Attached to this email is a complaint I have filed with the Revdex.com (Revdex.com Case#XXXXXXXX) on 01/06/(around PST 4am) regarding my Claim ID#XXXXXXXX with WageWorksYou should hear from Revdex.com about this complaint within two business days
After this complaint was filed with Revdex.com, I logged into WageWorks again this morning (01/06/216) around 10:15am, and realized that my $reimbursement has already been deducted from my HSA account, and not from my FSA
Nobody from WageWorks contacted me to provide a good solid explanation on why WageWorks could not reimburse me from my FSAI did exactly what your ORTHODONTIA PAYMENT GUIDELINES (FSA) (click here)) has stated but your claims department refused to honor it
Please reimburse my unclaimed amount of $from my FSA2016, and not from my HSAYou should check with your claims department to tell them to stop the $reimbursement from coming out of my HSA with immediate effect, as this case has not yet been satisfactorily resolved$was shown to be deducted from my HSA but it has not yet been deposited into my designated bank account as of now
I look forward to a resolution soon where you will honor what has been published in your orthodontia payment guidelines for FSAI should not be held responsible for your negligence and inability to convey accurate information on your website

Once again WageWorks has not addressed the problem as I originally stated "I checked with Wage Works SEVERAL times via phone calls and they state they forwarded the funds onto Clipper Card, however Clipper Card has stated that they never received the spreadsheet from Wage Works that would allow them to auto-load the $onto my clipper card."
Either WageWorks forwarded the funds to Clipper Card or they did not forward the funds to clipper Card, that is the issue
The first thing I did was to check with my employer and made sure my employer forwarded the funds to WageWorks, which they stated they did
I'm not sure why WageWorks is addressing this with my employer at this time since my employer has proved to me via payroll deductions that the funds were withheld from my paycheck on September 11, and forwarded to WageWorks

Once again WageWorks has not addressed the problem as I originally stated
Either WageWorks forwarded the funds to Clipper Card or they did not forward the funds to Clipper Card that is the only issue!
I'm not sure why WageWorks continues to state that they are addressing this with my employer since my employer has already proven to me via payroll deductions that the funds were withheld from my paycheck on Sept11, and forwarded to WageWorks
I have NOT received any further contact from WageWorks since the original communication

I have tried on multiple times to resolve this matter with the Wage Works call center and their Team. I have asked repeatedly for the matter to be escalated and for a supervisor to contact me and no one has reached out. The denial reason for my claim is - Denial Reason: The amount requested exceeds the cost of the lost pass. This is not true as the pass * never received from Wage Works was over $50 and I was only reimbursed for $20 so far. I have tried to resolve this matter with Wage Works but they have been uncooperative and refuse to let me speak with a supervisor who can make a decision. I wish to escalate this matter and continue the investigation.

This is the same form explanation that was previously sent. I have asked more than 6 times to speak with a supervisor at Wage Works, yet no one will return my call. The dollar amount of my claim for reimbursement does not exceed the dollar amount of the monthly metro card I would normally receive. I am entitled to be reimbursed for the additional $20 that I submitted. Wage Works is stealing my money and I will continue to escalate this matter until it is resolved. I request that Wage Works look into this specific situation and not send out blanket responses that do not pertain to my case.

On January 11, 2016 I received a call from [redacted] with WageWorks who informed me that because I was no longer with my employer the refund that is owed to me would be forfeited due to it being pre taxed dollars. She advised that I should have given the parking company (LAZ Parking) a month's notice to cancel my contract, however, as I advised her, this was not possible as I did not know 30 days in advance that I was going to leave my employer. When my notice to voluntary terminate was given to my employer I immediately emailed LAZ Parking to advise them of my departure date, which was only 2 weeks in advance. I am not satisfy with their response. There should be some way to work with the employer to calculate what it is that needs to be calculated so that people are owed their refund. To say because they are no longer with that employer and their refund will be forfeited is ridiculous!

I have attempted to resolve this matter with your customer support team on April 27th, May 10th, May 13th, and May 17th, 2016.  At all of those times your customer support team completely screwed things up and never resolved such matter as stated in original Revdex.com complaint.  Every time I contacted your customer support team they stated they needed to send it to another dept and asked for my phone number if anyone needed to call.  NOT ONCE did anyone contact me nor would they let me speak to the dept that was to handle the situation.  Hence why I decided to contact Revdex.com first then my next step will be to the Dept of Labor to file a complaint against Wage Works there.  As I know my rights and my rights are not being done under the Cobra law.
