Weight Loss Grants Organization Reviews (%countItem)

After loosing weight I have been told that my file is under review for the last 4 months.
I was promised 80% of my 2400 back if I lost the weight through their dalewood clinic. Which I had done.

Desired Outcome

I want my promised money back

Weight Loss Grants Organization Response • Nov 22, 2019

Ms. submitted her final documentation in July 2019. Her file has not been reviewed yet. The reduced number of personal working on the grants has affected the grant review turn around.
All files are being reviewed in the order that they were submitted. We hope to have Ms. file reviewed with the briefest of delays.

WLGO applicants who were referred to pay the hefty fee to Dalewood/Trillium Weight Loss for a remote program. The clients were given false promise as per agreement that once they reached the goal, they would get 80% of the fees paid returned to them. However, once you submitted the request to claim the grant, you would most likely receive invalid rejection reasons, rule changes, ridiculous delay excuses or even no response.

I signed up with WLGO in early 2017 and was referred to the Dalewood location at 2 Bloor Street West where Darka P. was the Clinical Director. I discovered flaws with the weight loss program and several *** of the registered dietitian and the Program Director, Sara J., also a RD. I filed complaints to the College of Dietitians of Ontario and they were given written reminders to adhere to the standards and guidelines of dietitians in Canada.

Sara J., without consideration of the patient/client's health, abruptly instructed the registered dietitian to stop providing support to me. I was left on my own with 4 months before the deadline. Fortunately through determination, I achieved the 20-pound goal in August 2018 and submitted all required documents.

David S., identifying himself as the adjudicator from WLGO called me in October 2018 and told me that WLGO is underwritten by insurance and that each time a grant request is sent, they will have to prepare the documents to claim the insurance. He also said that there is a threshold to this grant, indicating that funding is limited which is contrary to WLGO's own claim on the website that up to $3M is allocated each year for reimbursement. David S. said if I followed his instruction to write an email to Dalewood about withdrawing my complaints to the College of Dietitians of Ontario and Revdex.com, then he would give me the grant right away.

I naively followed his instruction thinking that I would get my grant. Instead, he wrote back a couple of days later saying that Dalewood disputed the legitimacy of my grant request. And since Dalewood is their premium, largest national sponsor, WLGO had to follow the agreement not to give me the grant when I was qualified and entitled to. In May 2019, Sara J. the Program Director at Dalewood confirmed that Dalewood did not dispute anything about my grant; David S. also confirmed to Sara J. that he did not mean that. However, no grant was issued to me and David S., after being challenged ***, told me to stop contacting him.

Later from July to Octobe 2019, George S., the managing director hired by the Darren M. to take care of issues from Dalewood and WLGO, reached out to me pretending that he would give me the grant if I agreed to a settlement and release waiver with terms that restricted on freedom of speech but without a real settlement amount. All these time he was stalling and delaying me from taking further steps. As of today (almost 16 months now), I still have not received my grant despite all the hardships I had to go through.

Now WLGO has begun winding down and may eventually stopped all communications before giving out all legitimate grants. Darren M. and his family/associate have moved on to operate the new Dental Grants of Canada. Sara J., the highest person one can escalate anything to at Dalewood, who pretended Dalewood is separate from WLGO, was in the promotional video of *** from U of T with Darren M. She has started a new business adventure called *** with Karen F., dietitian team lead position at Dalewood/Trillium Weight Loss.

Both WLGO and Dalewood have been misleading the public and giving false representation of the Weight Loss Grants Program and the true connections between these two organizations. For those who were denied of the grants that they deserved including me, we were *** of our hard-earned money by WLGO/Dalewood or actually Darren M. and his family/associates including Katherine B. (real last name) and Lola S. (OAAG).

I paid money to DaleWood which was a Grant to lose weight through Weight Loss Grants. I lost the weight and they refused to pay the Grant back to me.

Desired Outcome

I lost the weight and was to get back I believe $1900 dollars which for some reason denied me.***. I submitted all the documents they asked for including verification from my Doctor that I lost the weight.

Weight Loss Grants Organization Response • Nov 07, 2019

Ms. grant application was rejected because her documentation did not pass the scrutiny of our fraud prevention procedures.

Ms. submitted a weight declaration form from when she went for her final weigh-out, and when we went to verify the weight that was reported on that document, we found that it did not match the patient file and charting notes that were provided by her doctor's office.

We consider this case closed.

I had not gone to Dalewood to weigh in the same day I went to my doctor. I went to my doctor and he verified the weight. Just because it does not match my weigh in at Dalewood from a previous month does not mean I did not lose the weight as I did. My weigh out was to be a certain weight which I achieved. All documents were sent in on time, weight was lost by the time they allowed and the doctor verified it. They are trying to get out of paying.

Weight Loss Grants Organization Response • Nov 26, 2019

Ms. final weight declaration that she submitted, was compared to the records that were recorded by her doctor's office on the date of her visit.

The declared weight and the verified weight did not match, and hence her file did not pass the verification audit.

We consider this case closed.

(The consumer indicated he/she DID NOT accept the response from the business.)
I went to Dalewood some time in July and had until September to lose the remaining weight. It states nowhere that you have to weigh in at both places the same day to get the same number. I had well over a month to lose the weight I needed to from my last visit to Dalewood. It was not mandatory to weight in all the time at Dalewood as they even cut my times down due to the cost they pay the person to weigh you in. My weight obviously won't be the same six weeks later as I had to lose the remaining weight to get my money back. I went to the doctor for the weigh out as instructed. I did what was asked of me and like everyone else that did their program they are trying to get out of paying. These people should be held accountable!

I completed my weight loss before my deadline date and asked for the piece of paper to fill out with my doctor in order to receive my grant. My deadline was January 2nd 2019. I have, in writing, that I had 30 days to get this form filled out after my deadline date and confirmed with the staff member via message board so I had it in writing. A deemed me has rejected due to missing information on my doctor's form. Which I have a copy of and it is completely filled out. When confronted about this issue, they backpedaled and told me that I didn't weigh out on the right day. Which is false due to me having the date from the employee stated on the message board. I have yet to receive any calls or correspondence regarding my re-evaluation to receive my grant money back. I followed every rule that was implemented on the website and clarified everything with my representative. I have now lost $1,900. They made me pay via credit card in two instalments and refused to take debit or cash transaction. I dealt with Dale Wood health regarding my payments and initial assessment.

Desired Outcome

I would like my money back that I was promised if I reach my weight goal, which I did. The financial burden this is caused and the amount of Interest incurred on my credit card due to false promises by the weight loss Grant organization I would also like covered.

Weight Loss Grants Organization Response • Nov 01, 2019

's deadline date was January 2, 2019. She had to show that she went from 193.0 lbs to at least 162 lbs on that deadline date.

The 30-days that *** is speaking of is the amount of time that she was given to upload all her documents. we understand that even though applicants must have their final weigh-in on their deadline date, it may still take them a few days to collect all their receipts and proof of participation. That is why their portal stays open for an addition 30 days. It is not so the candidate can avail themselves with extra time to meet their goal.

Also, ***'s grant was rejected because the doctor that performed her official weigh-out did not provide us with a copy of the Patient File / Charting Notes for the date of her visit, for verification of the information.

Customer Response • Nov 01, 2019

(The consumer indicated he/she DID NOT accept the response from the business.)
I called in and spoke to someone who told me to request my grant earlier in the year (even though I had not lost all of my weight yet) to get the weigh out form and prepare for my doctors appointment. which then gave me 30 days to get my paperwork submitted from the date I requested it. This was reversed because the girl on the phone didn't realize this would happen. I then clarified with Abi on the message board and she stated:

Nov 05, 2018 05:22 PM EST
"No that is not possible. Please click request grant by your end date. You will be given 30 days to get your weigh out completed and to upload your documents online. If you cannot get your doctor to weigh you out in time ...please go to a walk in clinic to get weighed out. "

From this statement it was said that I have 30 days after January 2nd (my end date) to get my forms completed. On January 1st I requested my grant to get my form COMPLETED and uploaded.

Abi also stated Oct 18, 2018 08:49 PM EDT
"It will be emailed to you when you click request grant. Please go to a walk in clinic if your doctor is not available to do your weigh in within 30 days from date you click request grant. "

I clicked the request grant button and received and received an email minutes later on January 1st,2019, giving me the form to get my doctor to complete and 30 days (January 31st) to COMPLETE and UPLOAD my weigh out form. As my doctor does go home for the holidays I got an appointment with him on January 31st and submitted ALL required paperwork that day.

"I have uploaded ALL documents and weigh out form. I requested a few minutes ago ! "
Jan 31, XXXX XX:XX AM EST

It is against the patient privacy act to email my PERSONAL file from my doctors office that contains private and very personal information. I refuse and do not feel comfortable to give the Weight Loss Grant my personal file since birth with all my medical documents/information. My doctor had filled out the 1 required form and signed it the day I got weighed out (January 31st). Asking for more proof later on for my re-review, he wrote numerous letters and faxed them as he advised that I should not give them my whole file.

For my re-review I did provide letters signed and dated stating that I did reach my goal and he verifies this.

I have jumped through all the hoops and followed the instructions provided to a T to ensure a smooth process.

Weight Loss Grants Organization Response • Nov 13, 2019

As part of the audit procedure when evaluating all grant submissions, the Patient Visit Record (PVR) and Charting Notes for the visit are required.

Ms. was informed of this and when she requested a re-review, a time-limit of June 30, 2019 was given to submit the PVR and charting notes. Those documents were not submitted.

We do understand the concern that some clients have over some sensitive non-weight related information being sent to us. All documents that are received are kept on a secure server and are only viewed by the adjudicating committee.

If Ms. is concerned about non-weight related information appearing on her PVR and Charting Notes, we would accept a redacted version of the PVR and Charting Notes.

I have completed my weight loss submission and been approved for a grant last year. In spite of a positive review which would allow me to get my grant's cheque through registered mail, I have yet to receive said grant.

I have reached out to them by email multiple times since then and have not been given any indication that they will be giving me the grant I was due to get. Even if the weight loss support was amazing, their actual customer service regarding the grant is ***.

Tl;dr: I still want my grant

I signed up for a weight loss grant through Weight loss organization back in August of 2017, whereby reaching my goal would grant me a refund of $2,000 from the original $2,400 disbursement out of pocket. I have met all of the requirements of the program and obtain final approval letter that the cheque would be issued in June 2018. It has been over a year since the approval and I have attempted to reach management at weight loss grant multiple times (over 15 times) and the cheque still has not been issued! I feel that the advertising on the radio from this service is ***. It is not right to promise a refund for this magnitude and not deliver on the offer. Further, although beside the point, my experience with the weight loss service provider was awful! Basically, they provide no service for the initial payment and no support. I have attempted to get a refund 2 months into the experience and they would only offer a fraction of my initial payment. Continued on by myself expecting to obtain the grant in the end.

Desired Outcome

Looking to obtain the grant promised when initially signed up for the program.

Weight Loss Grants Organization Response • Aug 12, 2019

Hello ***,

I am sorry for the delay. As you may have read on our website, we have had to institute new fraud-prevention procedures due to the number of fraudulent claims that we were receiving.

We hope to be caught up to date shortly.

The Conditional Funding Certificate shows a beginning date of Feb-09-18. This date was then changed to Mar-16-18. On Mar-15-18 I was provided with login information to communicate with them through their Message Board "MB", "Use the internal message to communicate with your Dietitian, Health Coach, Clinic Director or Fitness Specialist. (Click on the 'Message Board' tab.)"

The first week of Feb-2019, I called *** at *** Brampton to make an appointment to weight out. She told me to go to my GP and that I had until Mar-16-19 (which is a Saturday). A weight out form was to have been ready for download Mar-13 or 14-2019. On Mar-17-19 via MB I asked for the weight out form and they replied ".... you may submit a doctor's/medical note with your ending weight documented...".

On Friday, Mar-22-19 I saw my GP and submitted all required documentation to WLGP via MB. Since I had not heard back from them I called WLGP on May-31-19 and was told that my grant was denied as I did not submit the documents by the "deadline" and that the rejection was sent to me via gmail. I requested that the decision be appealed and a note was put on my file. On May-31-19 I requested the appeal via MB. I do not have access to my gmail at work and the internet at home is not the best. I relied on their MB for all communication.

On Jun-05-19 via MB I followed up as I did not see my name on the list of individuals requesting review of their file, so I appealed again.

On Jun-10-19 via MB I requested a list of documents that was requested to be uploaded, to confirm that *** list of Mar-18-19 was completed. The reply via MB "Go into the request grant section".

On Jun-16-19 via MB I followed up. The reply via MB "I don't have any updates regarding your file.

On Jun-24-19 via MB I followed up. No reply via MB.

On Jul-12-19 I was able to accessed my gmail and there was an e-mail from *** of WLGP ".... Pursuant to our terms, your weight-out must take place on the date of your deadline in order to demonstrate your weight as of that date. If your deadline occurs on a weekend or statutory holiday, then you may weight out a day prior or a day after..... you weight out on Mar-22-19 thereby granting yourself an additional 7 days to lose more weight. Accordingly your grant request is hereby rejected".

At no time through their website/verbal communication/written material with WLGP and ***, was I ever advised that the weight out had to be EXACTLY 365 days from the weight in. I was ready to weight out in February and told to wait.

Desired Outcome
I paid $2,640.00 and seek a refund of $2,112.00 being 80%.

Desired Outcome

I paid $2,640.00 and seek a refund of $2,112.00 being 80%.

Weight Loss Grants Organization Response • Aug 12, 2019

Hello ***,

The rules of the Weight Loss Grants Organization has always been to lose at least the agreed to weight, at the deadline date that was chosen.

That has never changed.

Your grant was denied because you did not show that you achieved your goal on your deadline date.

Customer Response • Aug 12, 2019

(The consumer indicated he/she DID NOT accept the response from the business.)
I do not accept their response. They have not produced anything to back up their position, then and now. No documents were ever produced to explain what they mean by "terms", "deadline" etc.....

They did not explain the reason for not providing me with the Weight-Out form and when I inquired about same, they did not inform me that my doctor had to weight me EXACTLY 365 days from the weight in.

March 16, 2019 was a Saturday, March 17, 2019 was a Sunday and my doctor's office is closed on weekend. I am off on Fridays, so I made the appointment Friday, March 22, 2019. Not counting the weekend, my weight out was 5 work days from the so called "deadline" date and I was actually 1 pound less than the goal I set for myself.

Weight Loss Grants Organization Response • Aug 21, 2019

The Weight Loss Grants Organization allows applicants to conduct their official weigh-out with a doctor at a walk-in clinic. This accommodation is made for applicants so that even if they could not get an appointment on their deadline date with their doctor, they would be able to still follow the rules and procedures of the grant program.

The goal weight and the deadline date are both chosen by the grant applicant at the time that they register for the grant.

Customer Response • Aug 26, 2019

(The consumer indicated he/she DID NOT accept the response from the business.)
How can I follow the Rules and Regulations, when same was never provided to me. I was not made aware of the Rules and Regulations either in writing or verbally.

I successfully completed a 6 month accredited program agreed to by WLGO. March to Sept 2018.
My forms were uploaded and I was congratulated and told they would verify my claim. Two days later I received a Welcome email and told to go for an initial weigh in. I emailed my contact person and was told it was a Fake appointment ignore it.
In January 2019 I looked at my profile and saw my end date had changed from Sept 2018 to Sept 2019. I contacted them and they said my weight loss should have been 18 months not 6 and they will send an email three days before my new end date to upload my forms.
With all their problems their website asked people who did not receive their money to request a review by June 15 2019 I ignored it as my end date was September 2019 now.
My accredited program received a message in May 2019 that I am a new client.
I was to start June 2019 and end December 2020. I was disqualified for not sending in a weigh in form for this fake new client date. I contacted WLGO and had several message board conversations to let them know they made a mistake and needed to reinstate my second end date back to September 2019. WLGO says they can no longer help me.

THIS IS NOT A GRANT IT IS A ***!!!!
***
I don't know where to begin.
#1- *** Perspective clients are rushed through a signup process where the money is given upfront by the client and then a percentage is returned if the goal is achieved.
#2-Dieticians, kinesiologists, therapists, nutritionists and personal trainers are provided to the client. Yes, they are...online! And the client is only told this after it's too late too back out.
How does a personal trainer advise you safely and constructively without personally seeing you????
#3-Science-based testing. The initial DNA and Metabolic test that are given to provide a "tailored" weight loss program for each client are junk science.
https://www.self.com/story/i-tried-dna-testing-for-fitness-and-weight-management... /> #4-The rules are constantly changing.
#5-The program is unhealthy. Only months in I developed a severe lung condition that prevented me from doing most of the activities required. I was given a 6-month extension but no option to stop the program due to health conditions. Fearing I would lose money I persevered ending up with a knee injury that may require replacement and both of my rotator cuffs are torn beyond repair. I have lost my job because I can barely walk and have no shoulders preventing me from doing pretty much anything. ***
#6-Communication breakdown. It is impossible to contact or connect with anyone regarding retribution. Though I didn't meet my goal (an impossibility considering my health conditions), I lost 50 of the 90 pounds I set out to lose, it has become a neverending situation of passing the blame with only web site forms to fill out for "review" and no responses or results returned.

The company I was set up with was called Dalewood Health and Wellness. When word got out that the program was basically *** the name was changed to Trillium Weight Loss Centres and a disclaimer was put up on their website that their affiliation with WLGO has been suspended?
***

I WANT MY MONEY BACK!!

After completing the program and submitting all paperwork I was approved. A check was never sent. I followed up and was told there was a delay but that I would receive a check by June 2019. This did not happen. Below is a copy to confirm all terms of the contract were met. I would like delivery of payment promised.

Dear ***,

This email notice is to confirm that your Weight Loss Grant has been approved for $702.36 and has been sent for final adjudication, following which your cheque will be sent within 15 days from which your deadline was to occur. (Your can view your deadline in your Funding Portal)

Your cheque will sent to the following address:

Desired Outcome

I would like delivery of the payment o was promised.

Weight Loss Grants Organization Response • Aug 12, 2019

Hello ***,

Due to the new fraud mitigation procedures that have been put in place, all files must be reviewed again in their entirely.

We expect to have the everything up to date shortly.

Weight loss grant has not payed me the money owed to me. They come up with stalling tactics and excuses for not paying out the amount they sent to me in an email. The email was received in October 2018 and said it would be payed out within 10 business days. Every conversation since then is excuses and scripted plans of what they would do. Now phone calls just go to answering machines. I have contacted the *** clinic and it is shut down and the person I talked to there said "good luck" getting your money. *** I will be consulting my physician to see if they are involved in this.

Desired Outcome

I am only seeking the money back they promised me in an email, no more no less.

Weight Loss Grants Organization Response • Aug 15, 2019

We have not been able to get the doctor who conducted the necessary weigh-out to confirm the accuracy of the documents that Ms. has submitted.

Our fraud mitigation procedures require that all documents that are submitted by an applicant are verified to be genuine.

Without the doctor attesting to the genuineness of the documents, the file cannot be approved.

Customer Response • Aug 23, 2019

Here is the Approval letter, stating that I would be getting back the grant! The document signed is from a Doctor at a walk in clinic, that I was advised I can see, as my family doctor was on Vacation during the expected weighin time frame! The documents provided were signed by a doctor!
They are using every reason to not want to pay the grant! *** This company is a *** and the concern is they are using "Grant" as a way to lure new clients in, and not paying them in the end!
They ask for a thank you letter up front with our story before they will even send payment so the reviews will look positive, when in fact they don't actually pay!

Weight Loss Grants Organization Response • Aug 29, 2019

's file has not been closed. We must receive collaborating documentation from the doctor who conducted the weigh-out.

We will send out the registered mail again to the doctor, and wait an additional 30 days.

Unfortunately we cannot control why doctors sometimes do not want to authenticate documents that were submitted to us by applicants.

I signed up with Dalewood Health Clinics in Newmarket with ***, that was recommended by WLGO as a clinic to use to lose weight on their grant program. You get a grant, so 80% of your money paid back once you lose the weigh. I lost the weight, submitted ALL the proper forms and on time on WGLO's website. 3 months later they sent me an email that I had been rejected "due to not submitting the weigh-in form". I did submit it, via email ( or possibly directly to the portal- I don't have proof of that documentation any more) a year prior to that, but *** at WGLO claim I didn't. This is utter BS and I have the form here in my hand and can show it to anyone. They *** and owe me $2100!! Pay up WGLO!!

Desired Outcome

I want to be paid my 80% of the $2640 I paid to Dalewood for this program. I want WGLO to accept my weigh-in form

I enrolled in the weight loss grant program. I needed to lose from 185 to 153 in one year. I lost the weight, received a letter from my doctor stating that I weighted 152 lbs on my due date. I do not have a home computer and was having a hard time uploading the requested documents. I tried calling the weight loss grant organization number that I used to sign up on and for over a week I could not get through to a live agent. My call kept being dropped after the recording asked my to press a number, did not matter what number I pressed the same result, call was dropped. With my frustration I called *** from Dalewood which was the organization I reported too for regular weigh-ins. I explained my frustration and was assured it would be fine as they were very busy with lots of woman signing on and as *** said I had nothing to worry about as I lost the weight. I finally got through a few weeks later and the lady on the phone could not or would not tell me what documents came through on my uploads... she said it looks as though I added the required forms. I didn't hear anything for months so I called again and was told I was still in the early stages of the process.... frustrating. May 9th I received an email stating that my grant was being refused because I did not upload weigh in declaration form. This was the first form I tried to upload. I uploaded more forms of proof than required and feel that this is unfair. There was no way to confirm that all forms were uploaded properly. The fact is that I lost the weight and have all the proof! It would be unfair to deny me for a computer glitch or as they say a form was not uploaded when I did upload it. It is not what they advertise.... lose the weight and get your money back! I lost the weight and they are being unfair.

Desired Outcome

I paid $2400.00 for the grant and was told I would get 80% back. I lost the weight and Dalewood can confirm as well as my doctor. I uploaded my documents and they say they didn't receive them... I should be able to send them by registered mail to make sure they received them. I want my $1,920.00 back from them.

They closed the oshawa location since then l am unable to reach anyone even at there head office l have piad 2650 for the program ,l am l my 4th mouth in ,if they are not going to give me any support like they said they would l would like what money that is due to me.

Desired Outcome

To see if you can get them to call me and settle this.

I have the same complaint as others. I was promised reimbursement of my fees to an accredited supplier as long as I followed the rules. I applied for the grant but was turned down because my thank you letter wasn't signed. I could have easily fixed that. They also said I weighed out early on May 13/19. I was supposed to weigh out on May 20/19 but was unable to get an appointment for that day. I really don't think one week was going to make a big enough difference in my weight that I wouldn't be eligible. I would have had to gain more than 10 lbs between the 13th and the 20th to not be eligible. I paid $1297.00 plus HST to the service provider and was promised 80% of that back from the weight loss grant organization. I feel that their rejection of my payout was unjustified. They had an option on the website to put in a claim for a review but I just got rejected today Jun 21 and the deadline for the claim to have your file re reviewed was Jun 13.

Desired Outcome

80% of the amount I paid out of $1297.00 which would be $1037.60

This organization claims to assist low income individuals to afford dental implants. They offer a grant. All this is just a way to convince you to invest in the procedure. In the end you end up paying more and more. If you don't pay you end up with an abutment at the most. They have you believe you will get a great deal, when you can actually get a much better deal on ***. Now I have post without the crown because after scamming out of $1200 plus a so-called grant of $650, they want an additional $1,160. It seems every time I turned around I owed more. I was never informed from the beginning that I would have to pay more and more. They never explained the plan clearly and only after having the abutment inserted did they start asking me for sums of money I cannot afford. There was a lack of transparency from the start.
Product_Or_Service: Dental Implant Grant
Account_Number:

Desired Outcome

Other (requires explanation) I want my investment of $1,200 refunded so I can find a doctor that will put in a crown. ***. They never told me this was going to continue costing me. This was supposed to be a grant for LOW INCOME people. I cannot afford to pay any more.

I have submitted my request grant documents after the due date and I missed one important document which is the weight out declaration form from my Dr. and I've been begging them since June 6, the due date, to provide me with any way to send it to them or a link or email with no luck. I even emailed it to them at an email they provided it online they said I'm blocked and I have to wait for them to finish reviewing my file.
Thank you.

Desired Outcome

Award for loosing weight and get my goal on specific time.

. I have been in contact withthe weight loss grant on several occasions , particularly since recieving this message on the message board
*** says:
"Your grant was approved Aug 10, 2018. We are doing our best to get them out. Keep in mind it does take 15 day business days to process your file before you cheque gets mailed out. We have requested an update from the department that handles the cheques. We can advise you as soon as it has been mailed.

Aug 31, 2018 11:27 AM EDT

I just keep getting the run around, I am told every time hat my file has been "Escalated" and all I can do is wait. I have been waiting since August 2018 for my 1900.00 refund. I have tried and tried to speak to or be heard by a superior with no success. Plese help

Desired Outcome

I want my 1900.00 refund as promised in August 2018

I was approved for the weightloss grant may7,2017.I set my weightloss goal at 50lbs in 1 year.I did my weigh out july28,2018.I recieved an email on nov7,2018 that I would receive a check for 1710.00 within 15 days.I have been calling and calling getting excuse after excuse as to why my money has not been returned. The last excuse was that everyone was being audited for fraud I called my family dr on may31,2019 and they resent my documents for the 2nd time to the weightloss grants program.I still have not received my 1710.00 back.It has almost been a year of excuses.

Desired Outcome

Return of my 1710.00

- First installment paid on Aug 1/18 of $1475, second installment of $1475 paid on Sept 1/18. Sales rep (clinic direct ***) Paid by *** Credit
- The first 9 months were fine and I lost weight with help from the clinic director *** and the regular weigh-ins. When I went for a weigh-in (which was part of the service I signed up for and set up as a drop-in) on June 10/19 and I was told by the receptionist of the floor that Dalewood Clinic had not been operational in the building for 3 weeks. When I finally spoke to someone at head office the following day, they told me that all the clinics had been closed and they were no longer conducting weigh-ins as part of their services (even to existing clients). This had been done with no notification to clients, although she said it was posted on the Patient Portal. When I logged on to the Patient Portal it was not the closure of the clinics that was posted but rather their name change from Dalewood to Trillium Weight Loss and they stated that they would be letting clients know of this change efficiently (I was never notified of this change either). The last line also stated that clients would not have their services impacted "in any way whatsoever." I took a screenshot of this screen for proof. Their website also still boasts of 32 locations - all of which are apparently closed according to *** who I spoke to on the phone. I have emailed to cancel my services and receive a partial refund for the remainder of my services as they are no longer offering the services they promised when I signed up on June 11/19. *** emailed me back that evening. I replied right away and since then I have no heard one word from the company, I have emailed them multiple times and have called them as well. They are no longer returning my emails or calls.

Desired Outcome

I am seeking at least a partial refund in the amount of $2360, which is 80% of the amount I paid and the amount I was promised for losing the weight according to the grant.
