I have reviewed the response made by the business in reference to complaint ID [redacted], and have determined that the response would not resolve my complaint.  For your reference, details of the offer I reviewed appear below. I do not think Seven Corners considered that I was covered from August 7 to December 31, 2016 (Certificate ID: [redacted]) at the time the symptom happened and it was physically impossible for me to immediately go to a nearest provider as I was travelling at that time. The August 7 to December 31, 2016 coverage is the same group coverage I had from January 1 to February 28, 2017 (Certificate ID: [redacted]). Both of these coverages are attached.If my claims are still denied after consideration of my coverage from August to December 2016, I will have to file a complaint against Seven Corners in the Indiana Department of Insurance and the Hawaii Department of Commerce.

The complaint was filed by [redacted] and we appreciate the opportunity to address her concerns. [redacted] purchased an EF - Educational Tours travel insurance plan with...

scheduled trip dates: 10/06/14 - 10/15/14. During her travels, she sustained an injury and sought out medical treatment. On November 20, 2014, Seven Corners reviewed presented claim documentation and found the information submitted to be illegible. This office requested legible documents from the insured in order to appropriately assess the presented loss. On December 19, 2014, this office noted we had not received additional information for review. Our records show that March 24, 2015, this office evaluated documentation that was received from the insured party regarding her injury and medical care costs. Seven Corners received an invoice from [redacted] for charges incurred at the [redacted]. This invoice/charges are not reimbursable as the costs were incurred prior to the date of injury and were not related to her medical treatment. Please be advised that I never requested for the check to be issued to anyone other than Mr. Pantazis.   Kindly issue the check to Mr. Nicholas Pantazis and mail to  38-11 Ditmars Boulevard Suite 678 Astoria, NY 11105.   I feel that this really has gone on long enough and that Seven Corners is deliberately finding excuses not to mail the check. I do not believe the first check was ever sent out either as I never received even one letter from them.   I expect this check to be issued today and sent via FEDEX.   A prompt reply will be appreciated.

I have reviewed the response made by the business in reference to complaint ID [redacted], and have determined that the response would not resolve my complaint.  For your reference, details of the offer I reviewed appear below.I am still waiting to be reimbursed by seven corners for multiple bills and reciepts that I have submitted.  Regarding Wesley S[redacted].  The bill he obtained for me was not legible and was rejected by [redacted].  He said he would get me another, that was months ago.  I have repeatedly asked what the status is and he ignores my emails.  After waiting over 5 months, I emailed him again.  NO RESPONSE.  Then, even more frustrated, I emailed him asking if he had been fired for being so bad at his job.  He responded in less than 5 minutes, sarcastically thanking me for my question and informing me that yes he still works there.  He did not update me on the status of the itemized bill.  This guy needs help.  He is also the one that left me in a hospital for 3 extra days in Costa Rica until I MYSELF, FROM A HOSPITAL BED, called him directly and inquired why he would not approve an ambulance plane home.  I have worked with [redacted] to get some of the bills refunded, I am still waiting on others.

the attention of Dennis W[redacted] on June 7, 2016.  The complainant, [redacted], has expressed concerns with a denial of a claim for his son, [redacted].  [redacted] purchased a short term travel insurance policy for his son, [redacted], covering his trip dates June 22, 2016 – July 8, 2016.  The tour package and insurance policy were purchased through EF – Educational Tours.    On April 4, 2016, Seven Corners received a claim for Trip Cancellation for [redacted].  The reason for cancellation of the trip was due to illness of the insured.  Based on the completed physician’s statement form provided, the diagnosis resulting in trip cancellation was ADHD and Social Anxiety.  The physician indicated travel was prohibited for the patient, due to the illness.   This policy specifically excludes benefits for any loss caused by or resulting from a mental or nervous condition, unless hospitalized or partially hospitalized for the illness.  Based on the information provided to us, the insured was not hospitalized or partially hospitalized.  A denial letter was sent to [redacted] on April 18, 2016, explaining the reason his loss was not covered under the policy.  The policy language which is pertinent to this matter includes the following: “SECTION IV. GENERAL EXCLUSIONS   Benefits are not payable for any loss due to, arising or resulting from: 16. a mental or nervous condition, unless hospitalized or Partially Hospitalized for that condition while the Certificate is in effect for You; SECTION II. DEFINITIONS “Hospital” means (a) a place which is licensed or recognized as a general hospital by the proper authority of the state in which it is located: (b) a place operated for the care and treatment of resident inpatients with a registered graduate nurse (RN) always on duty and with a laboratory and X-ray facility: (c) a place recognized as a general hospital by the Joint Commission on the Accreditation of Hospitals; (d) other than a residence, a place where treatment in a Hyperbaric chamber can be received. Not included is a hospital or institution licensed or used principally: (1) for the treatment or care of drug addicts or alcoholics: or (2) as a clinic continued or extended care facility, skilled nursing facility, convalescent home, rest home, nursing home or home for the aged. “Partial Hospitalization” means an outpatient program specifically designed for the diagnosis or active treatment of a serious mental disorder when there is a reasonable expectation for improvement or when it is necessary to maintain a patient’s functional level and prevent relapse or full hospitalization. original claim determination.  The reasoning behind our decision is explainedbelow.In order to clarify the role of the parties involved,Nationwide Mutual Insurance Company (“NMIC”) is the insuring company of thetravel insurance policy purchased.  OneTravel is a travel provider of flight and other vacation accommodations; andSeven Corners, Inc. (“Seven Corners”) is the third party administratorcontracted by NMIC to provide customer service assistance and claimadjudication services for individuals who purchase travel insurance productsunderwritten by NMIC. On August 7, 2014, the insured completed an online purchase onwww.onetravel.com for one wayairfare with a scheduled departure date of August 8, 2014.  Per documentation from the insured, a representativefrom One Flight Travel contacted her and suggested that she purchase travelinsurance for her scheduled flight.  Theinsured purchased a travel insurance policy for $89.48 on August 8, 2014.  According to policy provisions, tripcancellation coverage takes effect the day after the required premium isreceived; as a result, Ms [redacted] travel insurance became effective August 9,2014.Ms. [redacted] stated within her complaint to the Revdex.comof Central Indiana that she purchased her travel insurance from “One FlightTravel.” Seven Corners contacted with One Travel and they confirmed the insuredparty purchased travel insurance from their online website, www.onetravel.com and the bankingstatement submitted by the customer confirms this as well. Neither, SevenCorners or One Travel has any affiliation with a company known as “One FlightTravel.”Our records show that on August 8, 2014, [redacted] and [redacted],the daughters of Ms. [redacted], were both treated by Dr. [redacted] O. [redacted], afamily member, for food poisoning.  Theinsured cancelled her scheduled trip with www.onetravel.comon August 8, 2014, and she presented a trip cancellation claim to Seven Cornerson November 26, 2014.Seven Corners evaluated the presented loss and determined nocoverage was applicable.  Travelinsurance was purchased on August 8, 2014, and [redacted] and [redacted]illnesses were diagnosed and travel prohibited on August 8, 2014; however, thetrip cancellation coverage benefit did not become effective until 12:01 a.m. onAugust 9, 2014.  There is no coverage forthis loss, as there was no active trip cancellation coverage at the time ofloss/sickness onset.  Policy languagewhich is pertinent to this matter includes the following:GENERAL PROVISIONSThe following provisions apply to all coverages:WHEN YOUR COVERAGE BEGINSAll coverage (except Trip Cancellation) will take effect at 12:01 A.M. localtime, at Your location, on the Scheduled Departure Date provided:(a) coverage has been elected; and (b) the required premium has beenpaid.Trip Cancellation coverage will take effect at 12:01 A.M. local time atYour location, on the day after the requiredpremium for such coverage is received by the Company or its authorizedrepresentative.BENEFITS TRIP CANCELLATION & TRIP INTERRUPTION The Company will pay a benefit, up to the maximum shown on theConfirmation of Coverage, if You are prevented from taking or unable tocontinue Your Covered Trip due to: (a) Sickness, Accidental Injury or death of You, Traveling Companion,or Family Member; which results in medically imposed restrictions as certifiedby a Physician at the time of Loss preventing Your continued participation inthe Covered Trip. A Physician must advise cancellation of the Covered Trip onor before the Scheduled Departure Date.TRIP CANCELLATION The Company will reimburse You for the following:Non-refundable cancellation charges imposed by the Travel Suppliers.In no event shall the amount reimbursed exceed the amount You prepaidfor the Covered Trip up to the maximum benefit shown on the Confirmation ofCoverage.SPECIAL CONDITIONS: You must advise the Travel Supplier and the Companyas soon as possible in the event of a claim. opportunity to address your concerns. In order to clarify the role of the parties involved, One Travel is a travel provider of flight and other vacation accommodations, and Seven Corners, Inc. (“Seven Corners”) is the third party administrator contracted to provide claim adjudication services for travel insurance policies issued by Nationwide Mutual Insurance Company. Seven Corners evaluated the documentation submitted by Mr. [redacted] regarding a trip cancellation claim. Mr. [redacted] noted he cancelled his scheduled travel due to illness (autism and paranoid schizophrenia). Claim documentation presented to this office did not indicate that [redacted] was hospitalized for his illness. The purchased policy requires that for mental or emotional disorders, the client must be hospitalized to be eligible for coverage benefits. Based upon the above plan language, his claim is not eligible for reimbursement and a denial letter was mailed to his address of record on December 30, 2015. On February 16, 2016, this office received a claim appeal from the insured party disputing the initial decision and providing a different reason for cancellation. Mr. [redacted] indicated within his claim appeal, that his travel cancellation resulted from the death of his grandmother who passed away on December 1, 2015. The purchased policy provided plan coverage from February 15, 2015 to March 9, 2015. In order for trip cancellation coverage to be applicable the presented loss must take place after the effective date of his policy. His family member’s death occurred after his policy coverage expired and therefore is ineligible for reimbursement. Further documentation was mailed to the insured’s address of record confirming that his losses were not covered under the purchased travel insurance policy. Should Mr. [redacted] provide documentation from his treating doctor that he was hospitalized for his illness during the policy effective dates, we will gladly give his claim further consideration. Based upon the above information, [redacted]‘s trip cancellation claim is ineligible for payment due to a policy exclusion. The policy language pertinent to this matter is as follows: BENEFITS TRIP CANCELLATION The Company will reimburse You, up to the Maximum Benefit shown on the Confirmation of Coverage, if You are prevented from taking Your Trip for any of the following reasons that are Unforeseen and takes place after the Effective Date: Sickness, Accidental Injury or death of a non-traveling Family Member; LIMITATIONS AND EXCLUSIONS The following exclusions apply to Trip Cancellation & Trip Interruption, Trip Delay, Missed Connection, And Accidental Death & Dismemberment: 6. Dr. [redacted] is part of Seven Corners' network. I do not think I am responsible for mediating the payment process between Seven Corners and the doctors office as I made sure I go to a doctor who is part of the insurance network prior to going to the doctors' office. The failure to communicate with the doctor should not make me a victim for harassing letters from [redacted] lab and their collection office and ruining my credit score. I hope Seven Corners finds a better way for reaching the doctors' office.

I stated before; this is unacceptable. As I said earlier; I was not made aware PRIOR to my purchase the extreme limitations of this policy. Nor was I made aware PRIOR to my purchase that I would be dealing with a company other than the one named on the policy. This would have drastically altered my decision in choosing such an inadequate policy and company. 
This was clearly misleading and a blatant bait and switch.  I expect my claim honored.

Thank you for your consistent responses. I now understand, based on the last letter sent, the differences in company names and the roles. However, this is not a case of "better late than never". I was not made aware at the time of purchase and was therefore severely mislead. 
I read nothing on the website of square mouth, describing about a 10day free look period and as I stated before I was not able to review the policy prior to purchase. I only saw the large print inclusions. There was no asterisk or fine print.  There was absolutely no mention of the many "exclusions".   I did review, the website thoroughly prior to purchase and only saw repeatedly, the square mouth "zero complaint guarantee" which turns out to really mean nothing at all.
I therefore cannot accept this answer.

purchased a PeaceCorp/ AfterCorp plan. Sara [redacted] sought out medical care on October 10, 2012. Seven Corners, Inc. received medical records affirming the incurred costs. However, to appropriately evaluate her claims, this office mailed documents to the insured party for completion. On October 29, 2012 and November 15, 2012, this office sent Ms. [redacted] correspondence which included: a Proof of Loss form and a request for Peace Corps PC-127C or PC-209B authorization form. Upon receipt of this information, the presented claims would be evaluated for eligibility; however the documents were not returned at that time and the claims were pended, awaiting the requested documentation.On December 19, 2014, Seven Corners received via fax, a PeaceCorps Authorization form from Ms. [redacted]. Regarding the [redacted] trip and the response from Seven Corners.  On the original claim form [redacted]'s physician selected the option provided for other in regards to the condition of cystic acne.  We completely understood it not being an illness and that is the reason the option provided "other" was selected.  Also the Dr. amended the date she first saw [redacted] for the condition in a letterhead from her office which was faxed to Seven Corners.  My daughter was looking forward to attending this trip but just was physically unable to do so due to this debilitating condition which left her with huge lumps on her face.  I ask you to remember being among High School peers during such an extreme condition and being on such harsh medication which requires monitoring from said physician.  It has not been easy and having to have cancelled such a great opportunity with [redacted] was beyond troubling for [redacted].  I have an original copy from Dr. [redacted]'s letter head submitted to Seven corners (to amend the date on the claim form).

(Actually, I didnot receive any check at the moment.) However, on September 25th,I received another mail from Seven Corners. This mail was a check (in the amount of 194.58 USD) dated August 20th,2015.  This mail used “Taiwan, provinceof China” as the country’s name. I asked the employee in the postoffice (night service) why this situation happened (I did not pay attention tothe date, September 4th, on the first mail, before receiving thesecond mail from Seven Corners).  We foundthat August 20th was quite a long time ago from September 25th.  After seeing the date and the address (withthe incorrect country’s name), the employee said that the second mail (receivedon September 25th) might have been sent to Mainland China and thensent back to Taiwan.  This was his onlypossible explanation. I think the current best way is toask Seven Corners to mail me a check in the correct amount of 225.91 USD.  I do not want the bank to take too much moneyfor the transitional fee, and prefer keeping/saving the money in USD but notchanging to the currency in Taiwan. I write down my address again:[redacted],[redacted]TAIWAN I really hope Seven Corners canaccept my request.  Thank you very muchfor your precious assistance. Best regards, [redacted]
