The account in question was placed with our office on January 1st, 2016 by our client, [redacted]. We thank you for the provided documentation this assisted us in locating the correct Explanation of Benefits through the [redacted] online portal reflecting the zero balance mentioned. Our offices have now forwarded these documents to the facility and requested the correct adjustment of the balance on the account.
