Good Day,Our apologies for the inconvenience. I have reviewed the medical documentation and although it does not state the required months, I have approved it for cancellation and a refund of all monies paid on 3/*/2015. Please do not hold the representative of the club responsible,
he/she was requesting the correct document and may have not realized that there was a time requirement.The refund should be available in your account within 7-days.Thank you

While we are pleased that the business has replied to our response, our concern is not only the failure of the company to deal adequately with our original cancellation and ongoing unauthorized charges, but the fact that the business has over five hundred lodged complaints dealing with similar unauthorized use of credit card for charges.Closure for this complaint will require that the business explain why, and how, the unauthorized charges were made, and why it won't happen in the future.A copy of the cancellation confirmation has been sent and received by the *** location, which is also investigating the claim.We are considering a civil action against the business if the business does not provide an explanation for the unauthorized charges.We also plan to request that the Attorney General's office investigate potential fraud of the business in relationship to the unauthorized credit card charges, and have begun the process of contacting other members who have experienced similar issues

Good Day,Rate Lock is not a "benefit" to the membership; it is a required fee associated with all memberships. Secondly, our agreements are not for months, the agreement is for a minimum of months or a "commitment" period. Once this period ends, memberships continue on a month to

Good Day,It is not a benefit option because it is part of every membership enrolled since May 2011. The fee maintains the rate for the life of the membership, not the following year. This fee is charged to all existing memberships that are active on January ***. The fee is not refundable and is part of the membership.Thank you

Good Day,The $was a usage fee for using a club outside of the consumers home club. The $fee was due to our inability to collect the monthly dues on time. Both fees are part of the terms and conditions. Please see attached signed agreement.Thank you

Good Day,We have no way to prove identity theft; we can only provide the information that we have. The membership was enrolled online and has a corresponding IP address. My recommendation is to contact the police of FBI regarding the identity theft.No refund is due.Thank you

 This digital subscription was not by me. This does have my corresponding email but it was not done with my permission. I would venture to say that this was done illegally by a email hacker or identity thief. I've never attended NYSC during this period at all. There was no confirmation by NYSC sent to me or my mobile device. This is how most companies complete identity checks. I am due a refund for the 5 years of illegal charges.

cancellations require 30 days notice, so [redacted]'s membership was active until January [redacted] and subject to the fee in accordance with the agreement.  I have attached a copy of the agreement for review.Thank you.
