It is not legalling binding for businesses to perform a service and then demand a different sort of payment that was never agreed upon, stating, "Well, you had it done; you owe us."Hudec has sent me a full bill. Ave. Cleveland, OH 44135 RE: Patient - Mr. [redacted] 4334 West 131st Street Cleveland, OH 44135 When [redacted] came in for his first appointment with us he did not have the time to go through the periodontal treatment that day, therefore he rescheduled for it. The patient presented with radiographic calculus interproximally (it is clearly visible on the x-rays taken on 5/22/15), localized vertical bone loss and generalized horizontal bone loss. The bone loss ranged from generalized slight and localized moderate with moderate periodontal progression on the mandibular anterior teeth. Probe depths ranged from 4-6mm during the time of treatment with heavy inflammation and bleeding tissues. Dr. G[redacted] agreed with this diagnosis upon full evaluation at the time of his visit. I received a denial on the claim shortly after the treatment, in which I wrote an appeal. After not hearing back from the insurance company for this patient, the office manager sent out a letter to request a peer-to-peer review and got an approval for a phone conference. The consulting dentist from [redacted] in charge of the claim was Dr. G[redacted]. I left multiple messages with him and we continued to play phone tag for weeks. He was also out of the office for some time. By the time that I did get a hold of him, Dr. G[redacted] stated that the claim is closed and the chart is off his desk and he cannot do anything to help the claim. I tried to battle this claim and do all that I could in favor of the patient to get the insurance to cover his treatment. I consulted with each dentist at the Puritas office upon this claim being closed and they all agreed that the patient would need periodontal treatment just from looking at the bitewing radiographs and panoramic image. We have documented the calls and messages in the DentalVision contact notes. Each time I received a call from [redacted], I would immediately call back once I was finished with a patient. I know the patient went for other opinions, but this was after the treatment was rendered and the debris was removed. There were then 2 scheduled appointments in 2014 and the patient failed to be present for either appointment. As with any full mouth extraction, a patient's bone mass will be reduced. The reduction of bone mass is the reason the hard reline is necessary for the dentures to fit properly. The fact that the dentures may not fit the patient is not the result of the work performed by Hudec Dental. Unfortunately, bone loss is the reason for the ill-fitting appliance. Hudec Dental, nor anyone else, can predict how much an individual's bone mass will change once their teeth are removed. All Hudec Dental can do is to assist once the bone loss takes place (most commonly 6 months after the teeth are extracted). Hudec Dental has not seen nor heard from the patient in over 2 years. The patient is correct that the all porcelain (all white) crown will cost additional out of pocket expenses. The crown that the patient currently has on (porcelain fused to metal crow) is what her insurance will cover. Her insurance will not cover the all porcelain crown that she prefers...
the look of. Thus, she will have to pay the out of pocket expense for the all porcelain crown if that is what she wants. The out of pocket expense would not have changed had she decided on going the all porcelain crown route from the very beginning, before the porcelain fused to metal crown (crown covered by her insurance carrier) that was seated. Therefore nothing will change with the expense, regardless of when the decision was made. If the patient wants the all porcelain crown (all white) then we will gladly fabricate and seat the crown for her. Unfortunately though she will have to pay the full fee for it as her insurance does not cover its cost. In the end, Hudec Dental has provided the covered service that her insurance carrier allows for. We are sorry to hear that the patient's onlay fell off. However that can and has happened. Every patient's anatomy, hygiene and eating habits are different. When something like this does happen we resolve the issue promptly for the patient by either re-cementing it or providing a...
new onlay. In this case though we were not afforded that opportunity by the patient. Since we did not see the patient, we cannot be expected to reimburse the patient for the new crown. If a crown was needed then we would have put the cost of the onlay towards the cost of the crown. But again we were not afforded that opportunity. We understand that the patient moved out of [redacted], however we have 18 neighborhood dental offices in Northeast Ohio and any one of those offices would have served the patient and resolved the issue. The patient's mailing address is Cleveland and we have 5 dental offices located within the City of Cleveland. We do feel for the patient and the inconvenience that occurred, but these things do occur. When given the opportunity by a patient, we always resolve the issue. In this example we were not provided an opportunity to correct the issue as we were informed after a crown was put on. As far as I see this issue I do in fact view it as an up charge that I was heavily persuaded to get a crown instead of initially getting my filling redone. Further more I was NEVER informed that there were different crowns and that this could have been a possibilitie. I think its clear that if I was informed of this being a possibility I would have stayed with getting my filling redone. So again I am completely unsatisfied with this issue being unresolved, the dental work that was performed and the extreme lack of communication prior to this procedure.

I received the medical records sent by Hudec Dental, with information about the products that were used on me that day. I did not, however, receive the apology due to me from anyone at Hudec Dental. I requested the names and professional titles of those connected with my treatment. Other than the dentist, all other names provided, were first names only- no last names were provided.This is unprofessional and unacceptable. There are parts of my medical records missing, that I should have by law received, but didn't. The parts that are hand written are nearly illegible. One page has been falsified, in that it is implied that what is writted was written by me, though it was not written by me. There are contradictions throughout. Though, most concerning to me are the blatant misrepresentations of truth that have been recorded as fact, in my medical records. This crosses a line and ends up in legal territory, in addition to Hudec Dental already having had a field day in trampling any semblance of ethical standards that should be expected from a medical services provider.

Please see the attached receipt/invoice from Hudec Dental. The total billed is $136. I paid $73 at the time of the visit. Hudec billed the insurance company $63.Hudec received $17 from the insurance company. Hudec then invoiced me for $93.I reviewed the information my insurance company posted on the insurance company's website. They stated I could be responsible for $54 so I paid $54 to Hudec. (upon contacting my insurance company they made a mistake and subsequently removed that explanation of benefits, they state Hudec can charge me whatever they wish for services and since the panoramic film wasn't covered they are not concerned with this situation)
Hudec still wants an additional $39. Hudec does not provide a clear invoice stating the total amount of money they wish to receive. I paid the additional $39 when I received a letter from them threatening to report this incident on my credit report but I wish to have Hudec's billing practices reported as deceptive.

I am not satisfied with the response because if they don't want to reimburse me for the crown then they should reimburse me (or my insurance company) the cost of the onlay. I would be embarrassed if I produced a product that did not even last a year when it should have lasted a lot longer than that and I would want to do everything possible to make the situation right for the customer. Seeing that when the onlay fell out, not only did it cost the me a thousand dollars but it also was very painful as my nerves were showing. I could not eat or drink and even feeling air on what was left of my tooth was painful. My priorities were to find the closest dental office that could get me in ASAP since I was in a lot pain. If they are ruthless enough to not back up their product than they should want to do something for the pain and suffering that I experienced. Its a travesty that this is even being discussed on the Revdex.com's site. Hudac dental should have rectified the situation before it got to this point. Its clear they do NOT care about the comfort or concern of their patients unless they come back to spend more money at one of their offices.

I have reviewed the response made by the business in reference to complaint ID [redacted], and have determined that this does not resolve my complaint. For your reference, details of the offer I reviewed appear below.
I am not satisfied with the response because if they don't want to reimburse me for the crown then they should reimburse me (or my insurance company) the cost of the onlay. I would be embarrassed if I produced a product that did not even last a year when it should have lasted a lot longer than that and I would want to do everything possible to make the situation right for the customer. Seeing that when the onlay fell out, not only did it cost the me a thousand dollars but it also was very painful as my nerves were showing. I could not eat or drink and even feeling air on what was left of my tooth was painful. My priorities were to find the closest dental office that could get me in ASAP since I was in a lot pain. If they are ruthless enough to not back up their product than they should want to do something for the pain and suffering that I experienced. Its a travesty that this is even being discussed on the Revdex.com's site. Hudac dental should have rectified the situation before it got to this point. Its clear they do NOT care about the comfort or concern of their patients unless they come back to spend more money at one of their offices.

The patient paid for rendered services using an [redacted] account. She notified us 2 months later that her [redacted] account had been locked and she wanted the payment refunded to her account. I let the patient know that we would be happy to refund the account when other payment arrangements were made. I worked...
with the patient on applying for our payment plan through [redacted] but the patient was not happy with her qualifying repayment terms. We are happy to refund the patient's account when payment arrangements that meet our financial policy are made.

We would never charge any patient for any service that was not rendered or any product that was not taken out of the office by the patient. when her insurance did not cover the x-ray claim. Attached is Aetna’s( her insurance carrier) Explanation of Benefits. Circled it states that the Patient’s Responsibility in this example is $93.00. Ms. [redacted] also would have received a copy of the attached EOB from Aetna. The patient’s bill is correct in that $97.00 is now due. The service was rendered and the insurance carrier did not pay for the claim. Thus, the entire financial responsibility of $93.00 for the rendered service is now due from the patient.
