There has been nothing done or said to correct this POOR process and excessive time needed for precertification They sent me the approval after weeks of asking for it They have not even attempted to justify there process or lack of dutie to the contract I have to pay monthly to a company that has no reason to fullfill there end of the contract
Allow me and people everywhere to move from an inferior health service provider AT ANY TIME the provider is: said by health professionals, "this is a terrible company", or refuse to respond to medical precertifcation requests for weeks, or require the patient and the medical provider to call and beg and plead to have the test or treatment done for weeks before someone even looks at the the request
The company should and probably will have a class action law suit against them

We contacted the member on 3/8/and advised that the procedure that she inquired about would be subject to her individual deductible of $2,and 35% coinsurance at *** *** HospitalThe member wanted an exact estimate of cost, which we explained would not be possible due to variations in reimbursement rates from provider to provider, as well as all of the individual procedure and diagnostic codes that go into determining how claims are adjudicatedThe member is able to contact the provider to ask about billed charges, and then she can estimate her cost using an approximate discount rate between 20% to 40% off of the billed charge amountUnfortunately, we are not able to disclose further information as discount rates are proprietary and are not shared with health plan members industry-wideBest Regards, Land of Lincoln Health

However, the member’s statements from the year come from what the *** has on file, not what LLH has on fileAs an on exchange member, Ms*** would have received a 1095-A form from the marketplace. Ms*** will need to file an appeal with the *** using the following email address: ***@cms.hhs.gov
As a *** enrollee, she needs to request a 1095A form via a HICS case.

I am rejecting this response because:  Land of Lincoln has still not addressed all of the items  from the original complaint, specifically the issue of hold times of 30-45 minutes.  We encountered hold times of 30-45 minutes several times because we needed to make several phone calls in order to clear up the wrong information which we were given by Land of Lincoln.  Hold times of 30-45 minutes put an unfair burden on the doctor's offices which are required to contact Land of Lincoln on behalf of their patients in order to server their patients.

The member’s request was received but cannot be approved because additional information is needed. LLH mailed a request for relevant medical records related to the member’s treatment and diagnosis on February 22, 2016, however, we have not received any correspondence back since that date. We have mailed out an additional copy of letter requesting this documentation. Once provided to LLH, we will be able to review the member's request. Best Regards, Land of Lincoln Health

Not sure about all the negative reviews, but my experience with this company has all been positive. This is the second year and signing up and receiving the ID cards went without a hitch. Claims have been paid according to the plan.

1.       Date of service 9/9/2015, billed charges of $243, claim ending in -[redacted] processed as in-network 2.       Date of service 9/23/2015, billed charges of $1,009, claim ending in [redacted], processed as out-of-network   Based on the information submitted by the provider, both claims paid out correctly with the provider’s associated Tax ID and National Provider ID numbers.   In order for the 9/9 date of service claim to be paid as preferred, the provider will need to submit an updated claim using his alternate Tax ID number ending in [redacted]   In order for the 9/23 date of service claim to be paid as preferred, the provider will need to submit an updated claim using his alternate Tax ID number ending in [redacted] with provider [redacted] not [redacted] Ltd.   If the provider is unwilling to make these changes, the member will need to file a verbal appeal by contacting LLH Member Services at [redacted]   Best Regards,   Land of Lincoln Health

episode of care. The hospital and doctor utilized were considered in-network providers; however, the laboratory was not a contracted provider with Land of Lincoln Health. As such, the laboratory services were originally processed accordingly.
Upon receiving a verbal complaint from the member in January, Land of Lincoln Health did review and reprocess the claims in question. The member has since requested additional review of claims incurred for a second episode of care which is currently under review.
We apologize for any inconvenience the member has experienced in receiving service from Land of Lincoln Health and are committed to resolving the member's issue.

Land of Lincoln Insurance stands by nothing they post in their materials. No accountability whatsoever. I was told my doc was covered in network and then 2 weeks later when I showed up for my appointment they told me I wasn't covered.
Why? My doctor would be in network if I drove to a location over an hour from my house, but if I wanted to see him at my local location-the one I'd been swing him at for 1.5 years, I would have to pay out of network. The issue according to the LOL rep. was that his billing address is from his Chicago location. Even though all claims go through the same office and under the same tax ID number, LOL told me to tell my physician that if he want his other location included for in-network, that he needs to update the profile on his website to include that location. So much for continuity of care.
They put all burden on their clients and the physicians they have contracts with.
Their website is a joke and looks like a 3 year old put it together.
Why would I need to put in a date to search for their providers list? Insane.
They are behind in posting claims and deny parents the right to access their children's claims without opening a separate account for them....ridiculous.
FYI: if you are an insurance company and you are a reputable business, you ensure that the information on the website that YOU put together is accurate and up to date.
The consumer is paying YOU. Either stand by your company or get out of the business.

THIS IS THE WORST COMPANY IN THE HISTORY OF TIME. I had to go to the ER April of 2015 and before I went, I checked to see if the nearest hospital was in network. It was. They doctor wasn't apparently which isn't something you think about when you are in and IN NETWORK facility. I was seen and then for a year afterwards, I was being bill by the doctor for $900. At least 6 people that I had talked to at LOL assured me that they will resubmit this claim and I shouldnt have to pay this and they will contact me. Then I got my FIRST collections letter. I called LOL and waited for days, literally days, to hear back from someone. They called the collections company and said they were still looking into it. I just got another collections letter. NEVER HEARD FROM LOL AGAIN. I am a single mother with a very limited income and can't afford this. I could have been making small payments and been done with this nightmare by now but the principle of the matter is, this insurance company SHOULD PAY for some of this claim.This company is a JOKE and has treated me worse than any business I have ever dealt with. I'm not expecting them to pay all of it but they owe more than $100.

I agree with all of the other reviews thus far. There is no EOB information available on their web portal. The accumulator information for our account is wrong. You can't get anyone on the telephone, you can only leave a message, and no one calls you back. They have sent me the exact same letter three times explaining that they would make a claim determination within 15 days. The first one arrived over 30 days ago and the most recent one today. They have yet to process the hospital claim for a colonoscopy that I had on January 13th with a preferred, in-network hospital. The hospital is now dunning me and asking me to get my health insurance company to cooperate in paying the claim. They won't allow you to fill generic or other prescriptions that were written by the doctor for a 90 days supply, so each time the pharmacy has to call LOLH to get the claim to go through. Worst customer service ever!
