I am rejecting this response because: charges are not acceptable and not explained. Some services were not provided. Insurance carrier would have accepted and paid a negotiated rate up to 500.00 for services. Passing a full charge of 1600.00 to the customer is a crime. I had offered to pay the carrier negotiated rate and received no response. This is s pure example of how to take advantage of a general public and whats wrong with this system. My offer of 500.00 stands to resolve all issues. My next etep is to get the media involved. I am poeitive the negative publicity will only help to drive your unfair business away from this area. I am just joining the crowd of numerous unsatisfied local customers. To be continued...

American Medical Response has reviewed the consumer’s concerns on several occasions and determined their insurance carrier is a non-participating provider for services rendered in the state California. Unfortunately, if the insurance carrier is a non-participating provider, the consumer is responsible for any unpaid balance.  At this time, American Medical Response reached out to the insurance carrier along with provider relations, and requested the consumer’s ongoing issue be researched.  The insurance carrier indicated they would reprocess the claim accordingly.  American Medical Response contacted Bay Area Credit who confirmed that no credit reporting has occurred, and placed on a 30 day hold pending a response from the consumer’s insurance carrier. The consumer was contacted by a Patient Advocate and the concerns presented were forwarded to our Operations office for investigation. After reviewing all of the necessary documentation associated with the consumer's transport, Operations determined that appropriate care and transportation was provided. An ALS (Advanced Life Support) ambulance unit responded due to the consumer's condition, and provided ALS services during transport. Our ALS units have a paramedic on board with advanced training in the field of emergency medical care. Our crew followed all local EMS procedures and protocols.
AMR's rates are competitive for the communities we serve and meet all applicable local, state and federal limitations. Ambulance provider fees typically include a base charge for the transport, a mileage fee and any applicable ancillary charges. The charges incurred for the transport are correctly being billed with the appropriate procedure codes at an ALS level of service.

This case should not be closed. The company has not contacted me to resolve. In fact they continue to provide false info each time I call. I called again today because I've not received the form to get my medical records or received the callback and they said they would call me today and they have not. I've emailed and called Lori at Revdex.com to let her know it needs to be reopened and to call me. thanks

The Patient Advocate did contact me in August. At that time we discussed the fact that there was a fraudulent EKG service (I dislocated my shoulder) on the [redacted] and I also inquired about why this was an advanced life support charge for a dislocated shoulder set at the hospital. The advocate did not have answers for that and just tried to tell me that I needed to pay the [redacted]. We left that call in agreement that she would open an investigation with the operations team on the charges and get an answer on the EKG and Advance life support and contact me back. She still has not done that to date. I've also still to date not received the form for my medical records. I've called back twice after the August call and been told I would be set up for a call back both of which never happened. One time the billing group said the advocate had tried to contact me but that I was not answering. I asked for details on the number and times called and they said it could not be shared. I've not been called. I [redacted] try [redacted] at the number she provided today but I've still not received any forms, any answers on the EKG, or any answers on the ALS charge. Until I do, I'd like this case to remain open. Offering a 20% discount on fraudulent charges is hardly resolution. The AMR [redacted] is more expensive than my ER stay at the Mayo clinic to get my arm set.

Based on the consumer's rebuttal of the AMR response dated September 21, 2015, the consumer was contacted by a Patient Advocate on September 30, 2015, and requested an additional review by our Operations office. Operations contacted the consumer and went over the scope of practice, billing rates and procedures, confirming that the charges are correctly being billed. The consumer accepted a 40% discount, along with a payment arrangement on the new balance owing.
A letter has been mailed to the consumer outlining the discount and agreed upon payment arrangement. Should the consumer have further questions or concerns, I may be contacted directly at (XXX) XXX-XXXX.

American Medical Response billed the consumer's insurance, [redacted] through Allegiance Benefit Plan, on February 24, 2015. Due to AMR being non-contracted with the insurance carrier, any unpaid balance would remain the consumer's responsibility. On March 02, 2015, a letter of no contract was mailed to the consumer informing him of this issue. As [redacted] applied a discount to the charges in error, AMR appealed the claim requesting payment in full from the insurance carrier. [redacted] response to the appeal was that no additional payment would be provided. Therefore, the patient is responsible for the balance. It is suggested that the consumer contact his insurance carrier for a direct appeal process if he feels that a larger portion of this claim should be covered. At this time, the consumer's account reflects a balance of $176.66 which he is responsible for. Should the consumer need assistance in paying the balance, American Medical Response offers both discount and payment plan options.

Upon receiving insurance information, the collection agency obtained payment from the insurance carrier. I accept the response because it's the correct outcome. I do not accept the business strategy of sending one of your customers who has by definition of needing an ambulance already suffered a trauma to collections without lifting a finger to determine the billing address. The ER attending physician figured it out, AMR can too. It's a despicable practice and clearly something AMR does all the time given the number of similar complaints.

                        Revdex.com Case#:  [redacted]  Dear Revdex.com: Thank you for taking the time to contact our office with the consumer’s concerns.  American Medical Response (AMR) values our customers, and strives to provide quality service.  AMR reviewed the consumer’s account and determined billing is appropriate based on the rates in the area where services were rendered.  Our rates are competitive for the consumer’s community and meet all applicable local, state and federal limitations.   AMR may provide reduced cost health care services to patients who are uninsured or underinsured, and able to provide documentation of hardship.  A Compassionate Care Program application was mailed to the consumer on July 6, 2017.  Unfortunately, AMR did not receive the application or required documentation the consumer indicates was provided in August 2017.  As a result, AMR provided the consumer the opportunity to reapply for the program, mailing several applications over a 5 month period, and also attempting to contact the consumer via phone, but were unsuccessful.  In January 2018, contact was made with the consumer, who was advised to provide the documentation via email.  Based off the documentation received, the application was denied due to the consumer’s income exceeding the program’s guidelines.  A determination letter was mailed to the consumer on February 9, 2018.  At this time, a 20% discount off the remaining balance has been applied.  Should the consumer have any further questions or concerns or would like to establish a payment plan for the balance due, I may be contacted directly at (209) 236-8358. Sincerely,    Cecilia J[redacted]Patient AdvocateAmerican Medical Response

                        Revdex.com Case#:  [redacted]     Dear Revdex.com:   Thank you for taking the time to contact our office with the consumer’s concerns.  American Medical Response (AMR) values our customers, and strives to provide quality service.   AMR transported the consumer for continued care at the request of the attending physician.  Based on the patient’s condition at the time of service, the patient required ongoing care or monitoring by an RN or Paramedic with training beyond ALS level of care. Therefore, charges incurred for this transport are correctly being billed.  A claim was submitted to the consumer’s insurance carrier, and payment of $6039.25 was issued.  The remaining balance of $1315.75 was applied towards the consumer’s deductible, and billed accordingly to him.   At this time, a refund will not be issued, and we will continue to review the consumer’s account.
