practice this methodology as it provides a receipt of cancellation for the member in the event something isn't processed properly. We do require 45 day notice, with a pro-rate to be collected from the day the cancellation form is signed or the day the mail is postmarked. Medical cancellations can be immediate if a medical note is provided that states that the member is unable to work out for a period of 6 months or more. [redacted] is still within his year of obligation and cannot have a cancellation effective date prior to 4/**/2017 unless he provides proof of the following:-Relocation: Member has relocated more than 25 miles from an Equinox facility and can provide proof(45 day notice required)-Medical: Member is unable to exercise for a period to exceed 6 months and has a signed doctor's note stating this.(Immediate cancellation)-Job Loss: Member must provide proof that they have lost their job. They did not appropriately or timely address my concerns nor did they specifically respond to my requests for help.  If they do a yelp and other online and social media search, they will see my experience at this particular club was by no means unique and many others have expressed their dissatisfaction with this specific location's management.  If Equinox cares about maintaining a consistent level of quality, they should take customer concerns seriously and address them in a meaningful manner.

I've already provided proof refuting this statement. I personally signed my wife up under my Aegis membership, and there is no basis for anything contrary. If Equinox presents records, you'll see my signature at the bottom of both contracts, and my card used for the initial payment, both matching the rate given to Aegis memberships. I encourage Equinox to submit these records, as I was not provided a copy. I don't know know how long Equinox can delay and mislead. I was not told about the 12-month commitment, and I do not remember signing a paper contract agreeing to a 12-month commitment. I received an email copy of the "membership agreement" which appears to have an electronically produced version of my signature (highly pixelated), that I was likely asked to input on some sort of tablet device. If that was the case, then I was likely misled about what I was signing.
