reimbursement to the insuredsPlease note that the plan *** *** purchased on behalf of her mother and brother contains a listing of specified reasons making one eligible for coverage under the Trip Cancellation benefit. However, the plan requires that the reason for cancellation occur after the effective date of coverageImportant: The covered reason for cancellation or interruption of your Covered Trip must occur after your effective date of coverageBased on the information originally received, the effective date of coverage for the booking in question was 12/**/15. However, the date of death of *** *** sister was the day prior, 12/**/15. As such, the reason for the cancellation, the death of *** *** sister, was not a covered reason under the Plan, as per the above requirement. However, upon receipt of the additional information *** *** included with her complaint, in particular the timeline of events, we do now understand the particulars of this situation. While the protection plan was indeed purchased after *** *** sister’s death, we have agreed to honor the claim and provide the requested reimbursement to the insureds. Accordingly, payment to *** *** and *** ***, in the amount of $per person, will be forthcomingWe trust we have responded to the concerns raised in *** *** complaint. We have reviewed *** ***’s complaint and we are responding accordingly. In his complaint, *** *** states that he is unable to travel due to his own illness. The plan *** *** purchased does provide coverage for a cancelled trip due to one of the specified reasons
contained in the plan, both medical and non-medical. However, in order to confirm that the reason for cancellation meets the plan’s terms and conditions, we do require sufficient documentation of such situation. Unfortunately, the limited information *** *** has provided to date does not afford us the opportunity to review his eligibility for coverage As such, as indicated in our correspondence to *** *** of July **, 2017, we do require verification of the nature and onset of this illness in order to confirm the reason for cancellation meets with the plan requirements. Accordingly, we forwarded *** *** an Attending Physician Statement that should be completed by his treating physician. Once we are in receipt of this documentation we will be happy to continue our review of *** ***’s claim. Please note that the plan in its entirety is provided prior to purchase for review. Further, the confirmation provided to *** *** contained a link to the plan, including the specified reasons for cancellation as well as the contact information for our office for any questions regarding the plan We trust this has addressed the concerns raised in *** ***’s correspondence. We have reviewed *** ***’ additional comments and we are responding accordingly. We note that *** *** indicates in her complaint that she emailed our office to advise that if we require documentation from a physician, she can provide a letter from her cousin with whom she stayed during her trip. Please note that the email *** *** sent to our office advised that as she was out of town, her only option was to bring her son to the emergency room in the middle of the night and that she decided against doing so. If *** *** did in fact have her son undergo an examination by her cousin, a physician, at the time of the interruption of their trip, and received treatment from that physician, she can certainly have the physician send a letter to our office, on letterhead, confirming the examination, the diagnosis made and the date of the examination, as well as indicating the treatment provided. Upon receipt of such documentation, we would be happy to again review *** ***’ claimAttached is a copy of *** ***’ email for your reference We trust this has responded to the additional comments from *** ***. We have reviewed *** ***’s additional comments and are responding accordingly As previously advised, in order to qualify for coverage for a trip cancellation due to an illness, sufficient information must be received to confirm that such reason meets the plan’s terms and conditions. In order to do so, we are awaiting the completed Attending Physician Statement sent to *** *** previously. Unfortunately, while we understand that *** *** submitted documentation from his physician, the information provided only confirms he was excused from work on July **, and released to return to work on July 26, 2017. As previously noted, in order to make a determination on this claim, we need to confirm the onset date of the illness which prevented him from work as well as the nature of such illness, to affirm such condition meets the plan’s terms and conditions, and does not fall under any of the plan exclusions. As previously noted, once we are in receipt of this documentation we will be happy to continue our review of *** ***’s claim. We trust this has addressed the comments from *** ***. *** *** is a liarI never made any claim that either the two *** banks that I visited or the ACE check cashing service store stated that because this check was over $that is was not cash ableThose three places simply stated that this check was non-cashable regardless of its amount.I never asked that a replacement check be issued to me.I court, I will be asking for *** *** to prove her fraudulent claims either in writing or via recorded phone conversations.I am not allowed to open any bank account, and I am disabled (which will also be proven in court) and cannot visit any more banks or check cashing places, especially considering that I have already been denied at three of them.My next move will be to sue this company, along with reporting them to all applicable social and news media outlets *** ***

We have reviewed *** ***’s complaint and we are responding accordingly. In her complaint, *** *** states that she submitted the information needed regarding her cancellation and she received no response from our office. Please note that we responded to each of
*** ***’s email inquiries within the standard time frame, the first business days following their receipt. Our initial response to *** *** was to advise that the documentation she submitted was not sufficient to confirm the medical basis of her claim as it did not indicate the onset date, treatment dates or diagnosis of such condition. We advised, both via correspondence and by telephone, that additional documentation was needed and requested that she provide such verification from her physician. Following receipt of this complaint we reached out to *** *** on January *** to again explain that the letter she had submitted did not provide the full information needed to review her claim and *** *** did then agree to obtain same from her physician and submit it to us for review I am pleased to advise that this information was received and reviewed and *** ***’s claim has been approved for payment. Payment in the amount of $is being sent to *** *** directly We trust this has addressed the concerns raised in *** ***’s correspondence. We are in receipt of the complaint submitted by *** *** and are responding accordingly. As per the information originally provided, *** *** cancelled her Carnival Cruise due to concerns related to the Zika virus and her pregnancy The Plan *** *** and her family purchased
provides a reimbursement as a result of a trip cancellation in the event of certain risks, both non-medical and medical. One such specified medical reason is a cancellation due to a Sickness. Sickness is defined in the Plan as follows: Sickness means an illness or disease of the body which: 1) requires examination and treatment by a Physician, and 2) commences while the plan is in effectAn illness or disease of the body which first manifests itself and then worsens or becomes acute prior to the effective date of this plan is not a Sickness as defined herein and is not covered by the plan The Plan also contains specific exclusions to the eligibility for coverage. One such exclusion states the following: General Plan Exclusions In Parts A & B: We will not pay for any loss under the plan, caused by or incurred resulting from: pregnancy, except if hospitalized; or elective abortion; From the information originally received from *** *** and her physician, there was no indication that *** *** was suffering from a Sickness, as defined under the Plan, at the time of her cancellation. Rather, we understand that the reason for *** ***’s decision to cancel her travel plans was due to her pregnancy and the concerns for the potential of an illness occurring in relation to the Zika virus. While we certainly can appreciate and empathize with *** ***’s decision to cancel the trip, unfortunately, as the reason for such cancellation, namely the concerns for the potential of contracting an illness, is not a specified reason under the Plan, we were unable to consider her request for reimbursement. However, upon receipt of this complaint, we did reach out to *** *** to discuss the matter. *** *** advised that she in fact experienced a complication with her pregnancy and indicated that was the basis of the decision to cancel the trip. In order to allow us to further review *** ***’s claim, it is necessary to obtain additional medical documentation from *** ***’s physician. Accordingly, we are currently working with *** ***’s physicians, *** *** and *** ***, to secure this additional necessary documentation. Once received, we will be able to further review *** ***’s claim to determine if her reason for cancellation does in fact fall within the bounds of the plan purchased. We anticipate receipt of this information shortly. We will notify *** *** of our final determination at that time Please note that the Plan in its entirety, including all specified reasons for cancellation, as well as all specific exclusions to the eligibility for coverage, are provided to the participant prior to and concurrent with their purchase of the plan. We are in receipt of the complaint filed by *** *** and are responding accordingly. Shortly after issuance, *** *** reached out to our office on September **, to advise that the claim payments provided on September **, to he and his wife had not been receivedWe
reaffirmed the address and date of payment and advised if still not received after days, to contact us. In accordance with *** ***’s follow up call to our office on October **, 2017, we initiated stop payment and void procedures for the ***’ checksUpon completion of this process, we reissued the check for *** *** but received notice that the check for *** *** had in fact been cashed. Accordingly, we reached out several times to the *** to advise of the impending reissuance to *** *** and the status of *** ***’s check. However, we did not receive a return call from the *** We did proceed to forward *** ***’s check on October **, 2017, together with the status information regarding *** ***’s check. This check and letter was sent to *** *** for delivery this week. As reissuance of *** ***’s original, non-negotiated check has been completed, we believe this matter to be concluded. We have reviewed *** ***’s complaint and we are responding accordingly *** *** and his traveling companion initiated a Trip Cancellation claim with our office on September *, 2017. The reason noted for this cancellation was “Due to the projected path of Hurricane Irma to
strike our destination, the Bahamas, making air travel and staying on the island unsafe.” Based on the stated reason for cancellation, which is not one of the specified reasons for a cancellation under the plan purchased, we notified *** *** and his companion of the denial of their request for reimbursement later that same day. We have received no further communication from *** ***, either via telephone or email, requesting a new review of this claim, now noting a medical reason for cancellation. Please note, though *** *** contends that we have refused to reply to his emails requesting to proceed with this claim as a medical cancellation, after an exhaustive search, we have found no such emails received from *** *** at the email address we have on file, and which was indicated in the subject complaint. Further, we have reached out by telephone to *** *** to discuss same, and have not received a return call At this time, please note that should *** *** wish to have his claim reevaluated on this new basis, he is more than welcome to do so. Under separate cover we have taken the liberty to send a claim form to *** *** for his completion, as well as completion of the Attending Physician’s Statement by his treating physician. Upon receipt of same, we will be happy to further review *** ***’s claim on this new basis. We trust that we have addressed the concerns raised in *** ***’s complaint. We have reviewed *** *** complaint regarding his pending claim for Medical Expense benefits. Please note that at the time this complaint was received, *** *** claim had already been reviewed and was in line for payment. I am pleased to advise that such payment, in the
amount of $6,015.76, was sent to *** *** on December *, 2016. We are sorry that the claims process was longer than *** *** had anticipated. We have reviewed *** ***’s comments with regard to our response, in particular that there is no clear statement in the plan that states her reason is not covered Please understand that the plan *** *** purchased is a named peril plan. As such, only the named perils contained in the plan are covered reasons for cancellation. More particularly, this plan contains coverage in the event of an illness, injury or death of a traveler, immediate family member or business partner. The plan also contains a number of non-medical perils which would make one eligible for coverage under the plan. The following are the non-medical named perils for a cancellation, contained under the section entitled Other Covered Events: Other Covered Events means only the following unforeseeable events or their consequences which occur while coverage is in effect under this Policy: a change in plans by you, an Immediate Family Member traveling with you, or Traveling Companion resulting from one of the following events which occurs while coverage is in effect under this Policy: (a) being directly involved in a documented traffic accident while en route to departure; (b) being hijacked, Quarantined, required to serve on a jury, or required by a court order to appear as a witness in a legal action, provided you, an Immediate Family Member traveling with you or a Traveling Companion is not: 1) a party to the legal action, or 2) appearing as a law enforcement officer; (c) having your Home made uninhabitable by fire, flood, volcano, earthquake, hurricane or other natural disaster; (d) Your involuntary termination of employment or layoff which occurs after your effective date of coverageYou must have been continuously employed with the same employer for year prior to the termination or layoffThis provision is not applicable to temporary employment, independent contractors or self- employed persons As *** ***’s reason for cancellation of her flight, namely the cancellation of the event for which she had purchased the ticket to attend, is not among the named perils contained in the plan, she is not eligible for reimbursement under this plan and no payment shall be forthcoming Thank you for the opportunity to further clarify the basis of the denial of *** ***’s request for reimbursement. We have reviewed *** *** complaint and we are responding accordingly. Please note that though *** *** filed a claim with our office on or about August *, 2015, with a claim form sent to her for completion on August ***, we have not received back a completed claim form or any
supporting documentation from *** *** to date. Accordingly, we are only able to speak to the information contained in her complaint It appears from her complaint that *** *** had a need to change her original outbound flight on August *** to a different flight later that day. However, it appears from her complaint that she had some issues with the airline and the booking agency regarding such change. We did reach out to Hotwire to determine if *** *** had changed her airline ticket and we confirmed with them that *** *** had in fact used her full outbound ticket on August ***, as well as using and/or exchanging her return ticket on August *** As *** *** used the airline ticket in question, and as we have not received any other information from *** *** to date, we are uncertain what it is she is seeking to claim under this insurance. If she did not in fact make use of her ticket, and this was due to a specified reason in the plan, she should feel free to submit her completed claim form and supporting documentation to us for review. However, until such time, we are unable to consider any type of reimbursement to *** *** for any issues she may have encountered when requesting to change the time of her flight We trust we have responded to the concerns raised in *** *** complaint. We are in receipt of the complaint filed by *** *** and are responding accordingly. *** *** initiated two separate claims on behalf of her traveling party. One claim for her mother, *** ***, who was traveling on one itinerary, and one for herself and two others
traveling on a separate itinerary. *** *** and her companions’ claim was reviewed and payment went out to them on June *, 2017. *** ***’s claim was reviewed and payment was sent to her on June **, 2017. *** *** contacted our office to advise that she and her companion on the second itinerary had not yet received our payment, having apparently been sent to an incorrect address. We then began the process to void those checks in order to reissue new ones. Upon approval of the void on June **, 2017, these replacement checks were requested for reissuance to *** *** and her traveling companions and were sent on July *, 2017, the first payment issuance following such void We trust that *** *** and her companions have received such payment at this time. If there is any issue with the receipt of this payment, we request *** *** contact us to review As this claim is now settled, this will conclude this matter. We have reviewed *** *** complaint and we are responding accordingly. In her claim documentation, *** *** advised that she cancelled her scheduled trip due to a cancelled court appearance. Unfortunately, this reason for cancellation is not one covered under the
plan. Please understand that the plan purchased provides reimbursement in the event of a cancellation due to a number of both medical and non-medical reasons as specified in the plan. However, cancellation due to a postponed or cancelled court appearance is not among the specified reasons in the plan. As such, we are unable to consider *** *** request for reimbursement Please note that the plan in its entirety, including the full listing of all specified reasons making one eligible for reimbursement is provided to the traveler prior to and concurrent with the purchase. However, we understand from *** *** complaint that there was apparently some confusion regarding the information provided during *** *** claims initiation. As such, solely as a goodwill gesture, we have agreed to make an accommodation to *** *** and provide the applicable cash reimbursement. As such payment is currently being provided under separate cover to *** *** As this claim has now been settled we trust this will conclude this matter.
