I submitted my claim to Weight Loss Grant on February 28, 2019. All documents were sent in as required. Their reply to my numerous messages, when I was actually able to get through, were always very vague and my emails received a very standardized, cut & paste, reply. In encounters I have managed with 'customer service', I have been greeted with *** and impatient personnel. They did not have the time of day for me and eventually denied my claim saying that I did not upload proof of payment (receipts) and that my letter, attesting to full proof of payment from my provider, would not be accepted.
I expect to receive $1200 from them and have done absolutely everything as requested. This was supposed to be positive journey; however, they've managed to turn it into a very stressful process. As so many other people have said, I reiterate that all I want is what we both agreed to when we started with this program, for them to honour their contract and pay me the $1200!

I want them to houour their contract and pay me the refund owing of $1200.

They say right in their reply to me that the letter uploaded from my provider stated that I paid!

Then the next line they say it's not proof of payment and can't be accepted.

One sentence contradicts the other. The letter is absolutely proof of payment and I want WLGO to pay me the refund owing of $1200.

My complaint is two fold. One issue is with Weight Loss Grants Organization and the other with *** which has now changed names to ***. The summer of 2017 I heard an add on television and radio for Weight Loss Grants. After hearing the ads for numerous months I called the number provided and spoke with one of their representatives, ***. Per our conversation she directed me to a list of sponsoring accredited providers. I also received a letter confirming my 'conditional' approval from *** on July 24, 2017. A deadline of completed diagnosis had to be sent in by August 1, 2017. I selected ***. I set up a meeting of which I was given a great sales pitch by ***. I was informed that if I completed my goal the grant would be awarded from Weight Loss Organization. I was a new stay at home mom and the cost seemed high but my health was important and I needed some accountability. I accepted the terms, provided payment up front of the total amount on July 25, 2017, as required, and completed the weigh in, and so the process and misinformation began. I was given 18 months to complete my weight loss goal/target. I was provided with 'tools' to assist in the process and a metabolic intake test. By tools I mean a package printed off of the internet, a supposed assessment of my intake test which provided no insight, some vitamins and some rubber exercise bands. I was also provided access to a message board/portal from which I could contact 'my team' of specialists. Any information provided from the 'practitioners' was nothing new, nothing one couldn't find on the internet. It wasn't customized as promised. It was pretty much a self directed weight loss program that cost $2400. I went to weigh in appointments weekly/biweekly at first then when it became apparent that this was a *** I opted to stop wasting so much of my time. I continued to go to weigh ins as they were a way for me to continue to strive to reach my goal weight. On March 7, 2018 I received a letter from Weight Loss Grants Org confirming my grant up to $2000, file #***. Not the initial amount announced on the advertising of $1800. It was a higher amount so I didn't have an issue with it. After only a year I had successfully achieved my goal and then some. On October 31, 2018 I went for my final, or so I thought, weight in at ***. I was quite pleased with myself for fast tracking my weight loss and achieving my goal. I was then told by *** that I had to go for an additional weigh out at a third party facility ie. family doctor, clinic etc. Off I went to spend 3 hours at a clinic waiting to see a doctor to complete the process. At no point in time did she mention that you had to wait until the preset completion date of your program to weigh out. After I got the sign off from the Dr. I proceeded to send in all required grant forms via the portal as that is their only method of contact. I received a message on the board stating I was rejected because I had to wait until March 1 2019 to weigh out. After complaining they reinstated the grant. I was also informed that I had to do the final weigh out, by *** and ***, no sooner then 3 days prior to March 1, no later then 30 days after that. On March 14, 2019 I went to a clinic again to weigh out and sent in all of the paperwork. After waiting 2 months and contacting them frequently I received an email on May 13 stating I was rejected again because I didn't submit the paperwork in time. I went into the portal to check details and they had changed and updated information and terms again stating that I had 7 days from March 1, 2019 to submit my paperwork. This information had never been there before. I have emails, message board correspondence and screen captures to prove that I was continually provided misinformation in attempts to not pay out the grant money. I followed the rules and met the requirements twice!

Desired Outcome

As I have successfully met the requirements that were communicated to me by Weight Loss Organization and their staff and their sponsoring accredited provider twice, I want a full refund of the $2400 if possible. I have wasted numerous hours of my time in weigh ins, and clinics following their rules. I have also lost precious hours with my young son whom I had to get babysitting for during these times. If the full amount paid to *** is not possible the $2000 promised by the Weight Loss Grant confirmation letter would suffice. I would also like to see their advertisements removed from all Canadian TV and Radio so that others don't fall for this ***.

The information that you had specified that in that 30 day window that your portal was open (starting three days prior to your deadline date) you would need to complete your official weigh-out on your deadline date, and upload all the necessary documents and apply for your grant.

Your deadline date was chosen by you 18 months earlier. That date did not change, and the rule of completing your official weigh-out on that date did not change.

I do not accept this response!!! It is a blanket response to be vague
like the rest of their program!
I have no issues with the deadline that was assigned to me. I completed my goal early and have continued to exceed it. I attempted to weigh out in November. I was informed I'd have to wait until March 1st the program completion. They reinstated it and I had no issues with this and complying with the rules communicated to me by Weight loss org staff on multiple occasions in varying formats.
My issue is that I was given misinformation repeatedly in attempts to confuse me and not pay out. I was in contact with WLO over 50 times from Nov 1 to March 1 for reassertion, clarifications and to be on record.
It was communicated as by evidence of the attached that I had to weigh out no sooner then 3 days prior to march 1st no later then 30 days after.
I would not have missed the deadline considering I had been waiting for 4 months!!! It is a lot of money and I am educating and would not have made this error had it not been communicated to me.
Only once my grant was rejected did the 'new' information about weighing out on the deadline date and providing the paperwork within 7 days, appear on the portal.
The rules and regulations did change multiple times in the timeframe from November 1-March 1st!!
The WLO claims the rules have not changed however I have multiple screen shots, emails that prove otherwise. There was also an segment on CP24 last Monday with two other grant applicants facing the same issue as I am!!
They had also told me that a grant once assigned could not be changed. I have signed paperwork from WLO proving otherwise. I was originally granted $1800 and then had called in after hearing a new commercial for a higher grant amount. The were happy to reassign me a new grant for $2000. After hearing yet another commercial a couple of months later fr yet another increased amount of $2500 I messaged again and was informed by message board that they are not allowed to reassign grants, yet they had already done so for me months prior? That would count as rule and term changing.
Their communications are vague and thus disputable.
I would like them to revisit the case.
I weighed out twice, followed their various rules that were communicated to me over the 1.5 year period.

I was told by Ms *** that if I put a $50 deposit down that I would receive a copy of my dental x-ray. I never reived a copy and when I spoke to someone from *** office they informed me that I would have to pay and additional $35 dollars.

I would like the $35.00 dollars I paid to get the the x-rays that was promise to be included with the $50.00 deposit.

I applied for the weight loss grant Mar 6 2018. I had a year to reach my goal of 30 pound loss. I achieved my goal but could not get an appointment with my doctor until March 28 2019 to get my weight verified. I was told that a letter from my Dr would be sufficient to prove my loss and that it was ok despite my year date was to be Mar 6-2019.. I contacted them to verify that a letter from my doctor would be sufficient as I never received the form that they normally require. They wouldn't send me one but I was told that the letter would be sufficient. I borrowed the $2600 to pay the Dalewood Clinic to start this weight loss program. This is one of their approved clinics to qualify for the weight loss grant. I feel like I have been *** into giving up my money to do this. I only did it because I was assured they would reimburse me 80% which translates to $2,080.00. I worked very hard to achieve this goal, not just by following the diet but I also worked out at the gym 5 days a week. I was feeling so encouraged and grateful that this program was available to me and now I have been rejected because according to them I didn't make the date even though I was assured after several phone calls that it was ok as long as I had my dr verify my weight and supply me with a letter and,that I didn't submit the proper document, the one my Dr's letter was supposed to replace. I asked more than once before I submitted my documents. I am really stressed out about this and feel like I haven't been treated fairly. ***.

I want them to honor their promise and give me the grant they promised if I did what I was told which I did.

Your deadline date of March 6 2019 was know, and chosen by you 12 months earlier. That is enough time to make arrangements to be weighed out on that date. We also allow the official weigh out to be conducted by a doctor at a walk-in clinic as a convenience to ensure that applicants meet their deadline date weigh-out requirement.

In addition, your official weigh-in Weight Declaration Form was not uploaded.

In the first place mo-one told me that I had to have someone else besides Dalewood Clinic weigh me out. I found out a week before my deadline from the employee at Dalewood who was weighing me. I made the appointment right away and this was the earliest date I could get. I wasn't going to go to a walk in. I was told my Dr was to do it. I was also told I had 30 days to get my paperwork in. I never received a weigh out form. All I had was the original form when I applied for the grant and I was told by Dalewood that was not the form I needed, that the grant would have had that already. When I asked Dalewood if they had a copy they said no. I then telephoned the weight loss grant and informed them I had never received that particular form and was told that the letter from my Dr would suffice. If this is incorrect information your staff should be advised not to tell prospective applicants incorrect information. I feel like I am being disregarded in this matter by the weight loss grant program. I did the work and I paid the money and they need to honor their commitment. ***. I acted in good faith and believe that they should also act in good faith.

You have until June 15, 2019 to submit the form.

The company claims it will refund you if you join a program and lose 16% of your current weight within a certain time. You must work with one of their approved providers. I paid $2400 to that provider. When you do reach that goal, they find every excuse in the book not to pay.

You are not able to reach anyone in their office but a receptionist who has a script on what to say. "They have a lot of applicants who have reached their goals"

Their contract say they will refund you within 14 days of completing the program but you are later told up to 45 days and in some known cases even after 7 months the funds are still not received.

There is nothing that stipulated we needed to upload anything at the time of application. *** Only when we requested the refund, did they advise us to upload to some website. They had their other company *** updating them on all our progress.

I applied for the weight loss grant in January 2018 and had until January 24th 2019 to lose a designated amount of weight. I attended a weigh in session in Ottawa and followed their criteria to the letter. I was given the deadline of January 24th 2019 to have lost the weight and upload all the required documentation. All of this was completed before the deadline date. I tried throughout the process and after the submission of documentation to contact them to ask questions, get clarification and a follow up. I called and was told to submit a message on the message board in my login portal which I did every time. Their communication was virtually minimal and no help whatsoever. Now over 3 months later I received the following email:
Dear ***,

We regret to inform you that your Weight Loss Grant was not approved for the following reason(s):

"Applicant weighed out Jan 18th, 2019. Deadline Jan 24th, 2019. Applicant did not upload signed thank you letter. Applicant uploaded proof of payment contract that is only signed by applicant and is incomplete. Validation documentation has different date than weigh out declaration form. "

Every visit to the login page has had different information.
Therefore I wish to appeal for the following reasons:

1. I was given a deadline of January 24th to be weighed by. A deadline is the date to have procedures completed by so I was weighed in good time. THEY DID NOT SAY I HAD TO WEIGH ON THAT DATE, but now the site conveniently says that.

2. My thank you letter was most certainly uploaded. I now cannot access the area now where my documents were uploaded on the portal. Why?

3. I have a legitimate *** contract and still attend ***. I can happily get them to call the Program and confirm that.

4. At no point during this process was it stated that all validation documentation has to have the same date. I cannot think how that can happen?

I lost the weight required by the programme and the lack of communication along the way and this dismissal is disheartening encourages me to feel like a failure which is ***.
I wish to appeal the decision.

The rules for claiming a grant have always been the same. The deadline date is chosen by the applicant and it must be the exact date that the official weigh out is conducted. We allow the flexibility to have the weigh-out conducted by a doctor at a walk-in clinic to make it easier for some applicants.

The rules for verifying matching dates, is applied to all applicants. It is part of our fraud mitigation procedures.

If flexibility in dates is allowed why was I declined because I didn't weigh out on an exact date? Where is this information about verification flexibility? There are 3 other people going for the grant at my gym and they have different information from me.
Total lack of consistency, *** and false advertising. ***. No I do not accept the "***" response.

This Weight Loss Grant Organization is *** and very misleading. You spend all this money and do what they ask in order to be approved only for them 7 months later to send an email saying you are not approved because documentation failed to meet standards. Everything was sent on time(doctors weigh out, receipt of payment etc.) and weight loss was proven. They then even sent a congratulatory email in the interim saying I was approved and how much I was getting back, a timeline as to when I should expect the cheque, address confirmation and if I would post a review! They never get back to you and they do not make good on their promise. You will not receive one dime without a fight.

I completed my weight loss and all documentation was submitted in August of 2018. That was NINE MONTHS AGO. I weighed in at my doctors office. My deadline was November 2018. I recently got a message saying that my claim was not approved because I didn't meet the deadline. The message quoted both the dates that I have mentioned below. How did I not meet the deadline when I actually reached my goal BEFORE my deadline? I have called in and emailed messages many times. At one point I spoke directly with the General Manager as she said I "was lucky that she answered the phone and that she would escalate my claim asap as all my paperwork was in order".

I am a young university student who could really use my money back.

Program started Apr 18/17. I paid $2100.00 by March 10/17, as they requested. I met all their requirements and my goal weight was reached on July 18/18. Two emails were received on Jul 18 & Sept 10/18 from Weight Loss Grants Org. indicating my "grant was approved for $1,680.00 and I would receive my cheque within 15 days of my end date", which was July 18/18.
Despite NUMEROUS phone calls, I have not received my reimbursement to date. Every phone call results in what sounds like a scripted answer. They say I am on a prioritized list and will be receiving word as to my reimbursement. So far, nothing has progressed in my favour. I worked very hard to lose the weight and I am not prepared to lose such a large amount of money either. I am a Senior and $1680.00 is a lot of money to be left outstanding. It will soon be a YEAR and I am still waiting. I had to meet a deadline to succeed in their Program, so why can't they reciprocate? This is causing me a lot of stress and anxiety and I would very much like to get it resolved and done with.
I would appreciate anything Revdex.com can do in this regard. Thank you in advance for your efforts on my behalf.

I want the return of 80% of my investment ($1680.00) from the Weight Loss Grants Organization as soon as possible. Waiting almost a YEAR is totally unacceptable business practice and this needs to be resolved.

Upon further review, *** has not reached her deadline date yet. The deadline date that *** chose was July 18, 2019 at which time she must have lost at least 29.0 lbs, from her starting weight of 177.0 lbs.

*** must have an official weigh out on Thursday July 18, 2019 by either her doctor or a walk in clinic in order to qualify for her grant. Is this another "stall tactic" by the Weight Loss Grants Organization? I started the Program on April 18, 2017. I had to pay $2100.00 by March 10, 2017. On February 18, 2018, I received an email confirming an extension "from 9 months to 15 months, which makes your new end date July 18, 2018." Upon reaching my goal weight, my doctor completed the Weight Declaration form on July 16, 2018. All documents were submitted as requested by August 6, 2018. On July 18, 2018 and Sept. 10, 2018, I received two emails from the Weight Loss Grants Organization indicating "my Grant was approved for $1,680.00 and I would receive my cheque within 15 days of my end date" (which was July 18, 2018. I have an "End of Program" document which confirms all of the above. As you can see, all documentation refers to 2018 as my completion date! I very much doubt a weight loss organization would give an individual over 2 years to lose 29 pounds - that is ridiculous. Perhaps the Organization should take another look at my File ID #XXXXX to see that I have indeed completed the Program as of July 18, 2018 and have all documents to support such a claim, plus my physician would verify that this was true because I had informed her of my participation in the Program. Further, in numerous phone calls to the Organization, no mention was ever made about my not completing the Program because the end date was in 2019. Whoever I spoke to on several occasions would have been aware if my end date was July, 2019, but that wasn't the case. Why am I being subjected to this additional stress and anxiety after meeting all the requirements of the Organization? My End Date was July 18, 2018, NOT 2019. I have NO documents to suggest it was 2019. I would sincerely hope that the Organization would do the right thing and reimburse the sum of $1,680.00 to me as their emails in July and September of 2018 have indicated they would.

I do not have faith that this organization will honour their obligation to reimburse me. My file was already reviewed and my grant approved. An acceptable resolution to this matter would be receiving my refund as their emails indicated I would.

I started the with the Weight Loss Grant Organization in 2017, I was required to lose the requested weight by 11/28/2018. I did all of my appointments through ***. When it came time for my final weigh in, I was advised that I would receive a weigh out form via email. When I didn't receive it I contacted the Help Desk asking how I should get it. I was advised that I would get it 2 days prior to my end date. Due to this I cancelled my appointment with *** and made a new one for 11/28/2019. When I didn't receive my weigh out form, I followed up with the Help Desk who advised me that if I didn't receive it they wouldn't be able to send it to me. They advised because it wasn't received I wouldn't be able to go to *** for my final weigh out I would have to go to my doctor. I let them know I don't have a family doctor and was advised that I could go to any walk in clinic. I once again cancelled my appointment with ***. I went in to ***, the doctor weighed me and provided the information on a prescription pad as was requested by WLGO. I submitted this along with the rest of my paperwork. This is where it gets a bit murky, I've been advised by WLGO that for fraudulent reason they can't advise me of the process to complete the application. What I believe happed was a letter/document was sent to the clinic asking for my patient file. The letter was sent back signed by the doctor along with the prescription stating that I was in fact weighed by a doctor on 11/28/2019 and that the info on the prescription pad was accurate. WLGO is saying that this is insufficient and that they require my patient file for the day. I have spoken with the Ontario Ministry of *** & Long-Term Care who advised me that what the walk in clinic has and has sent to WLGO is sufficient to constitute a patient file. That the doctor might not have made notes, and isn't required to have made notes. That what goes in to a patient file for a single visit varies wildly. ***, who I have been in contact with just comes back that this is for fraud prevention, but fraud prevention shouldn't stop people who have followed all the rules and met the requirements from getting their grant.

Due to an inordinate amount of fraud and false claims, our adjudication process must entail more than just 'the receiving of documents'.

Your file is in our system and will be re-evaluated to ensure that the information submitted is genuine and accurate. I can see from your file that some key information that has been requested from your doctor has not been sent to us.

I know that waiting must seem frustrating, however it is necessary to ensure that only qualified applicants receive their grant.

I don't feel that you have read my complaint. This seems like you have just copy and pasted a response that you use for everyone on this site. I have been advised by your organization that what is missing from the walk in clinic is the patient file. However, per my complaint which advises that I reached out to the Ontario Ministry of Health & Long-Term Care, the patient file can be the prescription and doesn't require any additional information. I appreciate that you are trying to prevent fraud, but I would like to know how you plan on processing my claim if everything has been sent to you? Have you followed up with the clinic to request something else as they have already sent you everything they have in their patient file. They are not required by law to have anything else. So, through no fault of my own you are advising me that you can process my application because you haven't received the patient file, but the clinic has sent you the patient file (the prescription and the additional letter). They have confirmed with myself that they don't have anything else. I have confirmed with the government that they are not required to have anything else. As WLGO is made up of a group of professionals they should be aware of what a patient file may or may not contain and make requests accordingly. If you need a letter or something additional, I suggest you request that information from the clinic. You also need to investigate your fraud prevention practices. They shouldn't prevent someone who has done everything asked from having their application processed. Please read my complaint and address it accordingly.

The documents that we require from the practitioner that conducted the official weigh out are clearly stated and understood by all medical practitioners. When a practitioner decides not to abide by our request, we do not question them regarding their motives. Without all the necessary documentation to verify the genuineness of the weigh-out document, the application is rejected.

I signed up for the Weightloss Grant - upon approval, completion, I submitted all required documents. A Cheque was issued but the calculation DID NOT include an expense of ober $1,000 that was the final payment for my personal trainer session. The document was resent, and I have been in communication with the organization since - either no answer, or asking for more patience. My main contact was *** wheile last time -on Jan 30th, 2019 - I talked to ***... that was almost 3 months ago. I did my part of the agreement, and although received partial payment, did not receive all of it. I just need them to send me the final cheque, I need to close this case.

The case cannot be closed - All documentation was indeed provided at the same time - Weight Loss Grant simply failed to include one receipt that was for over $1,000 which resulted in partial payment only. As soon as the letter arrived, BEFORE even cashing the partial cheque, I contacted them and resubmitted everything. I spoke to *** multiple times, over the phone, via e-mail, then in January with *** and I was told that they have all documentation and should get the 2nd cheque shortly. If they would look at my e-mail trail they would see the multiple communication, documents being resent - everything they asked me to provide was indeed provided immediately. They are the one who are failing to provide me with the final payment. It was an unusually high amount (most of ***'s payments to her service provider were around $100-$200 per month).

I've completed my weight loss in Oct 2018 and submitted all my documents required before the deadline, I have many confirmation emails from the company saying my documents were in order. they've told me I had to wait until the deadline. I've been waiting over 6 moths and now they are telling me I have to re-submit all the paperwork again. ***
The lack of communication and respect by this organization has brought a lot of distress in my life. ***, however they are still advertising in all medias (TV, radio, etc). I have a big folder with all the email communication, documents, receipts, grant award, etc. that I've send them.

's deadline date is the last day of April 2019. On April 30, 2019 *** must be weighed-out at her doctor's office or at a walk in clinic. *** must then submit that weight declaration form as instructed, along with the rest of the required documents through her client portal.

I did upload everything yesterday on my deadline. including a SECOND weigh out. This organization changed the rules that the applicant need to do the weight-out EXACTLY on the date of the deadline. They changed their term and condition recently in their website. However many people, like myself, should not be included in this rules, as it was not what we signed up for and we were not advised of the change. I have all the proves for that. So someone is penalized for achieving their goal BEFORE the deadline, what type of organization is this? ALTHOUGH, I NOW DID a SECOND WEIGH-OUT and have everything in order, they still do not answer my questions directly.

I signed up with WeightLossGrants.org in August 2017. I was approved for a grant of up to 80% of the cost of a weight loss program with an approved provider. I paid the provider directly and completed my weight loss plan within the time limit. I submitted all of the requested documents and was advised that my grant was approved and payment would be issued in August of 2018. To date, I have not received payment. I have sent multiple messages on their message board and called numerous times. Each time, I get the same reply - they are dealing with a high volume of claims and will put me on a priority list.

Desired Outcome

I would like to receive the grant that was promised to me. I would not have signed up for the weightloss program had I not been promised a grant. The grant total is $1,920 which is 80% of the cost of the program.

Hi,

I was sent a message on the portal on August 28, 2018 with a follow up email on August 29, 2018 saying that my grant had been approved and that a cheque would be sent to me shortly. This message has since been deleted from the portal by your organization.

I contacted your organization via the message board on October 18th, 2018 saying that I had not yet received the cheque and was told that you were backed up with a high volume of claims in the previous 2 months and that I would be placed on a priority list.

Since my claim was submitted on April 25, 2018, and was approved on August 28, 2018 well before the "2 month backlog" in October, your response does not make sense.

As I continue to hear your ads on the radio, and your website mentions nothing of a claims backlog, ***

From your response to this file, you deal with claims in the order of their deadline date, mine was April 17, 2018. Are you saying that no one who has completed a claim in the last year has been paid their grant?

If I had not lost the weight within the time limit, you would have rightfully denied me the grant for not meeting my end of the contract. Please uphold your end.

All files are now going through a stringent fraud mitigation procedure, that was put in place due to the high occurrences of fraudulent applications being submitted.

Even files that appeared to be correct, are being re-evaluated.

I hope that the backlog will be cleared up shortly.

I was granted the weight loss grant if I was to get downs to 140 pounds lbs by October 12, 2018. I have all of the documents that was provided to be by the weight loss grant program. Due to an illness I was granted a month extension which I did loss the proper amount of weight by November 12, 2018 as well I up loaded all of the information that was required of me. I received a couple of e mails to say that my documents were in order for the grant. I waited and heard nothing until I looked on the website and my status was disqualified so I called the place that I dealt with for the weight loss and they sent in a e mail for me to the manager of the weight loss grant (***) who called to tell me that I had all of my documentation to say that I up loaded everything properly and before the date it should have been uploaded. *** at that time stated that there had been a significant amount of fraud with people changing the doctors information so I needed to get another form signed which I did and faxed it back. I received an email of the 28th of January to say that I would receive my grant 30 - 45 days after this e mail and I have not heard from them nor have I received my grant that was for $1920.00, which is a significant amount of money not to get back as promised. I call the pay out line and there is always no room to leave a message so it has been very difficult to get in touch with them and if I call the customer service line they say oh it will be coming which I have stopped believing that. I have tried several times either by e mail via the message board on my site for the weight loss grant, e mailing the manager (***)or trying to call both lines. the last phone call the gentleman on the phone state that he will e mail a guy by the name of *** who is the person who gives out the grant money but I have not heard from him or anyone for that matter. I have done all that I was suppose to do that was asked of me by the weight loss grant program and have not received any money from them as well I did call *** news to see why they are still advertising the program because of the amount of people that they still owe. All I want is to get the money that was promised to me by them. please help me recover the money I am deserving of. thank you

This has been the response that I received in January 2018 and I received another form for verification from the doctor who signed off on my weight loss. Prior to this I spoke to *** when I received the disqualification notice on the website. She looked into this and called me to say all my documents were in order.

I have all of my emails from the weight loss grant and the last one stated that I was approved and I would get the grant 45 days after that date which was for $1920.00. The 45 days has long passed and every time I tried to contact someone I did not get an email or response of any kind.

So because I have been approved I should have already had the money by now which I do not.

I have all the emails from you and ***. As well I reached out to the manager of *** my weight loss facility.

I am looking for the Cheque to be sent out as soon as possible

I understand the frustration that you are feeling. We found that the procedure that we had been using to approve grants was allowing for fraudulent applications to be processed successfully.

I wish we would have caught it earlier. We have however updated our fraud mitigation procedures, and all applications that did not have a grant cheque mailed out to them already, must be reviewed again under our new procedures.

The delay is unfortunate. We hope to have all applications addressed with as brief a delay as possible.

I received a message from a representative at the Weight Loss Grant that they had received confirmation from my Dr. and that I was now in the review stage. This was dated March 22, 2019. I have now been in review status going 5 weeks. But they told the person that I did the program through that they hadn't received all the information.

I submitted documents to receive grant for losing 40 pounds. The organization says I did not click on request grant. I did within the 30 days of my extension and received an email stating received January 26th. I received the email my date was December 29th approved extension. I also faxed the information as per there letter dated March 9, 2018 stating forms could be faxed. I was told by there call centre I could get a ***'s note as I did not receive the weigh out form from there office 3 days prior to deadline.

My compliant is that l am getting the run around about getting my money back plus getting my weigh out form.

So, on Dec 22nd, 2018 I started asking for the weigh out form which l got told that *** would provide the weigh out form to me. (*** is the place where l did my weight loss program).

Then, *** tells me that the Weigh out is to be send from the Weight Loss Grant Org. So, this went back and forth for awhile.

Then on February 4th, 2019 l called the help desk 1-866-670-6578( my daughter was listen to the phone call too since l has it on speaker).

The Help Desk told me that they would accept a prescription pad from my doctor with my name, weight and date. It also had the doctor's name , address and signature. So, l upload everything that the help desk told me too. The prove of acceptance, prove of payment, thank you letter and my weigh out on a prescription from my doctor.

So, now it is 2.5 months later and l am still getting the run around about both my weigh out and my refund.

I have messaged the Weight Loss Grant Org. and l am getting told "Not sure how long it will take". I messaged them again on March 5th and asked what was going on and again. I got "We can't give you a time frame". Then, I got a message back from The Weight Loss Grant Org. saying that they never received the confirmation from the doctor for my weigh out. So, here we are again going back and forth about my weigh out prescription pad, they are now saying that a doctor's note would be fine instead of the prescription pad which is the one that the doctor first did for me. So, l got the doctor's note from the doctor and upload it on the weight in form because that is what they told me to do.

I always sent the doctor's note it in on April 14th by faxed to ***. Then, I was told that l am not allowed to fax it. I upload it again on April 16th, so they couldn't say that they didn't receive it.

So, the whole purpose of this is that it shouldn't be so hard to get back the refund, when it was the main reason why l went with the Weight Loss grant Org. It gave my motivation to loss my weight. They should never offer a refund to people that want to loss weight if they don't intend to give it back. I shouldn't have to fight with them every step of the way. I am seeking that the Weight Loss Grant Organization pays me the $2000 that l was to get back after l lost my weight within the time frame, that l chose.

I have sent you everything that you've asked for regarding the doctor had prescription pad with her name and address her signature stating the date my weight my name my phone number plus I went back and got a doctors note which you insisted on. So I am not really sure what you were expecting now because I've gotten everything you've asked for. So what else is it that you want from my doctor that you haven't already got . This time please send me exactly what you want with an example or a document so that I know you can't use this excuse again

The fraud mitigation steps require the doctor who conducted your official weigh-out to provide us with clinic documentation to support that you submitted on the form. It is a check-and-balance to ensure that all the information is genuine.

We cannot ask you to obtain the information from the doctor's office because of the risk that the information that we receive can be compromised.

The request for information from your doctor is clearly stated on the verification form that your doctor received. Only your doctor knows why they chose not to supply the essential information.

In January 2017 I entered into an agreement with the weight loss Grant organization. I paid them $1,740. Week after the weight I agreed on was lost I would receive 80% that amount back in the form of a check. Completed that goal and submitted all documents that were required. I was told I would receive a check within 14 days. This did not happen I contacted the weight loss Grant organization and was given instructions to resubmit the information they required okay I did that and yet no check was if you'd I contacted the weight loss Grant organization on numerous occasion by phone and email. Delay tactics are ongoing by the company.

Ms. is incorrect. The grant organization never ask for, nor accepts money from any of the applicants.

Ms. file is in an audit review. A key piece of verification information has not been submitted by the weigh-out doctor. Until that document is produced, the audit review cannot be completed.

Totally false , I was told by my sponsor I had until February 11 20018 to download forms and submit, before that time I received and email from WLO on January 23 2019 stating I had 30 days from that date to submit my paperwork or my file would be closed. when I went on their site it said file deactivated and I was unable to download the paperwork, I sent messages with the above asking them to reactivate the file . This was done as I was able to download all paperwork on February 14 2019.. I then received an email saying all paperwork was received and it has been given priority and it is being reviewed, I would get a response within 15-45 days . why would it say that if is was disqualified. why would they reopen the file so I was able to upload the paperwork , which I could not before. No one seem to be addressing the email I received on January 23 3029 saying I had 30 days to upload my documents. That email supersedes everything and 30 days would be February 23 2019. all paperwork was submitted before then..
Several things happened, which they accepted and now they are saying no. shame that is breach of contract, If this goes to court the last correspondence would be valid . email from WLO January 23 2019 to me.

Our decision stands.

I joined the Weight Loss Grant Program on Sept 22,2017 through the *** Centre. I paid $2400 cash on that day. I followed the program, lost the required weight in the time allotted and submitted the required paperwork on time October 15, 2018. When I followed up with the WLGO on their online message board I never received a response for months. I was shocked when I went online to discover I was disqualified. Turns out I was never set up properly in their system and this is why I apparently never received a response to my messages. I was told there was a problem with their system. I then found my case was under review. On Jan 21, 2019 I found that I was again disqualified for not meeting the requirements of the weight loss program. I contacted *** Regional Director (GTA),*** and forwarded the documents to her. She contacted the WLGO on my behalf to investigate as the documentation I provided was correct. Most recently the WLGO advised they sent the Weight Loss Grant Declaration form to my family doctor to sign in January. When I checked with me Dr's office I was told they never received anything from the WLGO. They finally sent the Declaration of weight loss form to them by registered mail in March. My doctor emailed the form back and since then I waited weeks before they confirmed they have received it. Now I'm being told my doctor didn't complete everything she should have. As of today I am still "under review" and have not been provided with a timeline for my reimbursement of $2,000. I even sent them a picture I took of me standing on my scale at home with my final weight of 138lbs. I also mentioned that I have a tattoo on my left foot that says Angels Among Us, to prove it is me. I also worked out with a personal trainer and lost 16 inches. I took 188 classes for a total amount of $7,520, I did not submit this amount to the WLGO. *** I just want my money and want to be done with this. ***. My coworkers and family can vouch for me. I worked so hard for this!!!!!

I have submitted the required paperwork and have completed all of the required requirements of this program. I expect payment of $2,000 which I was promised.

I submitted all of the required information. My doctor signed and dated my Weight Declaration form twice. It also included her office stamp. I'm highly insulted I have been red flagged. I have many witnesses to my defence in this matter and would be happy tp bring them forward if required. I want my money now!!!!
