The patient is correct that the all porcelain (all white) crown will cost additional out of pocket expenses The crown that the patient currently has on (porcelain fused to metal crow) is what her insurance will cover Her insurance will not cover the all porcelain crown that she prefers the look of Thus, she will have to pay the out of pocket expense for the all porcelain crown if that is what she wants The out of pocket expense would not have changed had she decided on going the all porcelain crown route from the very beginning, before the porcelain fused to metal crown (crown covered by her insurance carrier) that was seated Therefore nothing will change with the expense, regardless of when the decision was made If the patient wants the all porcelain crown (all white) then we will gladly fabricate and seat the crown for her Unfortunately though she will have to pay the full fee for it as her insurance does not cover its cost In the end, Hudec Dental has provided the covered service that her insurance carrier allows for The patient's displeasure with the type of crown that was placed should be directed to her insurance carrier Sincerely,Hudec Dental

Good afternoon The patient had her teeth extracted in August of and then had dentures seated in November of The dentures were adjusted a month later in December of at no cost The patient returned five months later in May of and was treatment planned for a hard reline of $per arch, as her insurance carrier does not cover the cost of a hard reline There were then scheduled appointments in and the patient failed to be present for either appointmentAs with any full mouth extraction, a patient's bone mass will be reduced The reduction of bone mass is the reason the hard reline is necessary for the dentures to fit properly The fact that the dentures may not fit the patient is not the result of the work performed by Hudec Dental Unfortunately, bone loss is the reason for the ill-fitting appliance Hudec Dental, nor anyone else, can predict how much an individual's bone mass will change once their teeth are removed All Hudec Dental can do is to assist once the bone loss takes place (most commonly months after the teeth are extracted) Hudec Dental has not seen nor heard from the patient in over years We remain willing and able to assist the patient at a day and time that is convenient for her schedule Sincerely yours,Hudec Dental

We try to always do what it right for our patients at Hudec Dental Unfortunately this was a transaction from over years ago and at this point we can't do anything to waive or discount the balance Multiple attempts were made back in to collect the balance and if the patient did not want the products they should have been refused on the date of service or returned in a timely fashion We would never charge any patient for any service that was not rendered or any product that was not taken out of the office by the patient It is unfortunate that he did not use the products, but they are now expired so they cannot be returned Thank you

She was rescheduled for oral prophylaxis, due to unavailability of a dental hygienist on this date J.F.D***,DDS The patient was walked to the front desk at the end of her procedure laughing and smiling with the EFDA I asked her during check out how everything went and she responded with "good". I then scheduled the patient for her oral prophylaxisI asked her if there was anything else we could do for her and she responded with "no"I told her it was very nice meeting her and to please call us if she needs anything and to have a nice day, she replied with "you too" and left appearing content. The patient then came into the office and requested her x-rays and the ingredients in the Resin material we used for her fillingsI had the patient provide a picture ID as well as fill out the Records Release formI informed her that I was not able to locate the ingredients on the bottle, but did provide her with the brand name and type of Resin that was used, and she said, "that is fine, I can look it up with that". I asked the patient if she had any questions or concerns about her procedure and she replied with "no"I then asked her if she had any upcoming appointments and she replied with one being in MayI looked up the patients appointment date and provided it to her and asked her if she would like to keep her cleaning appointment and she said, "yes"I asked her if she would like an appointment card or a folder to put her X-rays in and she said, "No, I have one hereThank you." *** *** presented in the office on December 1, and was diagnosed needing a root canal, post and crown. *** *** was given a treatment plan detailing his co-pay, and informed we could get started with his work that day but he would be in the office several hours. *** ***
signed the treatment plan agreed to get started that day. He paid half of his co-pay at that visit and agreed to pay his balance at his next appointment. On 12/19/15, *** returned to have the crown seated, when he was asked to make his payment; *** informed us his wife had the credit card and said he would call the office with the with the credit card information when he arrived home. He then, mentioned that *** *** had to numb him a few times which was rough and he was here a few hours. He was reminded him he was aware of the length of his visit. *** *** never called us back to make his payment. *** *** received a billing statement each month starting in January and never called to make the final payment. On March 8th patient called and said crown felt "rough" and was brought in the same day to have the crown looked at and reminded of his balance. When he arrived at the office *** *** polished the crown and the patient was happy with the results. When he was checking out he was asked to pay his balance, he said, as he was laughing, that he should get a discount due to his time spent in the office and his unhappiness with *** *** again refusing to pay his balance. I reminded him again, he was aware he would have a lengthy appointment. I called *** *** on 4/4/and 4/12/and left messages regarding his outstanding balance. On 5/10/*** *** called me asking for a discount due to the inconvenience of his long appointment. He thought that he shouldn't have to pay for his services since he was not happyI informed him I would reach out to the regional manager and get back to him. The regional manager called *** *** on May 12th. *** *** explained his concern with the lengthy visit, felt that *** *** was rough and felt he should get some type of discount I apologized and confirmed that everything was fine with his dental work at this time. I did explain he has a PPO dental plan that provides him a significant discount and he is required to pay a copayment, which he had not done. I apologized he felt *** *** was rough, and confirmed he was aware he would have a lengthy initial appointment. *** *** became upset that that he wasn’t’ going to get a discount, and said he knows if we send the account to a collection agency they would take 30% so I should just give him the 30% discount and he would pay the bill. The regional manager apologized to *** *** and let him know we did provide the dental services and we wouldn’t be able to do that. At this time, we are happy to provide *** *** a gas card for the additional visit he had to make to our office to have the crown polished. *** *** - Office Manager

*** *** presented in the office on December 1, and was diagnosed needing a root canal, post and crown. *** *** was given a treatment plan detailing his co-pay, and informed we could get started with his work that day but he would be in the office several hours. *** ***
signed the treatment plan agreed to get started that day. He paid half of his co-pay at that visit and agreed to pay his balance at his next appointment. On 12/19/15, *** returned to have the crown seated, when he was asked to make his payment; *** informed us his wife had the credit card and said he would call the office with the with the credit card information when he arrived home. He then, mentioned that *** *** had to numb him a few times which was rough and he was here a few hours. He was reminded him he was aware of the length of his visit. *** *** never called us back to make his payment. *** *** received a billing statement each month starting in January and never called to make the final payment. On March 8th patient called and said crown felt "rough" and was brought in the same day to have the crown looked at and reminded of his balance. When he arrived at the office *** *** polished the crown and the patient was happy with the results. When he was checking out he was asked to pay his balance, he said, as he was laughing, that he should get a discount due to his time spent in the office and his unhappiness with *** *** again refusing to pay his balance. I reminded him again, he was aware he would have a lengthy appointment. I called *** *** on 4/4/and 4/12/and left messages regarding his outstanding balance. On 5/10/*** *** called me asking for a discount due to the inconvenience of his long appointment. He thought that he shouldn't have to pay for his services since he was not happyI informed him I would reach out to the regional manager and get back to him. The regional manager called *** *** on May 12th. *** *** explained his concern with the lengthy visit, felt that *** *** was rough and felt he should get some type of discount I apologized and confirmed that everything was fine with his dental work at this time. I did explain he has a PPO dental plan that provides him a significant discount and he is required to pay a copayment, which he had not done. I apologized he felt *** *** was rough, and confirmed he was aware he would have a lengthy initial appointment. *** *** became upset that that he wasn’t’ going to get a discount, and said he knows if we send the account to a collection agency they would take 30% so I should just give him the 30% discount and he would pay the bill. The regional manager apologized to *** *** and let him know we did provide the dental services and we wouldn’t be able to do that. At this time, we are happy to provide *** *** a gas card for the additional visit he had to make to our office to have the crown polished. *** *** - Office Manager

After seeing the complaint we investigated the charges and was able to find the receipt from this date which shows the agreed plan of 24m with Rhonda's signatureWe did call Care Credit to see if we would be able to switch this to a 12m interest deferred plan and was informed by their
representative Martha that it was not possible and that for this to take place it needed to be taken care of in the first daysWe attached a copy of her signed receipt for you to view
