], 2014.

On March [redacted] I had not heard anything from Health Republic. So I called in, the lady on the phone instructed me that they would create a case and look in to seeing if my fax had been received. On March [redacted] I had still not heard anything from them - So I called in again to try to make sure they had cancelled. This representative instructed me that because I had signed up through the health insurance market place I needed to cancel through the market place instead. Previously they told me I needed to send a fax.. On that date I removed my insurance from the marketplace in the best way I saw how.

Now come the [redacted] of March, I see a charge coming from Health Republic from my bank account. they had billed me for the next month (I assume April) already, Its possible I had singed up for autopay - however I had done everything in my power to cancel! So now I've paid for 2 months I should not have paid for.

Keep in mind the people on the phone told me not to worry, just not to pay my bills if they were unnecessary. Review: I was a member of health republic briefly while living in buffalo, new york, and subsequently moved to New Jersey at the end of January 2014. I have repeatedly received bills forwarded to my new address requesting that health coverage for february 2014 be paid. I informed Health Republic before my move of this, and I was told my plan was cancelled. Since then I have been receiving bills for the month of february 2014 ever since. I have phoned on two separate occasions to correct their records. Each time the employees assured me on the phone that I was correct, and that the bill was nullified, and yet each time I get another bill. I was assured during my latest phone call in august 2014 that management was informed and this issue was corrected. I have tried in good faith and with patience to resolve this, but I am very concerned that my credit score is being negatively impacted by this erroneous outstanding bill. I am writing this complaint because I just received a third bill requesting payment when I thought this issue was resolved. Health Republic Insurance of New York (“Health Republic”) hereby responds to complaint ID [redacted], received on May *, 2015, from the Revdex.com (“Revdex.com”). The member enrolled with Health Republic, through the New York State of Health (“Exchange”), effective, January *, 2014. The member states she requested termination of the policy in January 2014 due to relocation of the complainant’s residence from Buffalo, New York to New Jersey. Since enrollment and enrollment changes are done directly with New York State of Health we included review of their call history in our investigation. Neither Health Republic nor New York State of Health have record of phone calls from the member. However, Health Republic confirms that the complainant’s policy has been terminated retroactively to January **, 2014 and any outstanding balances have been removed from the policy.
