Aspen Dental branded practice owned and operated by [redacted] DMD Holdings, PLLC. I certainly do apologize for any inconvenience.I have received your complaint filed with the Revdex.com. T have performed an audit on your Aspen Dental account and have found that your patient responsibility for completed treatment is $337.80. I have reviewed this matter with the Aspen Dental Office Manager, [redacted]. The total cost of your treatment is $1,343.00. and a 25% coupon discount of $335.75 was applied. That amount is pending a final response from your dental insurance for the open claim. As a courtesy I will refund the estimated credit to you now; however please note we are awaiting payment on the claim for your crown. She never told me that they were charge the whole $3000 right then and there. They should never charge the credit card before services were even started. I feel like I really was taken advantage

To further explain. What the company should do is fix what they broke in my mouth, not a hard or unreasonable request to make since they fired the Dr. who messed up. From a company stand point it would be easier to except that small loss, rather than what I suggested before malpractice suit. Their earlier response was disrespectful in pointing the blame on me and a slap in the face with taking off the $50 that I already spent with them the $500 bill that is needed to fix what they broke. If they do not, I will see them in court for whatever their highest liability + professional + umbrella limit coverage is for.

Your scheduling people did not say it was a starting price. Still need to get adjustments to my top plate I got from you but as soon as I paid for them you seem to not have apts that are reasonable for me, as you did before I paid. Then the apts I do make don't ever seem to make it to your computer so I have to sit and wait for hours

To whom it may concern:Mr. [redacted] has spoken with the Quincy, IL Aspen Dental Office Manager, the Regional Manager and myself (on 4/28/17). It has been explained to him each time that discounts offered towards treatment can "not" be combined. His original treatment estimate included a 20% discount off the usual and customary fees. He decided not to move forward with treatment. Later he received a coupon flyer offering a 25% discount off our usual and customary fees. He thought he could combine the two discounts. It was explained to him on more than on occasion that he could not combine the discounts, but that the dental office would give him the greater of the two. He is not accepting that response. There is nothing further that can be explained or responded to in this inquiry. I have apologized for any misunderstanding. The discounts will not be combined as it is not the dental practice's policy to do so.Sincerely,[redacted]Senior Patient Satisfaction Specialist Aspen Dental Management, Inc.Phone: [redacted]Fax: [redacted]E-mail: [redacted]@aspendental.com

From: Byron S[redacted] Sent: Thursday, April 14, 2016 4:56 PMTo: [redacted]@clevealnd.Revdex.com.orgCc: [redacted] Dr. Andrei P[redacted]; [redacted] Dr. Murali L[redacted]Subject: Aspen Dental Response #2

Per our conversation, below is a description of the case with ID [redacted]...

[redacted].

The patient originally agreed to get her hygiene treatment, crowns, extractions and both partial upper and partial lower dentures. In July of 2015. As the patient states she received her hygiene treatment and the Oral B tooth brush which is part of that treatment. She later stated that she had no idea that she had to pay for the Tooth brush, to which she was told that she could return it in the original box for a full refund. When she stated that she no longer had the box, as a courtesy we still refunded the balance for the brush to the finance company.

The patient stated that the crowns that were done were too short and had to be redone. The patient had extractions done and the crowns were around the extraction areas. After healing, the gums subsided around those crowns exposing more tooth structure, which made them more sensitive. So Dr. P[redacted] remade those crowns to the gum line to take care of the patients sensitivity issue.

When the patient came in for her crowns, an Associate Doctor was at the office substituting for Dr. P[redacted]. She inadvertently pinched the patients lip during the process causing it to bleed. At this point the patient decided that she no longer wanted to see any other doctor than Dr. P[redacted] and also made the decision to not move forward with any work on her lower teeth. She only wished to finish the work on her upper teeth that had been started.

The patient was told that the entire package for the partial was included in what she paid by the Office Manager at the time. This does include a separate charge for the temporary and the permanent partial dentures. The patient was told a total amount for both sets, and given a Treatment Acceptance form that has a line-by-line accounting of every charge. The patient signed that agreement and also had a copy to take home. All costs for her entire treatment that the patient desired were outlined in this agreement that she signed. When the patient decided to not go forward with further treatment, she was refunded the amount for the work that she would not complete. With most insurances, the insurance does not actually cover the entire amount of a given procedure. The insurance pays for a portion and the patient must pay for the remaining balance. The patients insurance was only billed the contracted amount for work that the patient did receive. There is no balance to be refunded further either to Dental First (the financing company) or the insurance because the patient has only been billed, and has only paid for, dental work that has been completed.  I wanted to make sure this was added: After the patient had decided that she wanted a refund and had these issues I personally sat with the patient and went over every issue with her, including the costs and what would be refunded and why. After many attempts she stated she still did not understand why she could not get a larger refund and would be taking the issue up with the Revdex.com.  Byron S[redacted]OM-Brooklyn, Ohio7351 Northcliff AveBrooklyn, Ohio 44144Phone: 216-661-8077Fax: 216-661-8022

about the treatment you received. I am writing to you on behalf of the Stow, OH Aspen Dental branded practice, owned and operated by James P Maurer DDS Inc. I certainly do apologize for your experience at the Stow, OH Aspen Dental office. I am in receipt of your complaint filed with the Revdex.com. I have reviewed it along with your Aspen Dental account. You were not charged for the service to re-cement your crown. You were charged for the exam and x-ray. Your last visit to the dental office prior to November 22, 2016, was on October 14, 2015 (13 months). The estimate for the visit on November 22, 2016 was that your dental insurance would cover it at 100%. They did not; therefore the balance due of $72.00 remained. As a courtesy, I have adjusted off $50.00 from your balance due. That amount reflects the estimated provider discount that we did not take based on the response from your dental insurance company Ameritas Life. Your new balance due is $22.00. Please contact your dental insurance carrier to confirm the charges and payments in question. I appreciate you giving me the opportunity to respond to your inquiry. A copy of this letter has been forwarded to the Revdex.com in response to your complaint. I am available for any further questions at the below contact information. Revdex.com:
I have reviewed the response made by the business in reference to complaint ID [redacted], and have determined that this does not resolve my complaint.  For your reference, details of the offer I reviewed appear below. If they are charging for the correct sedation code, they are saying they gave an 18 yo kid a sedation that was not necessary or needed for his wisdom teeth removal. And if they allow their dentists to give a sedation that is not covered under the New York State Healthy NY plan, then they should have to the pay the $900. I was present before and during the procedure, they never told me that they were going to give him anything but the minimal sedation needed for a basic wisdom tooth removal. This is NOT an issue with Excellus, Excellus would cover the basic sedation needed for wisdom teeth removal, and Aspen dental was fully aware that if they gave an 18 yo kid a deep sedation that it would not be covered by the New York State Healthy NY plan.

[redacted] sold me a defective product which if he was a responcible business owner, he would have replaced the deck. It was improperly installed, started cracking within 3 months time and had a U shaped piece of wood grain sticking up on the begining of my dock. The wholesaler of the product offered to give him his money back on the product. I even offered $4500.00 extra to replace with trex. SILENCE! I'm litigating to a solution as he has not responded to any requests. He does not stand behind his product, busines or responsibility. Please becarefull as I paid him appro 9k and now have to spend more to fix what should have been right from the beginning. Boards are raised because scwews did not go into stringers. Many problems and no solution from him.
