Mr [redacted] , we received a complaint with the Revdex.comWe understand that you were contacted by one of our customer service representatives and were given information regarding your newly issued Department of Transportation numberWhen our representative spoke to you he explained that he was with Unified Compliance Services, which is the name of our companyWe are a service provider that assists new applicants like yourself with the several regulations put in place by the Federal Motor Carrier Safety Administration (FMCSA) in which you are required to maintain for your business When you spoke to our service representative, you agreed to purchase three driver qualification files which meets the requirement CFR It appears that two of the three interviews have been completed, motor vehicle reports pulled and files were sent to you on April 8th and 12thPlease understand that the questions asked in the interview are required for us to complete the files correctly for your businessMissing or incomplete documents can result in loss of points at the time of your safety auditAll business information is safe in your secure business profile on our encrypted databaseThose records are for only the owner of the company and the safety auditor at the time of your safety audit You then called back in to our office and spoke to a different sales representative and you stated that you just wanted to go over everything again and make sure your business was fully coveredHe notified you that if your business was in fact crossing state lines you would need to file your Unified Carrier Registration in which you then proceeded with the payment on April 20thAfter you received our invoice for that service you called back in to cancel and requested a full refund for all services provided.I did look into your account entirely and it does appear that the Sales Representative sent the initial sale email on April 4th to an incorrect emailThis resulted in you not being provided with your invoice or written confirmation of your rights to cancel your services with our companyOur company has a strict policy to send a very specific email and receipt that not only confirms your purchase but makes you fully aware in writing of your right to cancelDue to this step not being completed correctly I have issued a full refund for all services renderedAlso as per your request we have deleted all of your company’s personal information that was gathered at the time of the driver’s interviews The Federal Motor Carrier Safety Administration has many requirements for businesses that require a DOT numberWe are only here to provide a service and be a helpful tool for those companies that do not wish to interpret the many complex regulationsWe are a valuable resource for many new businesses and I am truly sorry that you feel that we miss-represented ourselves in any wayIf you have any further concerns, please feel free to contact me directly Krystal S [redacted] (877) 647-4582Chief Operating OfficerUnified Compliance Services

Complaint: [redacted] I am rejecting this response because: After talking with the Attorney General he told me they had pulled this trick on a bunch of different peopleThey make you think they are DOT agents and work for DOT but they don't they trick people to pay for things that they don't really needAfter you pay them they send you paper work and then you realize they do not work for DOT but by then they already have your moneyI was told to go through only the DOT main office and they will never call or text me or email me like Unified Compliance Services doesThey are refunding most my money but should refund all of it and never call me againSincerely, [redacted] ***

Please see attached documents. Mr***, we recently received your complaint and understand that when speaking to our customer service representative you felt that he was implying he was the DOTI have carefully looked into your account and have listened to all of the recorded phone
calls. When Carson initially called you, he first did not get ahold of you and left a voicemail with his name, our company name and a contact numberYou then called back where he missed your call and finally he called you back and was able to talk to you personallyWhen Carson left a voicemail and then spoke to you, he stated “this is Carson with Unified Compliance Services”. When speaking to Carson, before discussing any required compliances he again explained to you that he was with Unified Compliance Services, you were on a recorded phone call and that we are registered with the Attorney GeneralCarson explained to you that there are requirements that you needed to be aware of now under your newly issued DOT numberHe explained and gave you step by step instructions how to file your UCR, he directed you on how to label your truck with company name and DOT number, along with other advice he has learned from being in the industry for several yearsHe then went into explaining Regulation CFR 391, your Driver Qualification Files for the two drivers in your companyYou agreed to purchase your two Driver Qualification Files on 4/21/After your purchase, you received your invoice with a complete audit checklist and full terms of cancellationOn that same day, you did call back in to CarsonYou explained that you were on the UCR website and needed assistance with filing your UCRCarson then proceeded to assist you with completing the UCR, free of chargeYou then called back again that day to complete the Driver Qualification File Interviews with the Driver Qualification File DepartmentBoth Interviews were completed and driving records submitted on 4/21/On 4/25/your companies online portal account was created and documents were uploaded and emailed to youOn 4/27/both Driver Qualification Files were completed, emailed and uploaded into your company online portalEach time you would have called into our company you should have heard the automated system saying “Thank you for calling Unified Compliance Services”.I have not been able to locate a phone call from you since the 21stAnd the phone calls I have listened to have not indicated you wanting to cancel your service or that you had thought Carson was with the DOTI apologize for any frustration you may have with our companyHowever, due to services being fully rendered, having given you our invoice and cancellation policy, and hearing that each of our customer services representatives said their name and company name each time they spoke to youWe will not be able to grant you your refund.We do hope you understand that we are here to provide a valuable service that is greatly needed for small operations such as yourself who are unaware of the full scope of requirements in place for all new entrantsWe recommend that you keep all the files we have sent you in a safe place because you will need them for your businessBe sure to read through all of the documents we have provided and complete all the necessary items so that your business may be operating legally These services are required to be updated annually and of course you do not have to choose to continue services with us at that time but we do hope that over the course of the year you will find our services and helpful staff a valuable resource and asset to your companyPlease contact us with any questions or concerns Monday-Friday 6am-4pm (MST) at(844) 296-Krystal S*** (844) 296-Chief Operations Officer Unified Compliance Services

To whom it may concern, Attached is the first and only voicemail Carson left meFrom his voicemail, he never states who he works for, but he does state that it was about my USDOT numberI was led to believe he worked for the DOTAs you hear in the previously attached UCS file, ***, Carson tells me that he was the agent assigned to my DOT applicationAgain, leading me to believe he is an agent for the DOTThrough the course of the conversation, Carson tells me he is going to transfer me to a lady to complete my driving history/background for our driversWe thought our DOT assigned agent transferred us to Unified Compliance Services to complete our registrationWe were then told it would be $for each driver (*** *** and *** ***)Thinking Carson was a DOT agent assigned to us, we went along with itAs far as the emails, we were operating on the premise that our assigned DOT agent led us to UCS because we had to use themIt was misleading, deceptive and borderline predatoryDuring this whole process, my business partner started to become suspiciousAt that point, we went on the Revdex.com site and did our researchWe have found Unified Compliance Services has done this before with new applicants obtaining a US DOT numberFive or six days after I paid my money, when I realized was being scammed, I called Carson immediately on the number he gave me and the extensionWhen he answered, he informed me he was walking his dogI am curious why that call wasn't recorded, when I called the same business number and extension he gave me. I asked him directly if he worked for the DOT, he hesitated before finally saying "No, no I do not." It was at this point I told him he could do the right thing and issue me a refundHe told me, "I will talk to my boss in the morning and get back to you." I didn't hear from him the next morning, so I sent an email requesting my refund, he refusedWith no other options left, my only choice was to file a complaint through the Revdex.comCarson mislead us into believing we had to have the driver backgrounds, making us feel like he was doing his job as the DOT assigned agentIn actuality, UCS provides us with nothing our business needs to obtain our US DOT number*** *** Co-Owner *** ** ***

Mr***, I have pulled all phone records relating to your fileIt appears that you received a voicemail from a representative, to which you then called back our company phone number (877) 647-where you would have had to go through our automated system which clearly says “thank you for calling Unified Compliance Service…”You selected to be transferred to our sales department where you were transferred to an available representative who answered the phone stating, “Unified Compliance Services, this is Forrest”
I have listened to the entire phone call and MrF* did not say or imply that he was with the FMCSA or with the Federal Government in any wayHe went through your application and answered many questions in which you had about your operationHe reviewed items that would need to be prepared for your audit and discussed completing your Driver Qualification Files, which are required in the state of TexasHe very clearly stated that because you did not know the names of your drivers, you would need them to call into the legal department to complete the files as soon as possibleYou also agreed to call him back to notify him of the GVW of each of your trucks so he could make sure you were exempt from Drug and Alcohol TestingAfter your phone call with MrF*, you received an invoice with the full cancellation policy as well as two reminder emails and two voicemails all of which had stated we are Unified Compliance Services.
We had not heard or received any correspondence from you or anyone from you company for days after the initial saleYou state that you read the invoice the day after the sale was made, and saw that it stated “UCS IS NOT AFFILIATED WITH, REGULATED BY, OR ENDORSED BY ANY GOVERNMENT AGENCY”In which you claim is when you became upset that we are a third party companyIf you saw your invoice, you would have seen the cancellation policy in which is on the same document as your invoiceIdaho State law requires we afford a three day right to cancelWe are always happy to work with our customers and change services or cancel services if necessaryHowever, you did not contact us back for days, in which you demanded your money backWe have done our part in this sales agreement to attempt to provide you the services you have purchased and complete them in accordance for CFR
We do apologize for your frustration; however, we stand by our policy’sWe have done our part in giving you plenty of opportunities to contact us back if you had felt confused or mislead in any way. We are a valuable service to many companies who also are required to follow federal and state laws while operating under their DOT numberI do recommend that you look further into your requirements through both the Federal Motor Carrier Safety Administration as well as regulations put in place by the state of TexasWe are a service that helps lift the burden on small companies so they can be prepared for safety audits and roadside auditsIf you do not follow the regulations put in place you do run the risk of paying fines and being put out of service

We are a service provider that assists new applicants like yourself with the several regulations put in place by the Federal Motor Carrier Safety Administration (FMCSA) in which you are required to maintain for your business.When you spoke to our service representative, you agreed to purchase your 2 driver qualification files for both of your operating drivers, which meets the requirement 49 CFR 391. You also purchased your drug and alcohol program and entered two of your drivers into our national drug and alcohol consortium, which meets the requirement 49 CFR 382. It also appears the sales representative gave you two sets of vinyl’s free of charge. After looking into your account, it appears that you purchased your compliances on 12/8/2016. Your vinyl’s and 2 drug and alcohol programs were sent out to you same day and uploaded on our online portal. I pulled the emails between you and the sales representative and you requested a refund on 12/9/2016. I see that you had several short emails back and forth and it resulted in the sales representative stating he would be issuing the refund. The sales representative sent an email to the billing department to issue the refund. It appears the problem that happened was that the sales representative did not include and information to the billing department for them to find your account in our system. The sales representative should not have sent an email but our company process is to create a task for the billing department in our company CRM system. The sales representative has now been re trained on the appropriate processes. We apologize for our mistakes and hope this has not inconvenienced you too much. We have taken your two drivers out of our national drug and alcohol program and you can keep the vinyl’s you received from us.  I had the billing department issue your refund of $600.00 on Tuesday January 3rd. You should have received the check in the mail by now. We hope that in future if you find your company in need of assistance with your compliances you will think of us and we can help you sometime in the future. Complaint: [redacted]I am rejecting this response because: After talking with the Attorney General he told me they had pulled this trick on a bunch of different people. They make you think they are DOT agents and work for DOT but they don't they trick people to pay for things that they don't really need. After you pay them they send you paper work and then you realize they do not work for DOT but by then they already have your money. I was told to go through only the DOT main office and they will never call or text me or email me like Unified Compliance Services does. your company and ours, it appears that the Department of
Transportation registration process has been a confusing one. As a leader in
the industry we can understand the confusion and complexities of the DOT process
in entirety. Preparing your company to operate legally across state lines is
not an easy one, especially if one mistake is made. Our company is a National Consortium
and Motor Carrier Compliance Agency, we contact companies like yourself to help
with a handful of compliances so that you don’t have to face the road blocks
such as the ones you have experienced alone. If you listen to the phone calls I have attached you
will see that it was your employee that was originally spoken to that stated
they filed the DOT application without adding the LLC at the end of the company
name, in addition marked 12 drivers instead of 6 and did not mark any cargo
carried. When you file your DOT application and MC number incorrectly it tends
to affect all legal filings as the information automatically uploads from the
information on the DOT website. Our representative was very helpful as you can
hear in the phone calls and showed your employee how to file the UCR on her
own. In the second phone call your employee states to our
company representative “let me tell you something, I called a place that said
they can do my BOC-3 for 39 bucks and then they called me back asking for more
money because they found an error in my application”.  She then proceeded to explain that she forgot
to put the LLC at the end of your company name and that they were asking to
charge $65 to updated the application. She said she was not stupid and could do
it herself. She asked if we could just file it for her after we make the
application changes. Our representative then continued to give her a checklist
of things that she needed to get done. As you can clearly hear that we were not
the company who sold or tried to charge extra for your BOC-3. I urge you to
listen to each of these phone calls so you can understand the amount of effort
we did show to help your employee who seemed very overwhelmed with the
complexities of the DOT regulations.When our representative spoke to you personally he
went through one by one all of the items that needed to be updated and completed
so that you could operate legally. You asked many questions and went through
each of the requirements that we could help you with. You handed the phone off
to your employee to move forward on the services because you wanted your driver
on the road as soon as possible. You purchased a Driver Qualification File,
Drug and Alcohol consortium and policy, updated your DOT Application, and
BOC-3. We completed each of these services in entirety and each of them is
uploaded on your online portal in Test Vault. You are still a company enrolled
in our consortium and it seems other than the confusion between us and the
other company that attempted to charge you for the BOC3, I feel that we have
proven to be very beneficial and helpful for your company. I hope that you could retract your complaint on the
Revdex.com against our company as we have fulfilled our promises above and beyond
standard expectations. Although, we do understand and sympathize that it is
very confusing the amount of phone calls you receive and it can be very hard to
distinguish one company from another. We would hope that we could maintain a
positive and truthful relationship with each of our customers. We value your business and will spend any amount of
time needed to make sure you are on the road as quickly as possible but most importantly

loan that she obtained and despite satisfying the loan she has still been summoned to attend court in regards to the loan. She is requesting to have the harassment of civil suits to stop and a reimbursement of $89.Cottonwood has investigated Ms. [redacted]'s claims and offers the following response: Cottonwood confirms that Ms. [redacted] obtained a loan from Cottonwood on January 30, 2015 for the principal amount of $1,050. The loan's terms and conditions were reviewed with Ms. [redacted] at the time of the loan origination, a copy was provided to her after she acknowledged and accepted to the terms.After reviewing payment history records Cottonwood found that a total of seven (7) payments were made during the life of tile loan. Of the 7 payments Cottonwood found that five (5) of the payments made via ACH were returned and not funded. The account was then forwarded to Cottonwood's internal collections department to service the loan. It appears that our Driver Qualification File Department called you at (214) 296-9744 on June 27th and July 7th and left voicemails requesting your drivers to call in to complete their driver interview. They also sent you two emails, one on July 5th and July 12th reminding you to have your two drivers call in. Please understand that these files are time sensitive for your company’s compliance review and must be complete per the requirements of Regulation 49 CFR 391. We clearly state that Unified Compliance Services is not a Government agency. We are only here to provide a service and be a helpful tool for those companies that do not wish to interpret the many complex regulations of the Federal Motor Carrier Administration (FMCSA). As you can see in the documents I attached, the state of Texas requires you to have Driver Qualification Files completed for all of your active listed drivers. If you do not meet the requirements set in place you may be at risk for fines and being put out of service. We understand that you felt mislead when our company notified you of the
requirements your company has under your newly issued DOT number....

                We are a National Drug and
Alcohol Testing Consortium as well as provide other necessary compliances for
business such as yourself that have obtained a United States Department of
Transportation number. We are a service provider and you are not required to
use us as your provider. When you spoke to our customer service representative
on January 24th, 2016 you did agree to purchase your Driver
Qualification File, Regulation 49 CFR 391. In addition to our Drug and Alcohol Program,
Regulation 49 CFR 382. Your receipt and our full cancellation policy was sent
to you via email as well as uploaded onto your online portal on January 24th,
2016. Our cancellation policy clearly states you have a three day right to
cancel.                 Your full Drug and Alcohol
Program was sent to you and uploaded into your portal on January 25th,
2016. Your over the phone Driver Qualification File interview was completed in
full on January 24th, 2016 where we then sent you a release form
that you signed in order to pull your motor vehicle report. Your completed
Driver Qualification File and your full motor vehicle report was sent to you
and uploaded onto your portal on January 30th, 2016. Please
understand that the state of Colorado has adopted the Federal Motor Carrier
Administration Regulations in addition to making them mandatory for intrastate
operators with a CDL. Please review the attached documents and visit the
Colorado State Patrol website at https://www.colorado.gov/pacific/csp/NewEntrant to view your state requirements for Regulation 49 CFR 382 and 49 CFR 391. You
will also find the rest of the requirements your new business will be
responsible for.                 Due to the facts of this
instance and because we offered you our cancellation policy, completed all
services and you didn’t request a refund until February 6th, 2016 we
will not be able to grant your refund. We do hope you understand that we are
here to provide a valuable service that is greatly needed for small operations
such as yourself who are unaware of the full scope of requirements in place for
all new entrants. We recommend that you keep all the files we have sent you in
a safe place because you will need them for your business. Be sure to read
through all of the documents we have provided and complete all the necessary
items so that your business may be operating legally.                 These services are required to
be updated annually and of course you do not have to choose to continue
services with us at that time but we do hope that over the course of the year
you will find our services a valuable resource and asset to your company.
