I have attached a copy of her card and her record from our system indicating all information in her file. email to [email protected] with your name, address and class date we'll get a new card sent out to you. I'll have to pull the hard copy of your file to verify your attendance, but it should arrive in about 10 days.
I can also make sure once it's printed that you receive a hard copy in your email.

I am very sorry that her email correspondence was not received by our office, but it has been more than a year since she experienced this problem. I have gone ahead and reprinted and mailed her card without a reprint charge and I do apologize but it is not our policy to issue free class credits under any circumstances.

I can no longer access this complaint for the file number, however I looked up [redacted] records here and her card was mailed out to her on 8/14/2014 - all known email addresses for her are returned to us as undeliverable and she does not return phone calls to confirm receipt of her...

When cards are printed an electronic copy is kept on file and the date of printing is recorded in two places, so I can confirm it was sent to her.
