The Company received your letter dated May 2, in regard to the above referenced file which was referred to my attention for review and response [redacted] is inquiring about the claim filed as the result of the death of his brother-in-law [redacted] ***.Policy number [redacted] was issued December 1, as a Whole Life policy insuring [redacted] Jr A copy of the application is attached The Primary Beneficiary is [redacted] , who is deceased, and the Contingent Beneficiary is [redacted] *** Because the application had to be produced from microfiche, it is not very legible and the Contingent Beneficiary was mistaken to be [redacted] instead of [redacted] *** The Claims Department has clarified that it is [redacted] who is to receive the benefits of the policy and has sent a Claimant's Statement for her to complete and sign in order for the claim to be processed.If I can be of further assistance, please contact me at [redacted] extension ***

United Insurance Company of America received your leter dated June 20, in regard to the above referenced file which was referred to my attention for review and response [redacted] is inquiring about a claim filed as the result of the death of her grandmother, [redacted] .Policy number [redacted] was issued on March 17, as a Whole Life policy insuring [redacted] [redacted] was a named beneficiary At the time of the death of [redacted] , [redacted] was a minor and could not receive the policy's benefits The Company asked for a copy of the court-issued Guardianship papers; however, no guardianship documents were ever received On May 28, 2017, [redacted] turned age and was then legally entitled to the policy proceeds The Company received a Claimant's Statement signed by [redacted] on June 9, and issued Check number [redacted] on June 20, payable to [redacted] for $2,000.If I can be of further assistance, please contact me at [redacted] extension ***

I did receive a call from United Insurance to get policy and date the July payment of $was clearedHaven't heard from them yet on the matterIf I can have a week or two more for their response I can better tell you if matter was resolved

The Company received your letter dated December 20, in regard to the above referenced file which was referred to my attention for review and response. *** *** *** has been trying to have the mortgagee updated on her dwelling policy with United Casualty Insurance Company of
America. The address on record was *** *** *** *** ** ***. The mortgagee clause states ** *** *** *** ** ***.In order for the mortgagee clause to be changed, the Company required a letter from indicating that *** *** and ** *** were the same address. Upon receipt of this document, the mortgagee was updated to reflect *** *** *** ***. A declaration page was emailed to Ms*** on December 22, and an updated policy was mailed to her at ** *** *** ** *** on December 22, 2016. I do not accept the response from Consumer Affairs because I called twice and did leave a message. I only...

received a call later because I asked customer service to send an email to consumer affairs in hopes that they would respond. I am disappointed that Consumer Affairs is claiming that I did not call when I in fact did.

I did receive a call from United Insurance to get policy and date the July payment of $50 was cleared. Haven't heard from them yet on the matter. If I can have a week or two more for their response I can better tell you if matter was resolved.
