I have looked into this for *** *** and according to our records on 11/8/*** *** was advised she may submit a membership cancellation request by visiting our online support desk *** *** was instructed to visit *** select "Cancel Membership" and agree to
the cancellation terms & conditions Please note, the cancellation terms & conditions stated on our online support desk are the cancellation terms & conditions also stated in *** ***'s signed WoW membership agreement If *** *** would like to submit a membership cancellation, she may do so as previously instructed If she has any further questions, she may submit a request to support.workoutworld.com and select the approproate category for her request

According to our records, on 1/21/*** *** enrolled under a membership agreement with an initial term of 12-months. Additionally, on 1/23/*** *** enrolled under a personal training agreement with an initial term of 12-months. As stated in each of *** ***'s agreements with
us, the initial term must be complete before her agreements can be canceled. In the instance that *** *** is deemed permanently disabled and the disability is fully described and confirmed by a physician, *** *** may cancel prior to the completion of the initial term and will be billed just 10% of her total contract price, as described in her signed agreements with us. On 4/4/*** *** submitted a note from her physician stating that she may not use the gym for a period of 6-months. Upon receipt of the doctors note, *** ***'s membership and personal training agreements were placed on a Medical Freeze. She is set to be billed our Medical Freeze rate of $+ sales tax, per month of the freeze. I would like to make *** *** aware that any months her membership & personal training are on freeze will not count towards the initial term of her agreements with us. Because *** *** is concerned that she may not be able to use the club and her personal training at the end of the 6-month period, I have extended the freeze for an additional 6-months. If *** *** is unable to use her personal training and/or the gym at the end of the 12-month freeze, she may submit an additional request to extend the freezeIf *** *** is able to come back prior to the end of the freeze, she may simply check in at the front desk of her local JERSEY STRONG location to end the freeze. All of the sessions *** *** paid for prior to the freeze will be available for her use when she is able to return to the club. The expiration date on her sessions can be extended so that she has the opportunity to use the sessions that she paid for. We are sorry to hear of *** ***'s accident and wish her a speedy recovery. Due to this situation, we have made an exception to back freeze her membership for the month of April and waive the past due balance and her Annual Membership Fee, billed on 4/1/2018. I apologize for any confusion caused by our prior response. On 9/6/*** *** enrolled under a personal training agreement that included half hour training session per month, billed at $69.99/ monthThis agreement was canceled after the 1st month's payment and on 10/10/*** *** upgraded her personal training to a training package that included personal training sessions per month, billed at $180. *** *** has made payments of $180. For each of the payments made, *** *** was issued personal training sessions, for a total of sessions. *** *** has used of the sessions issued for her current personal training packageAt no time was *** *** double billed for her personal training. According to our records, *** *** has paid off the remaining months of her membership initial term; however, she is currently responsible for more months on her personal training initial term. As stated in my prior response, we will be happy to make an exception for *** *** to waive the remaining months of her initial personal training term upon receipt of a one-time cancellation fee in the amount of $199. I have noted *** *** account of this exception and she may pay the cancellation fee at her local JERSEY STRONG club or over the phone by calling ***

term On 3/5/we received a written 3-day cancellation request for *** *** Personal Training agreement *** *** training agreement was cancelled accordingly and a refund check, in the amount of $was issued for the initial personal training payment made on 3/5/16. An additional written 3-day cancellation request was received for *** *** WoW membership account The membership 3-day cancellation notice was dated 3/7/ *** *** was notified through our online Support Desk that she was no longer within the hour cancellation period and therefore, not eligible to cancel her membership until the completion of the initial term *** *** was advised that her training and membership agreements were available immediatly after signing, by visiting our website at *** As a paperless company, we do not print the agreement at the time of enrollment.*** *** WoW membership is set to be cancelled on 2/28/17, upon completion of the initial term The last payment for *** *** membership will be on 2/1/ She will have full access to enjoy the facility up to and including 2/28/ We hope she comes in today to enjoy her membership!

We have looked into this for *** *** and according to our records on 10/27/she enrolled in a membership with an initial term of 12-months*** *** membership cannot be canceled until the initial term has been completed. As stated in *** *** agreement with us, any
months that her membership is on freeze will not count towards the initial term of the agreement. Although we do not normally do this, we will make an exception to cancel *** *** agreement prior to the completion of the initial term, upon receipt of an early cancellation fee in the amount of $+ sales tax. We will even go one-step further and credit the cancellation fee for the Annual Membership Fee and Freeze Fees *** *** has paid. This brings the early cancellation fee down to $+ sales tax. We have looked into this for *** *** and according to our records, on 2/15/she enrolled under a membership with an initial term of 12-months. Any months during the initial term that *** *** placed her membership on freeze will not count towards the initial term of the
agreement. *** *** membership was on freeze beginning 6/1/until 9/30/which is a total of months. *** *** membership cancellation cannot take effect until the initial term is complete. At this time, *** *** has completed a total of months of her initial term. A cancellation has been applied for the end of the initial term. *** *** membership is set to be cancelled on 6/30/2018. We have notified *** *** of our Medical Freeze policy, allowing her to freeze her membership at just $+ tax, until further notice. If *** *** would like to freeze her membership she may do so through our online support desk. If *** *** would like to submit documentation from her healthcare provider we will be happy to further review her request for a medical cancellation

I have thoroughly looked into this for *** *** and on 11/13/our office was in receipt of a request to cancel her WoW membership account There is no record of a cancellation submitted prior to 11/13/ *** ***'s WoW membership was cancelled accordingly on 11/30/
According to our records, on 6/15/WoW was notified of a credit card dispute filed for *** ***'s October and November membership dues in the amount of $each As *** ***'s membership was active at that time, she is responsible for payment of the charges billedThe outstanding balance of $was sent to collections We have confirmed with the collections agency that *** *** has paid the balance in full and our records have been updated accordingly As stated in *** ***'s agreement with us, upon completion of the initial term, her membership will continue on a month-to-month basis until a written cancellation was received As a request to cancel was not received prior to November 2015, *** ***'s membership remained active and she is responsible for payment of her membership dues through 11/30/ At this time, no refund is due We would be happy to give *** *** a month complimentary membership so that she may use the months she paid for, plus one extra month! beginning 8/01/- 5/31/A balance accrued on ** *** WoW membership account as of 6/01/until 10/01/when the account was adjusted to be sent to collections. information from our members. The initial term of *** ***'s membership is clearly disclosed in several locations on the first page of her agreement. However, we will take *** ***'s word that she was not aware of the initial term. If *** *** enrolled under a month-to-month membership during June 2017, she would have been billed for a $+ sales tax Enrollment fee and her Annual Membership Fee of $+ sales tax, on month #of her membership. Upon receipt of a payment for the $Enrollment Fee and $Annual Membership Fee, we will make an exception to convert *** ***'s membership to a month-to-month agreement, allowing her to cancel her membership and waive the remaining initial term of her agreement. We hope this helps!We will even go one step further! If *** *** would like to proceed with this adjustment, we will hold a one-month complimentary membership for her to use at a later date.
