filings, court records, magazine subscriptions, and address changes.We have removed the requested information from the US Search database, as well as affiliated sites.We recommend Ms[redacted] review our privacy policy located on the US Search website at https://www.ussearch.com/about/privacyIdentity verification is an important step toward fraud prevention.? We don’t want anyone to remove records of other people.? Generally, we do require an individual’s identity to be verified prior to processing an opt-out.? ? To do this, we request that proof of identity be submitted along with the removal request.? Proof of identity can be a state issued ID card or driver's license.? When uploading, faxing, or mailing a copy of a driver's license, we require that the photo and the driver's license number be crossed out.? However, we do need to see the name, address, and date of birth to ensure that all information is removed from our database.? If an individual does not feel comfortable faxing us their driver’s license or other state issued identification, they can send a notarized statement of their identity.Additionally, it is important to remember that the procedure to opt out is temporary.? US Search is constantly updating its database with new public records.? It is possible that an individual may need to resubmit a request to opt out for any new records that may have been added to its database since their last opt out request.? offer includes the standard full one-time purchase price and a second discounted report price plus a one-week trial of the Intelius Premier Plus membershipIf the user elects to continue with their selected report type but does not wish to continue the Premier Plus membership they are instructed to cancel the Premier Plus membership within the first days and they will only be charged $for the Premier Plus trial membershipHowever, if the user does not cancel, the member is enrolled in the Premier Plus membership and their credit card will be charged a monthly fee of $(plus applicable taxes) going forward ? Intelius provides clear disclosures within the report purchase process of the Premier Plus membership offer.? Below, I have outlined the process and various places in which Intelius notified Mr[redacted] of the membership offer and details? On May 4, 2018, Mr[redacted] purchased a discounted Background Report in the amount of $39.95, which included a 7-day trial membership of the Premier Plus membershipMr[redacted] was notified of this offer and trial membership when he selected his report type and again on the payment information pageAdditionally, he was required to manually select a checkbox on the payment information page before he could complete her report purchase that specifically notified him of the offer and the membership terms, including that his payment method would be charged $(plus applicable taxes) if he did not cancel his day trial membership, a phone number to cancel his membership, that his membership benefits would continue through the last month for which he paid and that there are no refunds or partial month creditsMr[redacted] was presented with language in the same checkbox that by clicking the button to purchase, he agreed to the Terms of Service and Privacy Policy (with hyperlinks to each)A representative copy of the purchase page is attached for your review ? Per the Terms of Service, Intelius has a no refund policyHowever, as a matter of customer courtesy, I have provided him a refund of his June 4, membership fee in the amount of $on June 7, Mr[redacted] should note this last refund in his PayPal account within the next - days depending on PayPal’s internal refund policy ? Previously purchased reports on Intelius can be viewed by selecting “My Reports”, which is viewable in the upper right corner of the website after signing into an account.? After purchasing a report, it will be available for daysSince Mr[redacted] was not able to view his People Search
Report, we have canceled charges in the total amount of $on 1/28/2016.? This will reflect on his statement ($& $1.97) in one of the next two billing cycles depending on the closing.? Again, all charges on the account have been canceled.Additionally, if Mr[redacted] would like assistance with purchasing and/or viewing a report, we urge him to contact our Customer Care TOLL FREE at (888) 445-2727.? This account was created on May 29, under User ID# [redacted]On October 1, 2016, Mr[redacted] purchased a USSearch OmniSearch monthly membership in the amount of $per month on his Visa card ending in [redacted]On July 6, 2017, per his request, a customer care agent provided a refund of Mr[redacted]’s last membership fees for a total of $and removed him from the paid membership program ? Mr[redacted] also has a free account on the Intelius website.? This account was created on July 30, under User ID# [redacted]This account has never been enrolled in an Intelius Premier membership program and the only purchases tied to this account are one-off, individual report purchases utilizing a Visa card ending in [redacted]The last purchase made on this account was for a social media report made on November 28, ? Based on his login records, it appears that Mr[redacted] logged into his free Intelius account on July 5, and his last login on the USSearch.com site was on May 29, Since Mr[redacted]’s paid account is on the USSearch.com site, he would have been prompted to purchase either a one-off report or a membership subscription if he attempted to use his USSearch paid membership benefits on the Intelius website ? In addition to our contact information appearing on Mr[redacted]’s monthly Visa statement, the phone number to contact USSearch is listed on the upper right-hand corner of the USSearch.com homepageThe Intelius website has a contact us link in the persistent footer that links to a page where the user can email us as well as providing a toll-free number for phone assistance.? The Help section on the Intelius website also provides a toll-free number on the main page of the FAQ section ? The billing details are reiterated on each page during the purchase process.? By choosing to proceed with the purchase at the promotional price, the customer agrees to the terms and conditions of the promotional offer.In this case, the promotional offer was accepted and the Intelius Premier Plus 7-Day Pass was activated? The Intelius Premier Plus Membership was canceled on 12/27/2016.? As stated within the Terms and Conditions which Mr[redacted] agreed to during his report purchase, Intelius has a no refund policy.? However, as a matter of courtesy I have refunded three charges of $for the Intelius Premier Plus Membership.? The refund will appear on Mr[redacted] statement ($29.95x3) in one of the next two billing cycles, depending on the closing date.? Ms[redacted] should note this refund on her credit card within the next to days depending on her financial institution’s refund policies
