First let me apologize to *** for any inconvenience or confusion we may have caused.
"margin: 0in 0in 10pt;" class="**oNormal">She has attempted to obtain documents from us evidencing the transaction her late husband entered into with our company. The copy of the annuity policy that we have on record does not list who the beneficiary of the policy is, however according to *** it is her two children, who I understand are both adults. If that is the case one or both of her children would have to make the request for the documents or they would have to execute an authorization allowing *** to seek the documents on their behalf. Such authorization would have to be signed by one or both of her children and notarized
Before any of this can take place however we need proof that the children are the beneficiaries of the annuity policy. We were informed by *** that they do in fact have a letter or something from the annuity company stating that her children are the beneficiaries. This document will have to be supplied to us
If any of these facts are incorrect please let us know
We are not trying to be difficult, but I think *** can appreciate that we can only release this information to the person(s) legally entitled to have same

We do not have a $rewards card program. He was a prospective customer who we sent a $*** Card promotion in early November. I have reviewed the contemporaneous notes and have spoken to the manager, but I don’t see any mention in the notes about him inquiring about how to redeem the card while we were completing his transaction, i.ebefore funding. He called in about the $promotion on January 31, 2014, after his transaction was funded, and again on February 13th and finally as recently as this past Wednesday, February 26th
We will make good on the $promotion card if that will satisfy *** ***, please advise
