As such, in the spirit of delivering best-in-class facilities, amenities and programming, we have implemented consistent membership guidelines to provide a uniform member experienceBy way of background, Life Time Fitness’ General Terms Agreement states that a member may terminate the membership for any reason by giving advance written notice. Advance written notice is described as submitting a termination request at a Life Time center or by sending a written termination request stating that the member is terminating their membership by certified mail, return receipt requested. We believe that it is in the best interest of our members to have a receipt of cancellation to prevent any possible miscommunication. Life Time does not accept cancellations via phone, fax or emailAdditionally, the General Terms Agreement states that all cancellation requires written termination notice, which requires a day advance written notice to terminate (e.g., written notice received by Life Time Fitness on January will terminate on the membership effective February 14). I have included a copy of Ms***’s General Terms Agreement for referenceUpon reviewing Ms***’s membership, Ms*** has told two Life Time Member Relations Representatives at our corporate office that she mailed a certified letter to cancel, but has yet to provide a copy of the certified receipt per their request. A certified letter is a type of special service mail offered by the United States Postal Service that allows the sender proof of the mailing. It also allows the receiver to track their package or envelope through the online system at usps.com using the unique tracking number provided by the mailerLife Time has no record of receiving Ms***’s certified letterWe suggest Ms*** reaches out to the postal service to get an additional copy of the certified receipt if she is unable to find the original copy. If Ms***’s can provide proof of certified receipt of her prior cancellation letter, Life Time is more than happy to readdress this complaintThe current balance on Ms***’s membership is $240.00. There are three ways Mr*** can pay her balance and they are as follows; in person at any Life Time facility, or, mailing in payment to the Old Orchard Life Time, or, by calling Member Relations at Life Time Fitness’ corporate office at ###-###-#### during the office hours of a.mand p.m., central time, Monday through FridayAbove all, we thank Ms*** for her patronage and allowing Life Time Fitness the opportunity to respond to his concernsTo this end, we trust that this matter will be considered closed by your office. Upon reviewing Ms***' membership, Life Time has record of Ms***' signing up for membership, signing the Member Usage Agreement, along with being provided a temporary membership card on December 13, Also a permanent membership card was also ordered on December 13, 2014, and mailed to the address she provided us within 7-business daysWe apologize if the letter with the membership card was overlooked by Ms*** or was lost in the U.SMail; however, the temporary membership card received at joining is sufficient for accessing the club
I have provided a Ms***' General Terms Agreement and Member Usage Agreement outing that she was aware of becoming a member with Life TimeMs*** also received an email at [email protected] with the General Terms Agreement and Member Usage Agreement which were completed on December 13, Again these were sent to Ms*** for her records outlining the terms of the membershipI have included these documents as well for reference

We reject this response for a few reasonsFirst of all, my wife came for the first time to the gym without infant appointment because this important fact was left out during the sales pitchIf we knew that appointment needs to be made, often multiple days in advance, this would be
deal breakerSecond, here are the options that were given to my wife regarding our 2-y old not being able reach the toiletOne option was to put a diaper onWell, if you knew what it takes to pottytrain a child, this would not even be an optionWhat kind of message do you send to a child that has no accidents elsewhere (day care, My Gym, other fitness centers, etc.) after she has been out of diapers/pullups for more than months alreadyMaybe you should try this brilliant option on yourselfAnother option, was for my wife to keep on checking on my 2-y old, during the stayBesides the obvious inconvenience where she had to run out for 10+ min in the middle of her yoga classe to check on our little one, again if you knew children, you would imagine that it’s not easy to leave the babysitting area again and explain to the child that she had to go back to the gymDespite this my wife in fact was checking on our 2-y old every half hour and that’s how she found our child on multiple occasions crying and wet in front of the bathroom because your staff would not open the door for herNow, the door to the bathroom is specifically designed that only a person taller than ft can open it (from inside)So not even our 4-y old, that you graciously allowed to babysit our toddler (and our infant on multiple occasions), was not able to open the bathroom door by herselfThen a question comes to mind, why are we paying close to $per month for you to babysit our kids, while we’re using your gym (and paying additional $200/mo for that), if my 4-y does the babysittingAll we were asking your staff to do was to open the door to the bathroom and to put a step stool there, so our 2-y can reach the toilet by herself as she does everywhere elseWe are currently trying a gym where babysitting costs $50/mo, without ridiculous infant appointments, and so far not a single accident in front of the closed bathroom door…

Leah informed him there would be no upcharge for future services, and will offer a complimentary skin care gift as his next facial. I thank Mr*** for another opportunity to address his concerns and apologize that he did not find our first response satisfactoryI want to apologize for the delay as I had to order paperwork to look into if a cancellation request was missed by Life TimeAfter further review of the paperwork from Overland Park, KS, Life Time has no record of a proper cancellation being submitted
I appreciate Mr*** providing me a copy of a membership change form, but that is not a cancellation form that is used at Life Time and it is not completeAs you can see on the form, the membership number is not complete or close to a membership number Life Time uses, the date is not complete, and the box is checked for a day money back cancellationAlso the date on the form looks to read, October 2014, however Mr*** contacted Life Time Member Relations on November 13, to update and make payment for November's past due balanceMr*** spoke with a Representative that day and never once mentioned that he had already terminated his membership, but rather asked how to cancel the membershipThe Life Time Representative explained to Mr*** that he would need to send a certified letter or cancel at a Life Time locationPer the facts that we have documented, Life Time respectfully declines to process a refundWe will however offer to waive the outstanding balance for June and July dues in the amount of $as a customer service gesture
On behalf of Life Time, I again apologize for any frustration regarding our policies and that the cancellation process did not meet Mr***' expectationsWhile I can empathize with Mr***' decision to cancel his membership, Life Time Fitness chooses to uphold the cancellation terms outlined in the General Terms Agreement to be fair and consistent to all membersI apologize that our decision to uphold the standard cancellation terms has resulted in a negative experience for Mr***

Upon reviewing Mr***'s membership, Life Time Fitness had no record of any cancellation request for his membershipLife Time does have documentation of speaking with Mr*** on November 12, 2014, when he returned our call about the past due balance owed for September through NovemberPrior to November 12, we had been trying to contact Mr*** via phone and email so we could assist in resolving the past due balanceOn that date Mr*** was offered to reduce his past due balance from $down to just $Mr*** took the offer by updating his form of payment on file, and was told he would need to respond to a Life Time Health Agreement, via email, so that Life Time could process the hold requestThe Life Time representative sent Mr*** the Life Time Health Agreement to respond to on November 12, as well as three more additional reminders prior to November so that the proper change could be submittedUnfortunately Mr*** did not uphold his end of the agreement to respond to the email which would have placed his membership on Life Time Health
Life Time has on multiple occasions explained the reasoning for the membership not being placed on hold, how to complete a proper cancellation, as well as the membership being past due once againMr*** has chosen not to complete the cancellation process that we have informed him to complete, and rather has opened disputes with his bankCurrently the balance owed for March through May dues remains outstanding in the amount of $If Mr*** would like to continue his membership, once again, Life Time would be able to offer to remove 50% of the balance owed with updated billing information

BOn OctX XXXX, I wrote an email to *** *** Member Services Department Head stating that, "I already canceled my membership and have moved to CaliforniaIf your records don't show that, then cancel my membership immediately."
My membership was canceled, per the Life Time Fitness General Terms Agreement, on November 12, Since November 12, Life Time Fitness has withdrawn $from my bank account prorated from November 12, My Life Time Fitness membership was canceled because of my written notice of termination on Oct6, 2014, the day notice required to cancel a membership and my balance of zero on November 12,
My e-mail notice of termination followed an e-mail from Ms*** dated September 23, In response to my notice of cancellation on Oct6, 2014, *** *** at Life Time Fitness e-mailed me on November 12, *** *** offered to "change (my) current club-accessing membership to a non club-accessing Life Time Health membership t Life Time Fitness, deactivating center access as of 12/1/14." And "If I agree with the Terms and Conditions below, PLEASE REPLY TO THIS EMAIL by stating 'Yes, would like to change my membership to a Life Time Health membership and I agree with the terms and conditions contained in this email.'" I did not respond to this email because I waned my membership canceled per my written notice on Oct6, I did not want to continue paying $a month for a non club-accessing membership
Life Time Fitness has communicated with me, you, and Band of America representatives by e-mailIs Life Time Fitness claiming my e-mail notice was not permitted? I have not received any US Mail communication regarding my complaintNor has Life Time Fitness replied to me by US Mail regarding my complaints with Bank of America or regarding this procedure with you and the Revdex.com

After reviewing Mrs***'s membership, I do show that the membership dues in Nevada are $for a Family membership per monthI understand Mrs***'s frustrating with having to upgrade the membership with Life Time to continue to have access to the two locations in Nevada; however, the market; demographics, and operating costs in Nevada warrant the higher feeI would be happy to assist with processing a refund for half of August membership dues as customer service gesture, which totals $The refund has been processed back to the Visa card, and will post as a credit to Ms***'s bank account in 3-business days

Dear Ms***, The email from the Aquatics Manager is not fabricated.Ms*** did not have her child checked into the Child Center prior to the start of the Kids Academy class at 5:30p. Additionally, as stated previously we are in the proper
