Re[redacted]al response, October 10,2016: On September 15, 2016, the claim was reviewed again by Mr. [redacted]’s Examiner, Ms. [redacted]. At that time, it was discovered that the incorrect payment amount was issued. We immediately issued another payment in the amount of $220.30 that was owed to Mr. [redacted]. Ms. [redacted] tried to contact Mr. [redacted] by telephone, but was unsuccessful and left a voice mail message detailing the error and explaining the issuance of the additional amount. Additionally, she requested that he contact her directly with any questions and provided her contact information. I trust that this matter has been addressed to your satisfaction. 2015.My review of the information submitted to your office indicates the primary concern of the customer was our coveragedecision for the above referenced claim.  I have reviewed our claim file, and noted the following information from ourinspection of his home: Water stains in the master bedroom ceiling in the proximity where the plumbing vent pipeWater stains to the ceiling in the left rear bedroom cornerThe roof has areas that had been tarred previously, but appear to be leaking.   No evidence found of any sudden or direct openings in the roof in the areas over or proximate to the portions of thehome with interior water damageOur coverage determination was communicated in writing on March 30, 2014. Notice of cancellation was senton May 22, 2015 because the previously requested form had not been received.Consumers County Mutual Insurance Company received the PIP selection form on June2, 2015. A review of the form and policy revealed the policyholder does not carry PIPcoverage on their daily auto policy. In accordance to Consumer County MutualInsurance’s new business practices, the policy was not eligible to select PIP coveragebecause it was not on the daily auto policy. NAIC#: [redacted],This letter is our response to the complaint from [redacted] which we received on August 5,2015. [redacted]’s claim was received on May 11, 2015 and was inspected on May 18, 2015. Therewas interior damage due to rain leakage with no evidence of covered exterior damage that hadcaused the interior damage. [redacted]’s policy has an exclusion for damage caused by “loss thatresults from seepage or leakage of rain, sleet, ice or snow, whether or not wind-driven, unlesscaused by a PERIL INSURED AGAINST.” There was also a crack noted on a bedroom ceiling andthe policy carries an exclusion for “loss that results from settling, cracking, shrinking, bulging orexpansion of pavements, patios, foundations, wall, floors, roofs or ceilings”. During theinspection, no exterior damage was noted under, around or on top of the home. A denial wasissued both verbally and by letter on May 18, 2015 to [redacted].On June 8, 2015, we received a letter from [redacted]’s attorney advising they did not agree withour coverage decision and requested payment. We acknowledged the letter and haveexchanged communication with her attorney since then. We have received a contractor’sestimate from her attorney and on July 30, 2015, we requested to be able to meet with theattorney and / or contractor for a re-inspection of her property to review the scope and cause ofthe damages. I am responding to your letter received in our office on July 25, 2016, regarding the above mentioned insured.The rate changes experienced over the last couple of years for this policyholder were due to a number of different changes. However, none of the changes were due to any claim activity.The...

rate changes between the 2014 and 2015 renewal were due to several program changes. We changed rates for this territory to get closer to the North Carolina Rating Bureau (NCRB) rates. We are still just slightly below NCRB rates. The policy was also indexed, meaning the policy Dwelling limit was increased in this renewal from $123,277 to $125,743 in order to keep up with the cost of replacing their covered property. This increase in limits resulted in a portion of the change. In addition, the included Personal Property limit was changed from 50% of the Dwelling limit to 30%, so the policyholder saw an increase in their Personal Property premium as the Personal Property limit was kept at the same 50% level, but they are now charged for the additional 20% of coverage. Also, since the Replacement Cost coverages for the home and personal property are a percentage of the Dwelling and Personal Property premium, those increased as well. Finally, there was a slight increase in the Personal Liability premium as the included limit changed to $25,000, so the premium increased to get to the customer’s limit of $100,000. A notice was included on the policy informing the customer of the Personal Property and Personal Liability included limit change and the impact of this change.Between the 2015 and 2016 renewal, the North Carolina Rate Bureau implemented new rates which included a rate increase for some customers. This rate change became effective 10/1/2015. This rate change also included a change in the rating territories and the county where this policyholder is located was moved to a higher rated territory. However, all individual rates and options for this policy were equal to the NCRB’s previous rates and are still equal to the recently implemented NCRB rates. Once again the policy was also indexed, meaning the policy Dwelling limit was increased in this renewal from $125,743 to $128,258 in order to keep up with the cost of replacing their covered property. This increase in limits resulted in a portion of the change. Since both the Dwelling and Personal Property premium increased, the Replacement Cost and Personal Property Replacement Cost premium increased as well. Finally, the overall increase in premium was also due to a rate correction on your prior term. The deductible credit on the prior term allowed for a larger credit than was intended. This miscalculation has since been corrected and the proper credit amount has been applied to your renewal term. Notices for both the Personal Property and Personal Liability included limit change as well as the Deductible correction were included on the policy informing the policyholder of the impact of these changes.I hope this response will bring closure to our correspondence. This letter will acknowledge
receipt of the correspondence from you on April 29th, 2016. We received this claim filed by
the insured, Quicken Loan Inc on March 11th, 2016. The damages
reported by the borrower, [redacted], indicated interior water damage to the
bedroom due to water leaking from...

the roof. 
The claim was assigned to [redacted] for handling who sent it out for
inspection to a local field adjuster. The inspection completed
confirmed interior water damage from the roof was found to a bedroom. The
inspection of the roof found that one slope was replaced and is newer than the
other slope. No wind or hail damage was found to the newer slope of the roof
and it was found to be in good condition. The inspection of the older slope
found no evidence of wind or hail damage. The slope was found to be worn and
deteriorated due to age and wear and tear. This resulted in water to seep and
cause damage to the bedroom. Upon review of the inspection
report and the policy it was found that the roof was not covered due to age,
deterioration and wear and tear. However, the interior water damage is covered
in the policy. An estimate to repair the water damage in the bedroom was worked
up and repairs came to a total of $550.13. The policy has a deductible of
$817.00. Since the cost of repair is less than the policy deductible no payment
was issued. [redacted] spoke with the
borrower and discussed the denial on the roof and repairs to the bedroom.
[redacted] informed Mr. [redacted] that if he finds that the repairs to the interior were
higher than the policy deductible to forward a contractor’s estimate for
