-Called CVS caremark mail order deparment and spoke with a representative to ensure and confirm the correct

08/24/16-Received an email of "Order Received Confirmation with the Correct mailing address
08/27/16-UPS tracking system states that the medication has been delivered by not in the mailbox of the correct
address
08/29/16- Called CVS Caremark and the representative states that the medication order has been sent to the
WRONG ADDRESS
-Went tot he wrong address location to inquire about the medication and the medication package has been
opened by the person living in the house --HIPPA VIOLATION !!

This letter is in response to the correspondence we received from your office on January 9, Thank you for the opportunity to address Ms*** concerns regarding being sent to collections for a balance due. Upon review of Mrs***’s concern it was verified Ms*** was sent to collections because the date of service of February 22, was billed several times and denied coverage by her insurance companyOnce Ms*** spoke with her insurance company we were able to reprocess the claim on August 17, The outstanding amount of $that was sent to the collection agency was paid on September 26, by her insurance companyUnfortunately the agency was not notified that the balance was paid off which is why Mrs*** continued to receive calls and notices of a balance owedAs of today, Ms*** has a $balance on her accountOur billing and reimbursement department has been in contact with the agency to cancel the balance from collection. Caremark is a horrible company that is dealing with medical, similar to doctors in that they are to ensure that patients get their proper medications prescribed by their doctors Unfortunately Caremark thinks they are better than your personal doctor and get to tell your doctor how to practice even though they are NOT DOCTORS and the doctors they have working for them have never met the patient They will deny medications playing games with your healthThey are indirectly in the medical field and causing harm to people

Upon review of Mrs***’s concerns we were able to confirm that there was no delay with shipping her order. On December 2, Mrs***’s order was shipped for delivery by December 3, to her local CVS retail pharmacy. On December 7, Mr*** contacted our specialty customer care, and was informed that the order was delivered on December 3, as expectedOur records do not indicate that there were any delays nor do we show any documentation stating that there were any issues with Mrs***’s order. In an effort to assure Mrs***’s package did arrive, our specialty pharmacy contacted the CVS retail location and verified the package was delivered

I contacted Caremark back in May as my Benefit Choice was open that month with a start date with new health plans on July 1, I was unable to find any info online or when I called, so I filed a Revdex.com complaint I had a representative from Caremark call me and tell me that he can answer my questions The reason it is so important to know what drugs are covered is that my wife is years post transplant with no problems When I spoke with the rep I told him *** (***) was one and said well generic is covered at $20/copay day supply, then asked about *** 360, which again said a generic $20/day supply So I decided to switch to the *** open access plan and cvs/caremark which is attached to it I go fill my wife's prgraf and its a $for a days supply!!!! I was like what!!!! I paid 0!!!! when I had my previous health insurance, they covered a small portion and a pharmacy voucher from the manufacture was applied, we paid nothing and that was for
