Dear Ms. [redacted],
The purpose of the receipt copies being forwarded to you, were for you to review the receipts.  And now that you have confirmed the signature is not yours. 
All that will be needed is an Affidavit of Fraud.  There are other terms financial institutions [redacted] call an Affidavit of Fraud, such as an R10 Affidavit.  
On this R10 Affidavit there are several options/reasons to select why the claim is being filed.  If I recollect correctly, you would select Unauthorized Transaction.  This is all something you would have to clarify directly with the bank. 
Once you have completed this Affidavit, and your Fraud Claim is honored. The completed Affidavit [redacted] be forwarded to me so your record can be updated to Fraud Status. 
I have placed your file at dispute status during this process. 
And the sooner I obtain this Affidavit of Fraud, the sooner your file can be updated to Fraud status.  This is the information that needs to be on this affidavit, check number [redacted] in the amount of $35.81 dated October 9, 2014 payable to [redacted];'s and the affidavit [redacted] list the whole the account number of the affected/compromised account.

Thanks for taking time out of your day to talk to me on [redacted].  Per our conversation, your [redacted] banking numbers was mistakenly associated with an active debt record in TeleCheck’s system.  This technical error resulted in a decline of your check.  Please be assured that this problem has been corrected and your file has been modified accordingly.   However,  there were additional declines not related to the decline previously issued; on your [redacted] banking numbers.  The declines received on the [redacted] bank account was the result of predictive risk scoring that associated a relatively high level of risk with the transaction.  The merchant’s  decision to initially decline the transaction based upon information provided by TeleCheck was not based on evidence of unpaid debt, account balance, or other banking status, and no indication was ever given to a 3rd party that your check was unacceptable because of evidence of this nature.  I have since made additional updates to both of your accounts to help you avoid receiving these type of declines.  Lastly, I’ve also reached out to TeleCheck’s customer service/call center management team to advise them of the unacceptable service you received from agent [redacted] that disconnected the phone call during your conversation.  Please feel free to contact me at phone number [redacted] with any questions or concerns.  I’ll be glad to help.

               You had no problem rejecting my check & now you want me to prove who I am. Since you have a hold on my checking account, I would like to see a copy of the check you have in your posession that is alledgedly mine. If you can't produce it, I will assume it is a scam & take appropriate action. People just don't get their accounts blocked for no reason.  Your office is the one that put the hold on my account. You should check around there before anything else. As I said before, all of my checks have cleared & I have none outstanding. The problem seems to be at your end.

  I returned the call and was told that basically nothing could be done about my complaint.  Was informed that maybe something could have been done about my check being declined had it happened mon-fri 8-5.  No reason for the decline has yet to be offered and the company has given me no guarantee that this won't happen in the future.  I recommend that anyone in my situation simply cease doing business with all companies that use telecheck services.

Just wanted to add that I talked personally with a David [redacted], who was very helpful to both of us in this matter. He talked personally to our bank which told him they had not record of the check & that do not turn a check over to TeleCheck & he spoke personally to the manager of the [redacted] store here a [redacted]. He got in touch with who he needed to at TeleCheck services & assured us that he would see the matter all the way though. We have been assured that the matter is now closed, this record has been taken care of & removed from the TeleCheck system & will not be brought back up again. He was so very kind & more companies need to hire more exceptional people like him. He was also very kind knowing that I was going through a medical issue at the time.
