Dear Sir or Madam,
Thank you for giving us the opportunity to address the beneficiary’s concerns regarding his prescription drug costs with *** *** *** *** Part D Prescription Drug Plan (PDP)This letter is in response to your correspondence dated April 24,
Please contact us with any questions or concerns regarding this response
Sincerely,
*** Insurance Company

Well, our pharmacy insurance changed to cvs Caremark April 1st, 2016, which you can only get that monthly prescription at cvs, *** or mail order even though my pharmacy is 2-minutes away on my way home.(my pharmacy gives me accentives towards gift card for food which we use as a family of 7!)
I don't trust mail order around our area, have it sitting in the mailbox
Cvs and *** are both over minutes away
I appealed once and was supposedly denied but never told until I called! I'm disappointed that a company would deny me or anybody to use the pharmacy of their choice! I read cvs Caremark comments on Revdex.com, almost unhappy people and I don't want to get caught up in that mess! It should be illegal to not get your prescriptions at certain places and only through place of their choiceI'm not the only one but this is disappointing!

This is in response to your inquiry dated October 14, which we received on October 29, regarding his compound medication.I spoke with the member on October 15, and advised *** *** *** has previously advised they were able to fill the medicationThe member advised his physician’s office stated *** *** *** was not able to fill the medication; however, the member stated he would contact them directly to verify if they were able to fill the medication. CVS Caremark has incorrect information on their data system concerning our doctor and they will not correct itI have given them information, they have confirmed it, but still they continue to act on this & now they have based on this erroneous information, decided our doctor is unauthorized to fill prescriptions for us and that we should find another doctor !! Dr*** *** is fully licensed - See “History”

This is the company I am required to use for my prescription service through the state of Illinois I wanted to start getting my rx by mail because it would be cheaper My MD has been trying to get my prescription through by FAX they have other patients that go through this process It has taken over a month and only of my scripts is in the cvs system I am currently out of this med but I have not received the med Cvs Caremark will not authorize a local pharmacy to fill this unless I pay full price It will cost me out of pocket a retail price over $ I am retired and cannot afford this Therefore I will go without the med my doctor has prescribed

I am writing to you as I do not know what other option to take hoping that you can assist me
A few months ago I tried to use my benefits at CVS to buy some speciality medications and it was just too expensive for me to affordI found another pharmacy that could fill the prescription at half the cost which would save me about dollars
I went ahead and requested for an override which was offered to me but due to finances not being enough I went ahead and put that on holdI have tried to call again to get the override activated and I have now been advised that I can no longer utilize that money unless I wanted to just go ahead and pay out of pocket
My questions were not really answered because from what I understand my benefits have not changed so I should be able to utilize the overrideMust I also mention that it is such a pain to be on calls for hours during the naïf stressful time for a patient trying to get medications instead of this being a painless process

We verified the medication was filled on October 17, and shipped to Mr*** on October 18, via Mail Innovations Per the *** *** Postal Service tracking information, the package was delivered to Mr***’s address on October 24, Mr*** would be responsible for the
copayment if the medication is considered lost or stolen The copayment of $was originally charged to Mr***’s credit card; however, he disputed the payment with his bank. We received a credit card chargeback on April 23, in the amount of $185. Mr*** contacted us on May 4, and paid $towards a prescription he received; therefore, Mr*** had a remaining balance of $for the medication he did not receive We sent requests to our finance department on December 31, and May 19, 2015; however, both requests were denied as the medication showed a delivery confirmation and had not been returned to us. I spoke with Mr*** on June 4, and advised of this informationHe requested we review the information again to verify if we are able to credit him for the prescription as he did not receive the medication. I sent a request to our Account Team for further review. The request was approved as a onetime exception and a credit in the amount of $was posted to Mr***’s mail order account on June 5, At this time, Mr*** has a zero dollar balance on fileI contacted Mr*** and advised of this information. Mr*** was satisfied and did not require any further assistance at this time

1-12-placed order for day prescription from which my Doctor wrote for a day supply CVS Caremark erroneously charged me $for the day supply citing they were restricted from filling the whole prescription for days 1/14/2016, after several lengthy attempts to resolve this matter with Call Center reps; ***, *** *** *** and continuously placed on unreasonably extended hold and finally forwarded to: "***" (Senior Clinic Advisor), and supervisors, "***," "***" (terrible disposition), and "***" who informed me because of the discrepancy, ** *** would be given a one time bulk up credit for $dollars and to keep the day prescription These supervisors informed me I would not be billed for the prescription 1/15/16, *** ***l left a voice mail stating what these supervisors told me earlier about the bulk of credit which I saved on my cell for documentation to support my voice records, date & time I did not receive a day s

While I highly doubt that this will go any further, I want Caremark to know that the process by which they approve or deny medication is not only poor but in some cases potentially dangerous. I believe that Caremark should be required to make it a business practice to alert any insured of these requirements. Caremark never contacted me during this episode, I had to repeatedly call them to ask for a status. My doctor sent in urgent requests alerting Caremark to my past history of suicide attempts due to severe depression and included the history of taking the generic version and it not working to relieve my symptoms. Caremark does not give any of that consideration and so someone sitting in an office somewhere just denied it without any explanation to me, putting me in a near incapacitated state for almost a week! (Note: I received a letter a week later after I submitted this complaint). Exactly what could the benefit be, to a company the size of Caremark, to require a patient on a specific medication to fill their prescription with a generic before they can get the medication that they need? They literally approved the appeal 2-days after I accepted the genericI don't see the sense in that. Given the size of Caremark, I doubt that anyone there will give this a second lookI just hope their current practice doesn't result in more severe harm to anyone else
