Western Dental Services, Inc("WDS") is in receipt of the Revdex.com's inquiry regarding this caseWestern Dental sincerely apologizes for any customer service that failed to meet patient expectations and any misunderstanding that my have occurredDue to patient having State Insurance (Dentical) responsible party would need to contact them directly as to why patients' benefit were terminated prior to patient turning years old If a patient qualifies for Dentiacal benefits the State will continue paying for treatment until the age of years old Dentical does not notify WDS why benefits have terminated Responsible party has been informed that a request has been submitted to Dentical for approval of removal of braces Once Dentical approves removal and retainers patient will be contacted to schedule an appointment for removal of braces WDS will only render treatment to the patient that is approved by Dentical

Western Dental Services, Inc("WDS") is in receipt of the Revdex.com's inquiry regarding this case Please note that as indicated in our previous response to the Revdex.com, protecting member information is not only our company policy - federal law now requires it That is why we must first get a release of health information from our patient authorizing us to disclose any information regarding this case WDS requires that patient complete and sign the "Western Dental Services Authorization Form" As a result, WDS responded directly to the patient regarding this matter on 2/20/ However, as soon as the signed release is received by WDS, we will forward any pertinent information to the Revdex.com.The release form provided is blank and has not been completed by patient

Western Dental Services, Inc("WDS") appreciates the opportunity to address the concerns filed by the patient with the Business Bureau.Western Dental sincerely apologizes if there has been any confusion in regards to patient making a payment, in the initial complaint patient did not mentioned that a payment was made towards the account or that she was expecting a refund In the initial complaint patient disputed balance and account balance was addressed in the responseIn reference to the special x-rays, these special x-rays (Panographic Film) were necessary in order to address patients current dental condition Patient was given a diagnosis in requiring Upper and Lower Dentures, and in order to determine if there was any bone loss or other dental factors that could jeopardize the placement of new partials therefore special x-rays (Panographic film) was required As noted on the previous response, $is for treatment that was rendered and patient accepted and agreed to treatment per signed consent from patient In the initial complaint there was also no mention that patient requested an itemized statement, there is also no record that patient requested an itemized statement An itemized statement has attached for patient Patient was contacted directly on 5/7/to address the payment that she states was made Unfortunately, there was no answer and voicemail was full Patient may contact our Customer Service Department at 866-319-to provide proof of payment

Western Dental Services, Inc("WDS") appreciates the opportunity to address the concerns filed by the patient with the Business BureauAgain, Western Dental sincerely apologizes for any miscommunication or misunderstanding that may have occurred Patient's insurance was verified with his insurance carrier Based on the information provided by the insurance company patient was presented with an estimate for his treatment plan As noted in the estimator worksheet, insurance benefits are estimated and patient's actual insurance benefit patient share may be different The information provide by insurance company states that patient's eligibility and benefits will only be determined at the time a claim is processed by Principal Life Insurance Company Please note that insurance company also downgraded procedures, therefore the difference not covered by insurance company is patient's responsibilityExplanation of Benefits from patient's insurance company states that patient is responsible for $764.40, patient is being billed accordingly to his Explanation of Benefits Patient can contact his insurance company to obtain further details for the treatment that was downgraded and underpaid and his total patient share responsibility Patient will be mailed an itemized statement reflecting all treatment rendered and all payments applied to his account

I would like to add that on Saturday 03/28/2015, I ran into my former landlord and was told there was some correspondence and wnated to know if they were mineThe correspondnce was sent to my former address of bell avenue, Bell CA and they were under my dad's name [redacted] ***My father co-signed for me in for a treatment but did not co signed for me for the treatment rendered in I specifically made sure he wasn't includedWestern dental has my current address of miramonte blvd, which i've lived in since December I do not understand why they mailed collection notices to my old address and under my father's nameI have a refund check that was mailed to me on 07/14/to miramonte blvd los angeles ca This is proof that they had my current address and mistakenly mailed collection notices to the wrong addressI have copies of all the mailed correspondence as proof that I was not notifiedNow I fear this will affect my father's high credit score due to their negligenceI have asked them multiple times to remove me from collections since it is affecting my credit and they either hang up on me or tell me someone will follow upThis has cause emotional distress on my part since I've felt helpless as to what they choose to reportPlease view the attached claim information that states my due amount for the services, the check that was mailed to me by Western dental to my CURRENT address on 07/14/and the collection letters mailed to my old address that was addressed to my father [redacted] ***My father never agreed to cosign for these procedures and I am not a minor, there is no reason why these letters should of been sent to an old address, this violates my rights acording to the FCRA that states Creditor must provide a collection notificationWestern dental had my current address yet failed to sent the necesary information to meAlso, Please view the lettere I received from my previous insurance company that states that western dental over charged y copays in I've also attached the payment receipt from my bank where it states I paid western dental in advance for treatment rendered after 07/14/I will add the copy of my insurance fee schedule where you can match the copays to the procedure codesI've made their representatives aware that their system is charging more for copays than the allowed amounts but nothing was done to correct this

Attached please find the signed Authorization for Release of Health Information.I received two different messages from Revdex.com one stated the following but I did not receive a form where section of the release form has boxes to checkCan you please clarify whether the form I attached meet the necessary requirementsThank you, [redacted] "Please fill out and return the attached release form back to our officeThe business is unable to provide a response to your complaint unless we have this completed form on fileAll sections of the form must be filled out with a handwritten signature by the patient that received services (unless patient is a minor then the parent can sign)Sections & indicate who is able to use/receive your health information (service, billing, etcrelated to the dental treatment received), which would be the Revdex.comSection is the health information being used/discussed, which is the treatment/billing/service issue(s) you are filing a complaint aboutSection is the use of the health information, which is our Revdex.com complaint processSection is the duration of the release form, please check the 2nd box as there is not a specific end date for any complaint being processed with the Revdex.com."

Western Dental Services, Inc("WDS") appreciates the opportunity to address the concerns filed by the patient with the Business BureauWestern Dental sincerely apologizes for any miscommunication or misunderstanding that may have occurred When patient presented to the office insurance benefits were verified Based on the insurance verification patient was presented with an estimator worksheet for the treatment he was diagnosed by the attending dentist In the estimator worksheet it also states that insurance benefits are estimated and patient's actual patient share may be different, patient signed and agreed to treatment In the General Dental Benefit Summary from patient's insurance company it also states that patient's eligibility and benefits will only be determined at the time a claim is processed by Principal Life Insurance Company Insurance verification is not a guarantee of payment from the insurance company Patient may contact his insurance company for confirmation and verificationPatient is responsible for any insurance short fall Patient is encouraged to contact Western Dental to schedule a payment plan for the remaining balance Western Dental will work with payment plan that best works with the patient
