The homeowner has spoken with a supervisor on this claim and the supervisor has left a message for the vendor to call in with diagnosis once the accessing has been done

The vendor contacted the manufacturer's technical support and HSA will reach out to the vendor to find out what the resolution is and contact the homeowner

HSA records show that on 10/6/a heating claim was filed and was paid out to the vendor on 10/27/and 11/2/14. HSA was not notified until 2/13/via email from the homeowner and HSA put the homeowner in touch with the vendor. As of 2/18/15, HSA has reviewed with the vendor
and confirmed the repairs were completed and advised the homeowner if he had any issues with the furnace to call HSA to file a claim, which has not been done at this time

The letter has been forwarded to me for review and response I have reviewed HSA’s records regarding Ms***’s leak claim. On July 13, 2016, Ms*** placed the service request and HSA dispatched A Water Solution, Incto diagnose the leak problem. On July 18, 2016, the technician reported to HSA that the pipe to the hose bib was leaking due to freeze damage. Section F-of Ms***’s HSA plan provides that coverage does not apply when failure is due to freezing. Therefore, HSA is rightfully not responsible or liable for the repair of the leak Ms*** disputed the diagnosis and on July 20, and again on August 22, 2016, HSA offered to run a second opinion service request on the leak claim. Ms*** declined these offers and cancelled the HSA plan coverage HSA admits no liability; however, in order to amicably resolve this matter HSA has waived the $trade service call fee for the leak claim. They called and originally offered an additional $150, I am willing to accept this as the missed work wages for the MULTIPLE missed appointments by the company they hiredI was NEVER asking for money missed for waiting while the technician worked, it was for the MULTIPLE missed appointmentsIf they can re-offer the $for missed wages with the $refund of the deductible for a total of $250, this will be accepted.]

After further review of the diagnosis. The service provider reported to HSA that there were missing shingles from the roof as well as holes in the roof. These failures were not due to wear and tear but due to wind damage, storms and branches hitting the roof itself
HSA's
contract does not cover for damage done by ice, mud, snow or wind and any acts of God. The diagnosis is reported by the professional in that field. HSA is unable to offer coverage on the claim. Thank you

HSA apologizes for the customers claim experience. If there are damages that were done by Ice Cube Air the customer would need to seek remedy with the vendor

The charges for routing from the roof vent pipe and installing a clean out in the basement are not covered under the HSA contract. HSA covered to route the line once the clean out was installed and HSA has issued a refund check for $that was mailed out on 9/19/

However, my previous correspondence accurately and correctly stated HSA’s position regarding this matter. HSA’s position does remain unchanged. Mr***’s contract limit ($750) has been met and any further roof repairs are not covered. HSA cannot “force” an independent service contractor to complete any non-covered repairs. Section F-states that secondary damage, consequential damage, or any damage caused by or resulting from the failure or malfunction; or any damage resulting from the actual repair or replacement itself is not covered. Based on the terms of the plan, HSA has rightfully upheld its warranty obligations and denies any further liability with respect to this matter. If Mr*** feels the service contractor performed negligent repairs causing damages, he and/or his home owner’s insurance company may contact Elite Contracting, LLC directly at Carnegie Avenue, Clairton, PA 15025; ###-###-####, to further discuss this matter as they should be insured for such occurrences. HSA apologizes for the customers experience. The customers contract has expired on November 5, 2015. The contract covers for mechanical failures within days of labor and or days of parts. The failure that the customer is reporting is outside the terms and conditions of the
contract and HSA would not be able to offer coverage through the warranty. HSA dis advise the customer that they may contact the service provider to see how they may further assist with their current failure. At this time HSA would consider this matter closed. Thank you

HSA will refund the deductible for the oven claim as a customer service gesture. A supervisor will contact the homeowner to advise of next step in this claim process
