I wish to again thank the Revdex.com for all of their help regarding this matter, as I do believe strongly if this matter was not addressed by your office, it would have not been removed as it had remained on my credit report by OAC Collections despite providing a removal letter. Our client, Doctors Radiology Group of Gainesville is contracted through North Florida Regional Medical Center were services were provided. When you sign into the hospital forms are signed, and part of the form states that a third party, us, has permission to collect on this if it is...

transferred to collections. We received this account on 9-15-2014. We have had contact with Ms. [redacted] a few times in 2014 in which time she informed us that she believed she was on Medicaid at the time of service. In order to resolve this account for our client we have to request a Letter of Credible Coverage with coverage dates so that we are able to establish whether or not the consumer was insured. To date we have still not received this Letter of Credible Coverage. We received a dispute from Ms. [redacted] on 1-9-2017 and mailed out an itemized statement to her at her address on file on 1-12-2017. If this is something that was covered by medicaid all we would need is a letter of creditable coverage and we can return the account to our client and request to have this deleted from the credit reporting agency. I she should have any more questions please feel free to contact me directly and I will be more then willing to help her through the process.

We have spoken with our client and they confirmed the address they received from the hospital was inaccurate as it was missing West as part of the address.  We sent several notices but never received them back so we consider the letters as delivered.  We have the account on hold pending...

When finding out that this was placed on my credit report illegally I immediately contacted OAC where I was informed by they're representive it would be removed from my credit report immediately. she asked that I speak with her supervisor as well which I agreed. Once the supervisor came on the telephone she immediately stated she will not be removing this item from my credit report despite providing her tracking number information of the Public Law 104-208 letter sent to her. I stated to her that my attorney located the letter and tracking information prior to my calling their company. she stated to me that since I am represented by an attorney she cannot speak with me. I stated to her that I am currently not, as I retained Attorney [redacted]. [redacted] to address this matter with his office mailing the letter. I asked that she not only speak with me further regarding this matter but stated to her I am not currently being represented by an attorney. She repeated on more than one occasion that since I am represented by an attorney she cannot speak with me concerning this account, as though she did not hear anything I said. I will be filing a complaint against OAC and its supervisor with the States Attorney General's Office Consumer Fraud Department, providing them with a copy of the letter sent to their office by attorney [redacted] including a copy of the tracking information sent by his office if this information is not removed from my credit report immediately.

Review: I received a bill with no explanation as to what the debt was supposedly being collected for. I had no prior bills from anyone that have not been payed. After contacting my doctor as well as my hospital and being told all of my bill were paid in full, I came to the conclusion that this company should be reported.Desired Settlement: I would like to know why I am being sent to collections without first receiving a bill, or any information as to why money is due.

Review: Months ago I began getting phone calls from an [redacted] of O.A.C. Collection Specialists claiming that I owed [redacted] for an emergency room visit I had in 2008. Ms. [redacted] claimed that I had not made a payment toward said bill since 2011 and that I still owed $503.00. I was skeptical that [redacted] would have forgotten to collect a debt for three years so I requested documentation of my initial charges and any payments that had been made. I also called [redacted] Hospital but was told at that time that they had no record of my visit, perhaps because so much time had elapsed. Over the ensuing weeks Ms. [redacted] would periodically call me asking to arrange payment and I kept telling her that I would make no payments without some solid evidence.

Eventually, I was sent something in writing. I received two sheets of paper. One was an apparent O.A.C. form letter which had, across the middle of the page, the following: Creditor [redacted], SC Account # -------- Regarding [redacted] Principal 503.00 Interest 0.00 Amt Owed 503.00. Underneath was a line referring to the aforementioned information as "THE ITEMIZED STATEMENT(S)" that I had been asking for. The second said [redacted],SC at the top, followed by my name, address and account number. [redacted] Hospital was listed as "Location". This paper said that the "Amt Due" was $643.00. I was now more confused, because of both the conflicting amounts and this being the first mention of a creditor other than [redacted] hospital. I again called the hospital and, thanks to the account number, was able to get some information. I was told that because I had no insurance at the time that my bill from them was entirely forgiven under what they called their Helping Hands program but that there could still be related charges if any tests had been done by an outside clinician. I remembered enrolling in Helping Hands but my recollection of the specifics of just how much the bill had been reduced was hazy. This explanation seemed plausible so I called [redacted]. The woman I talked to, after being given my account number, told me that my original bill was for $643.00 and that she had a record of only two payments being made: one for $30.00 in July of 2009 and one for $40.00 in December, also of 2009. She was unwilling to answer my questions about why [redacted] had waited so long to collect this debt or how paying $70.00 on a $643.00 bill reduces the balance to $503.00 rather that $573.00, saying only that the bill had been sent to collections.

By this time, I had looked into O.A.C. and found that they had seventeen Revdex.com complaints filed against them, some of which alleged attempting to collect previously settled bills. When Ms. [redacted] next called me seeking payment I told her that because if the numerous discrepancies in what I had been told regarding the timeline (did I make a single payment in 2011 or two in 2009? In either case, why would [redacted] make no attempt to contact me in the interim?), the amount in question (was it $503.00 or $643.00, or $573.00, an amount nobody had mentioned but that fit what I had been told by [redacted]'s employee?), and to whom the debt was supposedly owed (the initial claims that I owed [redacted] were, at best, misleading), combined with the allegations against O.A.C. itself I would not even consider making a payment until I could view my bank records from 2008 onwards. Weeks went by and on January 26th I got another call from Ms. [redacted] inquiring if I had been in touch with my bank. I had, but they have been remarkably slow in fulfilling my requests. Rather that drag the matter out further, however, I repeated to Ms. [redacted] all my previous red flags about the matter and told her I was unwilling to pay. She told told me that O.A.C. would continue to report the supposedly unsettled debt to the credit bureaus. Interestingly, to me at least, she did not mention taking me to court, which is what I would have expected from someone with a valid claim and evidence on her side.Desired Settlement: I would be satisfied with either of the following: I would like O.A.C. to present detailed, accurate, and consistent documentation of my supposed debt to [redacted], including the dates and amounts of all payments already made. In short the kind of evidence that would hold up in court, at which point I would feel comfortable paying. Alternatively, I would like them to admit that [redacted] and/or they were in error and to close this account, cease contacting me, and report to any and all credit bureaus that this matter should be stricken from any records concerning me.

Review: I was contacted regarding an account, which I am disputing. I have contacted my insurance company. They confirmed that they have paid everything in full. There are no additional charges. The charge that OAC is trying to collect for, my insurance company has told me was included in the EOB of what was already filed and paid.

They said since this is a preferred provider of my insurance company, that is all that is legally required and I also have documentation of that exact amount that I paid (and cleared), which I can provide after I receive the documentation of the charges that I requested from the company more than a week ago (3 Oct). At that time, they said they could have the charges out the next business day. However, it has been more than a week, and I followed up again today and they still do not have any documentation that they can mail me.

The company is now arguing that they still have to file additional charges (to a different company), which my insurance company is saying is untrue. However, they cannot produce any documents to back up the claims they are stating that I need to pay.Desired Settlement: I want something documented stating that I do not owe anything and that there has been no impact to my credit.

Review: It recently came to the attention of my husband & myself, that there was a large medical bill, in the amount of $4539.00, that had been sent to Collections on behalf of my husband. These were for services provided to him by a place called Innovative Pain Care Clinic back in May of 2014. After many hours, that turned into days, of trying to resolve this 'mistake', it came to light that the 3rd party billing entity, who works for Innovative Pain Care Clinic, did, in fact, have a completely INCORRECT address and phone number to not only my Employee Insurance (Anthem Blue Cross) BUT they also had a completely incorrect address & phone number for my husband & myself (the insured). Because they were not getting any response from the(incorrect)Insurance people OR from us(who never ever saw a single bill from them), they sent this medical bill on to a Collection Agency. I have been in touch with Wheaton Franciscan Hospital, Midwest Orthopedic Spclty Hosp the 2nd floor-which is a division of Wheaton Franciscan Hospital, Midwest Orthopedic Sports Medicine (my husbands actual Physician), Anthem Blue Cross (our insurance at the time), Innovative Pain Care Clinic (who was contracted to provide the services to my husband because of the nature of the procedure), the 3rd party Billing service who works for Innovative Pain Care Clinic, and of course, OAC Collection Agency-all in an effort to clear this matter up & get it off my husbands credit record. I have made many phone calls & left messages at this OAC Collections agency, who assured me that it had "been pulled", but because we are trying to apply for an auto loan, we are required to provide some proof to the bank that this was an error-not on our part & it should, subsequently, never even have gone to Collections. OAC has been completely Uncooperative in assisting me with this. According to the Billing entity for Innovative pain Care, they instructed OAC Collections to remove the claim& provide me w/documentation I am needing. It NEEDS to come from OACDesired Settlement: This error needs to be removed from my husbands (and subsequently mine) credit report immediately. Our lives, financially, have been put on hold & we can do nothing until this has been resolved. We need a loan for a car (ours broke down) & a new roof on our house & we will not be approved for any loan until this goes away. Even though MY credit rating is 784,and am responsibly paying off 2 other medical bills, I would NEVER have allowed this to happen. We also want a written apology.

We received this account for Mr. [redacted] on 9/2/2014 at which time a validation letter was sent out to the address the client had provided us from the consumers file. On 1/30/2015 we were informed the phone number we were provided with was incorrect. On 3/11/2016 Mr. and Mrs. [redacted] contacted us regarding this bill. Over many phone calls and three way communication with the insurance company the insurance informed us that the claim was denied due to an invalid id number. Over the course of a week we have received the correct id number and insurance billing information and have resubmitted the claim. The client has not pulled this from our office as it is a claim that is still due. When insurance pays their portion and the [redacted]'s pay the remaining balance we will be more then happy to agree to have this deleted from the credit reporting agencies.

Review: I filled bankruptcy on October 16th 2012 and this company was on the bankruptcy. This company is trying to collect for [redacted] and this was on the bankruptcy. This is still on my credit report under collections. I would like this item to be removed. When someone files bankruptcy these items are to be removed and this company still has it on my credit report.Desired Settlement: I would like this company to remove this from my credit report under collections right away. This shouldn't still be on my credit report.

Supposedly made numerous attempts to reach us. Never left a message on the VM and said they sent one mail notification which I never received. [redacted] was very unprofessional on the phone and actually threatened to report this to the credit bureau if I did not pay on the phone. Wanted to charge a 3% fee for using a cc instead of an electronic check.

Review: My son who is ** received a bill for $35.24 for Date of Service Unknown , Presumed to be from an ER visit in October and Already in Collections.

I am an authorized person on my Son's Healthcare,Hospital and Clinic and Bearer of His Insurance.

As a Full time College Student he is In School All Day or At work. Thus the reason he gave me the Pathology BILL.

I asked OAC after 10 min on Hold to pay bill and have the Date Of Service typed on Receipt and have it mailed to my son as I am paying with my Benny Card and it is Not On the Statement.

they exclaimed they could Not as My son is not available to authorize them to do so. I exclaimed that He should not need to authorize you to give him a DOS as it should be on Statement.

I asked to speak to the manager. After another 7 minute wait I was connected to" [redacted]" said [redacted], she stated that my request was undo able,

It was against my STATE's Statute's to even take a payment from me !"Until authorized by your son, we could not Discuss any Matters with you." I explained she is an extension of his already authorized Healthcare Release Of Information for me.

She said, WE ARE A COLLECTION AGENCY, I said, "FOR PATHOLOGY! , I want to make a PAYMENT, AND I WANT THE DATE OF SERVICE PUT ON RECEIPT AND MAILED TO MY SON" She repeated the same State Statute info again and refused to take a payment.

I told her That she could take a payment from me and her garble about the statute was a manipulation of the Law. I asked for her Name and she gave it to me Begrudgedly, I told her I would make a complaint. It is ridiculous!

TERRIBLE CUSTOMER SERVICE, UNBELIEVABLE MANIPULATION OF RULES, SHE WAS RUDE, ARROGANT, INCONSIDERATE AND SHADY BUSINESS PRACTICES THAT DON'T EVEN HAVE A DATE OF SERVICE FREELY LISTED.Desired Settlement: MY SON SHOULD NOT HAVE TO ASK FOR A DATE OF SERVICE. ALL I WANTED WAS TO PAY THE BILL AND HAVE THE DATE OF SERVICE PUT ON RECEIPT THAT THEY MAIL TO HIM SO HE CAN DISCLOSE TO "BENNY CARD" FOR VERIFICATION OF DEBT PAID.

Review: I have sent certified letter for settlement or validation of collection debt. The letter was sent sept.11 and sign for by [redacted] of oac on sept 12. I contacted oac via phone on oct 4 and spoke with erin anderson who acknowledged the letter and settlement ammount but said they don't settle and have not validated debt and refuse to send me the information. My certified letter included a response time frame of 10 days to settle or validate. It also included all of my current contact information including address phone and email so they have many ways of communication with me. [redacted] refused to acknowledge my contact information and said she did not have the letter even though she clearly quoted back to me the ammout I offered to settle for in the letter. Therefore it is my opinion I am being lied to in order to get more information from me then they need as all info was included already. Also was told [redacted] is not in office after asking to speak with him directly.Desired Settlement: I want a validation letter within 5 business day from them as they have had more then enough time to respond. Or, if they cannot validate debt remove it from credit report and send me a letter verifying its full removal.

Review: I recently did a credit report on myself and noticed a collection notice from OAC on 6/20/2014. OAC has never contacted me in writing explaining the amount owed, the creditor to whom I owe money nor given me any opportunity to have this disputed or resolved before going against my credit.

I requested proof of debt in writing from OAC and have not received any response. This collection is still on my credit report.Desired Settlement: Provide written proof of debt owed, give opportunity to dispute or resolve debt, remove collection notice from credit report.

I paid OAC (account #[redacted]) , and was assured it had not been and would not be reported to the credit bureaus. In August 2014 I discovered this was not the case, and I disputed their reporting with Equifax. OAC challenged my dispute, and refused to delete my account as promised.Desired Settlement: I would like my account to be deleted from the credit bureaus as promised at time of payment.

Review: Hello,

I had a emergency health issue requiring medical treatment back in February 2014 in [redacted]. which is not in the state I reside in. Although I was insured my provider failed to cover everything but $130 which I thought was to be paid and covered as I was admitted to the hospital at that time. While in the process of disputing the charge with my insurance company, my account was sent to OAC for collections on July 16th 2014 according to my annual credit report and it states as date reported. I paid the [redacted] directly with credit card after everything was settled with my [redacted] insurance and was assured it had not been and would not be reported to the credit bureaus. In Nov. 2014 I discovered this was not the case. I had not received anything or calls from OAC to my knowledge. I contacted OAC with my issue and was told nothing they can do other than state as paid. I never had contact with OAC and bill was paid to provider.Desired Settlement: I would like the credit collection being reported by OAC removed from all 3 major credit reporting agency's.

We received Mr. [redacted]'s account 7-16-14 and his validation notice was sent same day. We made several attempts to contact Mr. [redacted] by telephone but no phone contact was ever made.On 11-3-14 our client notified us that the account was paid in full. Mr. [redacted] contacted us via phone on 12-2-14 inquiring on the status of his account at which time we advised him the account was paid in full. He inquired about the account and his credit we advised him that the account would be reported as paid in full that accounts are not deleted because of the Fair Credit Reporting Act.

Review: I received a phone call from OAC in February 2014. This was the first contact I had from the company. They were claiming I owed approximately $140 towards a bill from [redacted]. I explained that this debt was paid in full directly to [redacted] in January 2014 (Check #706, dated 1/7/14, amount $139.76, cashed by [redacted] and cleared my [redacted] bank account 1/16/14) . I have explained this to OAC. After that phone call I received my monthly credit monitoring report that showed OAC reported an amount of $52 on Jan 2014/$88 in Feb 2014/$88 in March 2014 for [redacted]. I have had a ZERO balance with [redacted] since January 2014 (except for a new balance still pending insurance from an appt a couple weeks ago). I recently filed a dispute with Equifax on March 12, 2014 to have the account removed from my credit report, the results came back stating that OAC is claiming the amount is unpaid(as of March 27, 2014). IT IS PAID! I have NO statements from OAC. Again only received 1 phone call which they said they would clear it up with [redacted]. I would like OAC to report to the Credit Bureaus and have the debt REMOVED from my credit report. Not just listed as paid, but REMOVED. They did not appropriately contact me , they reported to the credit bureau before contacting me and AFTER I had paid the debt directly to the original creditor.Desired Settlement: I would like OAC to report to the Credit Bureaus and have the debt REMOVED from my credit report.
