I have reviewed *** ***'s account and would like to share the notes. On November **, our client gave us this account for collections after several attempts to collect on their own.
On November **, our first letter went out. On this letter in accordance with the Fair
Debt Collection Act, it stated that *** *** had days to dispute this debt or it would be considered valid
Our second letter went out December **, stating that we did not hear back and that the debt was considered valid and to please contact our office
On January **, our third letter went out letting *** *** know that we still had not heard from her and that her account would be reported to the credit bureaus
On February **, her account was reported to the credit bureaus
On November *, her mother called and requested a bill. We mailed the bill November *,
On November *, we were asked to fax the bill to ###-###-#### which we did
.
After that there was no additional correspondence from either *** *** or her mother. Neither disputed the account. The request for a bill was acknowledged and provided as proof of debt. The key issue here is that the paitent is a *** ***. *** *** was the responsible party for payment. I have listed her account as disputed and notified the credit bureaus as well. This may take up to days to be reflected by the credit bureaus. If *** *** would like to speak to someone here directly, she may call ###-###-#### and Joyce will be able to review her file. Please note that this complaint was filed with the Revdex.com yet she has not called into our office to review any information. If the paitent was covered at the time by any insurance or she was not responsible and show something in writing to support that, I would remove her account from our data base

unable to provide us with a bill as proof of service because it is archived.  As such I am closing out this account.  Because credit removal is done once a month and this complaint was emailed after the removal for this month, the next removal will be on or close to February [redacted].  At that time we are electronically submitting the request to the three credit bureaus to remove this from [redacted] credit history.  While I am closing this out of our system, once requested to the credit bureaus it may take THEM up to 30 days for this to be reflected on the credit history.  I have not spoken with [redacted] but should she have any addtional questions or require confirmation I would be very happy to speak to her directly to help explain the step by step order this will take and provide her with any confirmation that she may need.  I can be reached at ###-###-####.  Thank you [redacted]

 There was no check mailed! [redacted] PAID THE BILL TO Dr. C[redacted] office! This has been going on for months now! I am not sure what shady business you guys are doing but Dr. C[redacted] was paid by [redacted].  Whenever we go to the doctor there is only a $15 fee for [redacted]! So the bill that you are now gonna try to bill my spouse is still and invalid bill bc I was the patient and I know that [redacted] paid Dr. C[redacted] already. So if there is any fraud involved here I'm an going to contact [redacted] again and let them know.

We reached out to our client on 6/**/16 to review the information.  [redacted] told me that she had proof of payment from the insurance company and she would send it.  Once I reviewed the information, which is very detailed, I can see that [redacted] was seen at the lab HOWEVER Cletodell Titus...

is the subscriber for the insurance.  That is the person responsible for the account , not [redacted].  As such any check would have been mailed in his name.  We were not aware of this when the account was given to us.  Given the new information that was provided by the lab I have closed this account.I WAS VERY CLEAR THAT THIS ACCOUNT SHOULD NOT HAVE BEEN FORWARDED TO US WITH THE INFORMATION THEY PROVIDED.  Thank you for bringing this to our attention.  [redacted] has requested in her response that she does not wish for us to contact her.  If she reviews this and allows I would be happy to mail her a letter stating that this account has been removed.Thank youJoyce F[redacted]

[redacted], I understand your frustration - I am frustrated as well!  It is situations like this that lead people to believe that collection agencies are "shady".  I can assure you we are not.  You requested that we do not contact you directly so I can not speak to you and address your concerns.  We are a third party collection agency.  We are given work once a client has been unable to collect on their own.  We do not buy accounts.  We work off the information that a client gives us.  Once I read your complaint I contacted the client directly.  Once the client spoke to me and provided me with the paperwork to back up the reason this account was given to us I personally told the client they did not follow a proper protocol.  I canceled out this account.  I told the client that this was unacceptable and should not have been given to us to collect on.  I could not mail you a letter confirming this because you do not want us to contact you directly.  Also, this is a public domain so some information may not be written in detail.  I do not believe, with the information I was able to view that the client will be able to pursue your spouse.  I can not tell the future but I am fairly confident with that statement.  But again, this would be easier for us to discuss directly should you be able to call here.   As for the check that the client stated was mailed, they did not provide me with any proof of a check issued or copy of one deposited.   I hope this response better addressed your frustration and concerns.  It is 940am and I will be here for a half day today.  Please reach out to me if you would like to discuss any lingering questions you may have.Thank youJoyce F[redacted]

Thank you for your communication.  I have reviewed the information and I can provide the details as follows.  We have an account for [redacted] from 5/**/14.  At the time the address on file is a New York address on [redacted].  The information I have shows the date of service...

was 5/*/13.  After insurance was applied there was an outstanding balance of $116.01.  The medical lab mailed 6 letters to collect the amount due before turning the account over for collections on 5/**/14.  Our first letter was mailed 6/**/14 which ask for the consumer to contact us with in 30 days to validate or dispute the debt.  The second letter was mailed 7/**/14.  The third letter was mailed 8/*/14.  We were never contacted by the consumer.  This account was reported on 9/**/14.  After checking with the lab, a payment was never made to them directly and this complaint is the first communication that we have received.  I can provide the insurance policy number but would prefer to do so via phone or email to check for any possible transcription errors that may have been made by the client.  Please do not hesitate to contact either myself or Punita directly at ###-###-#### or via email so we can work together to correct this issue.

I dont appreciate [redacted] marketing pursuing a fraudulent account. Please provided me with documents that have my signature, Please tell me what kind of account is this. If we can not resolve this I will have to respectfully seek the counsel of [redacted] 
Im not responsible for this account and I want this taken off my report, I also want my information and name off this account. I went to the police to make a id theft report and the police told me that I would have to get a copies of all documents which should be provided by [redacted] company. So please provided me with all documents that have my fraudulent signature.

Review: I received a letter in May attemping to collect money from PMA. I called them on Tuesday, May **, 2015 at 10:38 and had a conversation for 3 minutes and 34 seconds. I stated that I was disputing the validity of the money owed. I received a letter a few days ago stating that I never responded to the previous letter and that they would be forced to report me to each credit reporting agency. I spoke with [redacted] there this morning and she was beligerent and stated that I never called and that they will continue to send letters until I pay.Desired Settlement: As collection companies are increasingly getting sued for harassment, I want to make sure their behavior is addressed and corrected before I have to seek legal action. I have proof that I contact the company which makes their letters and what [redacted] says lies. I wish to not be contacted by them anymore regarding the false claim that I owe their client money.

Review: I checked my credit and saw I was in debt for a medical bill in 2010. I called and spoke with a representative who told me she cannot help me for whatever reason and refered me to [redacted]. I emailed [redacted] and received no response. I was also informed they do not possess a fax machine for me to send my documents to. What type of business does not possess a fax machine? I have my proof but [redacted] refused to respond. The email given was [redacted] .Desired Settlement: I desire for this company to take the debt collections off my credit. I was covered by Medicaid insurance in 2010 and also sent a image to [redacted] showing I was covered. I have yet to receive any response or information.

Review: This collection agency has 11 negative billing collection accounts against me and its quite ridiculous that they do. I spoke to a representative about 2 weeks ago in reference to these collections and asked her why I have these collections against me when I have insurance at the time which was medicaid along with fiddles care, and I also stated to her that this hospital which is [redacted] went bankrupt and is no longer in existence, she then proceeded to tell me that I was marked as self pay and I told her that it wasn't possible because I had insurance. She then proceeded to tell me that the only way they can delete these accounts if I get them proof of my insurance, I told them thats their job and specially that this hospital no longer exist. Also I told her that I have [redacted] and I have proper documentation to state this and that I shouldn't be held accountable for these monies that I don't have and also that should be billed else where. I worry about my credit report being so affected and the fact that I am sick doesn't help its causing me anxiety and fear.Desired Settlement: I want them to delete these negative accounts of my credit report which are affecting immensely and I don't owe this money at all. This should have been charged to medicaid at that time which goes from 2007 till 2010. This hospital no longer exist and also the amount that is held against me is around $3,800 dollars I don't have. I have listened to the recorded conversation between our collector [redacted]. and [redacted]. In the conversation which lasted 7 mins and 27 sec., [redacted] double checked and confirmed the information that [redacted] gave us, she reviewed the information from our client and told [redacted] that the client listed her as self pay. When [redacted] stated she had coverage, [redacted] put the call on hold, called our client to see if the had that info on file and was able to provide our client with "metroplus"and medicaid as the carrier. When [redacted] returned to the call she informed [redacted] she should send us a letter with her dates of coverage for Metroplus as well as medicaid coverage for us to forward to our client and remove from her account. [redacted] was very polite., [redacted] asked if she could fax the letter, she then confirmed the fax number, double checked if [redacted] had an extention and thanked [redacted]. Neither party became irrate and [redacted] was not heard to be unsatisfied with the information that was provided. I understand the fustration on [redacted]'s part but her statement to the Revdex.com does not reflect the true conversation that transpired. If [redacted] could please just forward the proof of insurance for the time of her incidents, we will be happy to provide that to our client as proof of insurance. Removal from the credit history can take up to thirty days to be reflected on her credit history. Should you need, I would be happy to forward you the recording of the phone call. I look forward to hearing from you.

Review: I received OBGYN services while in college. While there I had medical insurance but the insurance was never billed. When I called to speak to a Representative and I was told my father did not change the insurance from New York insurance to South Carolina insurance. I called my father he changed and it and now years later this is still on my credit hurting my credit score. it its now 6 years later and I need for this to be removed from my report.Desired Settlement: I want the medical staff to fulfill the status as paid in full due to the negligence and carelessness in not billing the insurance company when they were suppose to.

Review: Since January *, 2009, Paul Michaels Associates have been sending me collection letters for the amount of $1800 for the services allegedly performed by Anesthesiology Assoc Boro Park on 10/**/2007. I have never received an invoice from Anesthesiology Assoc Boro Park. I have requested a detailed invoice from Paul Michaels in writing many times. However they only sent a print out of a delinquency detail report which was a communication between their company and Anesthesiology Assoc Boro. I have informed Paul Michaels in writing many times that I did not even know I received a service from this company on the date of my son's birth so I needed to see the invoice and even if I received a service then I should be given 60% charity care off since I was granted 60% off by Maimonides Hospital for all the medical services I received in Maimonides hospital between 3/*/07-2/**/08. Instead Paul Michaels sent me another letter requesting the same amount and they also reflected this conflicted claim in my credit report. I just need to see the invoice and if the reduced price was reflected by Anesthesiology Assoc Boro Park. During and immediately after my pregnancy I had paid all my bills as soon as I received them. I had recorded all my payments and the invoices. I do not have anything from Anesthesiology Assoc Boro Park, no invoice, no records or any information or document showing that I received a service. I would like Paul Michaels Associates to provide me with the proof that I received service, was billed on that date and my charity care deduction was applied accordingly. Otherwise I do not accept the claimed delinquency and I respectfully request that their claim is closed and the same is reflected to my credit report.Desired Settlement: I do not have anything from Anesthesiology Assoc Boro Park, no invoice, no records or any information or document showing that I received a service. We checked with client and ws told that only covered hospitalization not anest. When we told [redacted] we were resent the letter of charity. We offered to discount bill by 50% and offered payment plan. Again, was told by [redacted] that that was not a bill - it is. As per the copy that I faxed it stated date of service, procedure and codes, amounts recvd and what is paitent respon. With the offer of reduction as well as offer of payment plan we were again sent letter of charity.

Review: I have recently obtained a copy of my credit report and I noticed this collection account for a medical debt. I have no knowledge of this account and I have not received any correspondence such as the date of service, the provider who rendered the service and a original copy of the insurance company's explanation of benefits. Collection agencies must provide the validity of this debt or delete the information from my reportDesired Settlement: I would like to have this debt deleted from my credit report immediately.

Review: An open collection was placed against my credit reports in April of 2014 for $373. I settled all outstanding debts with Shiel Medical Labs in January of 2015 and received a letter from [redacted] indicating that the collection had been settled in full. As of March **, 2015, it is still listed as an open collection on my [redacted] credit report. I have tried calling their main line (###-###-####) multiple times. I receive a message indicating that they I have reached their organization and the next available representative will speak to me. The message repeats for 5 times and then disconnects me. I have tried to reach them via their website and I get an error message.Desired Settlement: I want this removed from my [redacted] report as an open account in collections.

Review: I am writing this complaint in hope for a resolution. I have never heard of Paul Michael Associates and I was billed for $197.40 regarding a creditor Shiel Medical Labs, Account # [redacted]. The letter says the date of service was 10-**-2012 and that if I did not make payment, this will be reported to collections. When I spoke to [redacted] Associates, they said it had to do with a doctor appoitment and lab work, and that Shiel Medical Labs has sent 'several' payments to me and I that had failed to make payment which is why a debt collector is now requesting for the funds - this is false and inaccurate. I have never received any bills from Shiel Medical Labs and I have lived at the same residency for years. They asked me for my health provider/insurance, and I provided them my Aetna member ID. A month later, I received the SAME letter from [redacted] Associates stating that they weren't 'paid' by the insurance and to make payment. This prompted me to call Aetna to understand why my insurance would not cover lab work. Aetna informed me the date of this lab work says 10-**-2012, and the submission of 'this claim' is 11/**/2013! More than a YEAR later. All lab work, and claims have to be submitted within a 100 day period. They asked [redacted] Associates for proof that this claim was actually submitted within this timeframe. They have not provided any documents and are not helpful. I am unsure what kind of business relationship [redacted] Associates has with Shiel Medical Labs, but this is completely unethical and unprofessional.Desired Settlement: Complete Removal and Cancelation of the balance $197.40. Please also take my information out of your systems.
