I appreciate this issue being brought to my attention. Phoenix Financial Services LLC is a third party collection agency. Upon review of your inquiry, I determined that the following concern was raised: ? Consumer states this debt is not owed. ? Consumer states that information was requested and has not been sent. ? Consumer states we have not previously tried contacting them via mail. I personally conducted an internal investigation, which consisted of the following: ? I reviewed this file as well as all notes that are associated with the file. Upon conclusion of my internal investigation, I determined the following: ? Our initial letter, which contains all required disclosures and notices, was sent on 5/8/2017 to consumer’s verified address. ? A second letter was sent on 8/14/2017. ? I have requested a verification of debt. It will be sent to the consumer’s verified address once it is received. Thank you for your feedback.  We do show the initial legally-required letter with notification was sent on 11/27/15. Additionally, the consumer called on 12/1/15 and requested a statement which was emailed on 12/1/15.  On 12/2/15, the consumer requested additional documentation...

and the original statement from the creditor was sent on 12/14/15 as requested.  We have not received any dispute information showing the account was already paid or not due for some other reason.  If there is a dispute, the documentation may be emailed to [email protected] for the client's review. I am re-mailing the statement for the consumer's review.  The account has been placed in a dispute hold and there will be no further communication on the account from this agency.Kimberly [redacted]Phoenix Financial Services LLC

Thank you for your inquiry.The account was placed on 11/5/15.  The initial letter was sent on 11/26/15 and initial phone call on 12/2/15.  We do not credit report for at least 30 days from the initial letter to allow time to set up payments or send a dispute, so credit reporting was not...

begun until January of 2016.After a certain time period of no payments or arrangements setup, creditors often pull accounts back to re-place or sell.  This account was pulled back by the creditor on 7/18/16.  The request for credit reporting deletion was sent on 7/18/16 due to no longer having the account in this office.  It does take the credit bureaus time to process the files upon receipt.  The call into our office was on 7/29/16, after the account was no longer in our office.  We are legally unable to accept payments or collect on an account after the creditor has pulled the account from our office, unfortunately.I have sent a letter confirming this closure and deletion for the consumer's file.Kimberly E[redacted]Phoenix Financial Services LLC

party collection agency.Upon review of your inquiry, I determined that the following concern was raised:The consumer states the account was closed by us previously and deleted from credit.The consumer states the account was re-opened and re-aged to be placed on his credit again.The consumer requests deletion of credit reporting on this account.I personally conducted an internal investigation, which consisted of the following:I reviewed this file as well as all notes that are associated with the file.I also spoke with the Client regarding information in their internal files.Upon conclusion of my internal investigation, I determined the following:The initial account placed with Phoenix in February was the account beginning with [redacted] for date of service 4/4/13. This account was sent back to the client as part of a standard recall and deleted from credit in September of 2016. We had no disputes on file when closed.Phoenix then received two new accounts beginning with [redacted] and [redacted] in October and November of 2016. Thank you for your inquiry. The account has been closed and removed from any credit reporting while the creditor obtains the requested information. This type of documentation typically takes 30 days or longer to receive as it must be requested from the facility at which services were rendered. The business broke the law by reporting inaccurate unverified information on my credit report. What address did the verify was my correct address? I never received such letter or correspondence from the business on the date the suggested the mailed the document. It is unfair and unethical for the business to conduct business in such manner. I demand that they immediately remove/suppress their trade line from my credit bureau reports. Again I have never received any communication from the business. What every address they used to mail me documents as the claimed is not my verified address.

Ms. [redacted] states we have refused to provide a copy of the bill.? Ms. [redacted] states the account was reported to credit with no communications. I personally conducted an internal investigation, which consisted of the following: ? I reviewed this file as well as all notes that are associated with the file. ? I also spoke with the Client regarding information in their internal files. Upon conclusion of my internal investigation, I determined the following: ? The account in our system is for a physician group, which bills separately from and is a third party to the hospital. The hospital unfortunately would have no information on the physician group invoice. ? Upon reviewing the calls, the information requested was provided other than the physician name, which is not in our system and has to be ordered from the client. We did not hear a request for invoice but I have requested one be sent to Ms. [redacted] for her review. ? We do send a letter and place calls prior to credit reporting. We did not receive a response at that time. Our COO did attempt to reach Ms. [redacted] upon receiving this complaint to discuss options but did not receive an answer. We are happy to work with her on this. ? At this time, I have placed the account in a cease communications status and added the telephone number to this agency’s internal Never Call List. I have sent to the COO to review the calls in question.  However, the information the patient states was given was incorrect.  The documents are being reviewed by the client currently.  The credit reporting is set for deletion upon receipt of the documents due to possible Medicaid....

 The reference number for this is [redacted]. Once the client verifies the Medicaid coverage, they will typically pull the account and not re-place with another agency.
The account is currently in a cease communication status awaiting determination by the client.

In January all three credit bureau restored the default that was originally reported on 5/26/16. They were originally  removed after the first Revdex.com complaint. The company has requested documentation. I won't send my whole credit report, but attached are the screen shots of the credit alerts. The company has requested that the information be sent directly to them as well. I have sent the file to that email address as well. I just want the defaults removed.
