I signed up with Weight Loss Grants on Feb 19, 2018.

My weight on Feb 19 was 206 lbs. I was given a target weight of 180 and a deadline of Nov 19, 2018 to lose the required weight.

Upon inquiring at the time of sign up, if I can get the grant earlier then the deadline upon losing the required weight. I was told that I can apply for the grant as soon as I hit my target weight of 180 lbs.

I hit my weight target in Sep 2018 and submitted all the required documents to receive my grant. I called them every 2 weeks to find out when they will be issuing the grant and they kept telling me that they still haven't started on my file. 5 months later they contacted my doctor to get my current weight, at which point I went into my doctor's office for weigh in and I submitted the updated weight form to weight loss grants organization.

I did my part and lost the required weight before my deadline and I believe they should issue me the grant in the amount of $2112 that I was promised at the time of sign up.

I joined the weight loss grants program in 2017 and lost the weight as per the agreement. I received the grant approval email on Sept 7, 2018 saying my cheque would be mailed within 15 business days. I have called, emailed and put messages on the message board several times and have been told it is being audited. Now they do not answer the phone and say check back in a few weeks. I have several emails and the approval one but I am still waiting for my $1920 they said I was going to receive.

I am looking to receive the refund as stated in the email.

I applied for the Weightloss Grant in November 2017. I then started with Dalewood as recommended by the program. I reached my goal and sent all the required documentation but I was rejected because I was not weighed out by my doctor within 24hrs. This had never been shared. I requested to speak to a director and was told someone would call me. They never have. I have tried to contact them numerous times. I paid $2400.00 for the program and would like to receive my grant.

$1800 grant offered upon the loss of 20 pounds by the end of November 2018. File was allegedly reviewed and reviewed and audited only to arrive at a denial for several wrong reasons. Letter to be supplied with details.

I applied for a grant whereby I was to lose 17%of by body weight (in my case 54 lbs) and I had a year to do it. I initially contacted the weight loss grants prior to my deadline to find out what to expect as my deadline was approaching. I was advised that they would email me a form to take to my dr. I never received it. I contacted them again about 3 days to my deadline(March 30th, 2019)and was advised that I had 30 days to weigh out and submit all paperwork. I weighed out on April 23, 2019 and submitted all paperwork. Was informed by email on May 14, 2019 that my claim was denied because I "weighed out on April 23, 2019. Deadline March 30th. Applicant did not upload weigh in declaration form" I contacted them and explained why I hadn't uploaded the information previously because of the misinformation and was advised that a manager would be in touch. I got an email today from a manager advising that my file had been reviewed and that my "grant request is hereby rejected". In rejecting my application, I am out over $2000. I held up my end of the bargain, I feel they should honor our agreement as I lost the weight. I did not wait "extra" days to weigh out. I weighed out as soon as I could get an appointment with my family physician.

I want them to admit and apologize for accusing me of cheating the system. Admit I was misinformed and approve my grant and issue me the payment owed.

I began with the weight loss grant organization in May of 2018.0n October 24, 2018 I reached my goal. The due date was Nov 4. I weighted in and had the form completed and stamped at my doctor's office, as requested and with the forms they provided.
I also submitted my contract with my weight loss program, proof of my weight loss journey and receipts of the cost .I also provided the thank you letter requested. All these submissions were received and confirmed they were received by the organization on November2nd of 2018.
Since that time I have received nothing but excuses. Finally they said that my weight in date was early? They also said they didn't have invoices and a contract, yet I have an email confirming they did have them.
I responded telling them they already have then.
Now they say the information I sent was "only a weight chart" which documented my weekly visits. I also had sent a contract.
Today they sent yet another email, saying I have not documented my services. Although the form clearly outlines my weekly visit. Also requested my patient file and chart notes from my doctor. They never asked for that before and why would they need my personal chart. I feel that would be breach of my privacy. They provided the form. I had that completed and sent in.
So almost a year later I have not received my grant although I have given them everything they asked for. Now they want more.

I would like to report that I have been *** of The Weight Loss Grants Program (WLGO) https://www.weightlossgrants.org/ in conjunction with Dalewood Clinic. https://www.dalewoodclinic.com/ (who now changed their name to Trillium clinic). I received a notice of rejection on May 31, 2019 (below). They sited a rule that did not exist when I first signed up. I responded with proof of the attached original Recipient Funding Agreement and other correspondence. I have yet to receive a response.

"Pursuant to our terms, your weigh-out must take place on the date of your deadline in order to demonstrate your weight as of that date. If your deadline occurs on a weekend or statutory holiday, then you may weight out a day prior or a day after. The reason for this policy is because we defer each applicant to select their own deadline. If we do not enforce this rule, then everyone will automatically select '18 months'. However, in your circumstance, you weighed out early on Aug 10th, 2018. Accordingly, your grant request is hereby rejected. ***, Weight Loss Grants Organization"

In response to the notice of rejection received on Friday May 31st, on behalf of The Weight Loss Grant Organization (WLGO). According the Terms and Conditions that were outlined in April 2017, I was to lose my goal weight "...within the period of time allocated for achieving (my) goal" as stated in the Client Recipient Funding Agreement. This was outlined as an 18-month period, which was scheduled to end in September of 2018 as per Consultation Confirmation. I submitted all the necessary documentation ahead of time in August 10th, 2018 and was rejected under the premise that I was supposed to weigh in on September 29th. As per the aforementioned documents, and the documents following, this firm deadline was in no way specified, monitored, or enforced.

Not only have these dates not been established in the Terms and Conditions, but they were also never referenced in any communications regarding my weigh in. Please see below for a description of these supporting documents:

1) In a letter sent on June 17th, 2017, the WLGO stated that upon the completion of my weight loss goal, I am to arrange for a weight-out, asp per Grant Award Confirmation. This in no way references the strict September 29th, 2018 deadline, and in fact specifies that I am to weigh in upon the time that I reach my goal weight. This is exactly what I had done, as I had reached my goal weight August 10, 2018.

2) Upon submitting my documentation August 10, 2018, I received a warning specifying that I would have 30 days to weight out as per Grant Payment Request This again does in no way reference the September 29th deadline, but instead gives a 30-day allowance.

3) On August 10th 2018, I received an email following the submission of my documentation that the WLGO "will review and verify (my) documents within the next few days" and "...if any documents are still needed or clarification are required, (they) will contact (me)" ***. I was never contacted following this email that I would be required to weigh in, and therefore surmised there were no issues with my submission.

4) On September 29th, 2018 the date that my weigh in was allegedly due, I received a reminder that "the deadline for submitting the documentation verifying your successful weight loss is due..." and was then thanked for already having submitted my documentation ***.

I am aware from reading news article that you have experienced falsified documents from other clients and have therefore become more hesitant in rewarding grant money. However, I feel that I should not be penalized for their mistakes.

. Ironically, I was referred to Dalewood Clinic to receive 80% grant as a sales tactic. WLGO *** promoted Dalewoods program as being sanctioned by Health Canada. The ***, Dental Grant and *** all run under the same ***. I have honoured the contract's terms and conditions *** I would like to receive my grant in the amount of $1680. ***.

In Jan of 2018 I signed up with Dalewood clinic in Brampton as per OWLG suggestion to lose weight to qualify for 85% of money back in case of successfull loss. I have reached my goal and had my final weigh OUT in my physician's office in Dec/18. Early Jan I submitted my grant application on OWLG web. I have since called and emailed them numerous times to follow up. There was an technical issue which they did resolve. After that they kept telling me they are overwhelmed with applications but everything in on track ans there is nothing wrong with my application. I kept following up and kept following up and *** told me there is a paper missing from my Dr's office. I contacted my Drs office to look into it and they did but a day before my Dr re-send information to OWLG I got an email disqualfying my application. There was never a timeline, and kept re-assuring me that there was nothing wrong with my (4 page) application. Then they turn around and deny me... I was supposed to get 85% of $2,600 I paid to Dalewood for this weight loss. I have worked hard to reach my goal and I only signed up for this knowing I will get 85% back. ***. I can't afford to lose $2,000. Thank you very much in advance for your help. Sincerely

the company is not refunding the grant money to me when I complied with all criteria in less time that I had to lose my weight. they emailed me that they would contact me if any clarification needed from all documents they received otherwise I would get my grant money in 15 to 45 days. past the end of 45 days they emailed a letter stating I was rejected because they did not have my thank you letter. in attempting to call them several times, I was told by another person I did not sign my letter, so they did have it but not signed and they made no attempt to advise me. again I tried contacting the manager *** and no response, brushed off by everyone.

I entered a contract to loose the weight through the weight loss grant. I lost the weight, submitted the paperwork. My grant was rejected.
I have all documentation, all the e-mails.
You are supposed to get the money within 6 to 8 weeks, it has now been 6 months.
I am still fighting this.

I successfully completed the neccesary weight loss and provided all the needed documents, however I was denied by the weight loss grant organization. I am looking for the money they owe me.

The deal was to lose 56lbs and maintain that weight loss for 18months (end date was May 9th) I got a doctor's note stating I had achieved the neccesary weight loss. The program cost $2400 and I went with the approved weight loss company (Dalewood Health) now Trillium. I was to receive 80% of the $2400

I joined WLGO on May 15th 2017 file # 77RXXXXXXX and was suggested to go to Dalewood Health Clinic in Kitchener for my weight loss, I paid Dalewood $2100.00 on June 8,2017 to help me lose 50 pounds over 18 months. On Jan 19, 2017 which was the first day that I could weigh out on I went to my doctors and he weighed me out and I had lost 56 pounds. I did not upload my documents to WLGO as they were having a glitch with their computer system, when the system got fixed I uploaded all my document on Jan 28,2019. I sent them a message on May 13,2019 as it had been over 45 days since I uploaded my documents, I was told all documents were in order and they were reviewing my claim. I called back again around April 9,2019 was advised that they returned my weight out form to my doctor for more information, ( the only spot my doctor did not complete on the form was his email address) I spoke to my doctors office, they did receive it, WLGO wanted verification that it was his signature on the weigh out form, he verified that and sent it back on April 11,2019, On May 10 I received an email stating I was rejected due to incomplete validation documentation form doctor's office regarding weigh out, at which time I tried to contact *** could not get a hold of her as she was either in meetings, on lunch, or out of the office, tried to leave a message for her on her direct line but her inbox was full, I would think being they sent the weigh out form for my doctor to verify and him returning it would be verification that I lost the weight

After numerous emails and as of today June 2019, payment is on hold as part of a general fraud investigation.

I would like my agreed upon amount of $1800.00 to be paid.

I signed up through ***, who recommended them. I had to pay $200 to qualify for the grant. I spent over $2,500 on the weight loss program. I only spend that much because I was promised to receive 80% back.

I hit my goal weight after 3 months. I was told I would receive my grant after 1 month of hitting my goal weight. I received an email saying I would receive $2,118.73.

I hit my goal weight August 2018. It is now June 2019 and they will not give me the grant I was promised.

I have called and emailed several times, and each time they ***.
At first they were saying someone would call me before they sent the cheque, now they are saying I am being audited because other people apparently committed fraud.
I was first told I was being audited in April. They have all of my supporting documents proving I hit my goal weight and all of my receipts.

I have been hung up quite a few times when I call them. The manager refuses to speak to me.

Other (requires explanation) I want the money I was promised with interest for every month that they went over the promised date.

I signed up with Weight Loss Grants Organization with a start date of February 26, 2018 (end date February 26, 2019) and paid for Dalewood Health's program their "accredited" provider. I spent $2640.00 and the grant program pays back 80% up to $2,500.00. They consistently change rules and make it impossible to collect your grant money. I hit my weight loss goal of losing 40 lbs with Dalewood Health December 21, 2018. This is when I found out that in October 2018 WLGO changed their rules that you can no longer weigh out before your end date. Again, this was not a rule when I signed up for the program. They said I had to follow this new rule and wait for my portal to open on February 26, 2019 to receive my weigh-out form. I was then disqualified January 23, 2019 for not submitting my weigh-in form to a portal that never existed when my program started. They let me "re-submit" my form which had already been submitted via email the year before. Then February 26, 2019 came my portal was opened and I did not receive my weigh-out form. I requested it twice and the WLGO team insisted I can instead visit my doctor to have her perform the weigh-out without the form. At this time the rules and my portal stated you have 30 days after your end date to complete your weigh-out and submit all final documentation. My weigh-out was performed March 6th and I submitted all my documents to the portal the same day. April 17th I received an email stating my documents were going to be audited to verify my weigh-out etc. *** my doctor was never once contacted and instead they waited until their rule changed THE VERY NEXT DAY on April 18. April 18th is when WLGO decided to change their rules AGAIN stating weigh-outs must be completed ON your deadline and documents uploaded within 7 days. They disqualified me again based on a rule that was changed after my program was completed. NOW they are defaming participants on their website that have come forward about their *** organization for wrongfully denying their grants. As of today, their website states you can get them to re-review your file, but their committee is basing "late" weigh outs on how much you weighed at your weight-out and how "late" you weighed out. I did not weigh out "late" I had 30 days to weigh out and submit documents. Which I did and hit my goal. They will continue to make up excuses to not pay out grants ***.

Receive the grant funds promised. I spent $2640.00, was promised 80% back if I achieved my goal within the deadline. I weighed in with my doctor and provided proof of the achievement of the required goal and followed the posted rules at the time of my program-- I am due a grant reimbursement of $2,112.00.

I lost the weight that I was supposed to lose, sent in the information and was told that I was DENIED for a rule that wasn't even a rule on their page.

I joined Weight Loss Grant December 2017 with a goal to loss 35 pound within a year. I received an email December 4, 2018 stating "Effective December 7, 2018, your document portal will be open for 30-days during which period of time you must attend for a weight out using the attached form and upload it along with your supporting documentation for prompt review." I sent an email confirming I had 30 days to weigh out and then I followed up with a phone call and specifically asked can I weigh out on December 14, 2018. He confirmed that was fine. I uploaded all my documentations on December 14 and prior to uploading I call Weight Loss confirming I had everything prior to submitting. This was confirmed. January 25, 2019 I received an email stating I had not uploaded my weigh in form "If you do not upload your weigh-in form within 30 days of today, your Grant will be cancelled and you will no longer be eligible for consideration" The next day I contact them again and they confirmed they had the form and it was a computer "clinch". I also have an email dated November 10, 2017 from *** stating she received and would file right away. After this I would then call every 2 weeks for an update. I continually emailed *** requesting updates. At one point I was told they had not received confirmation of weigh out from my doctor that they requested January 24, 2019. My doctor confirmed it was faxed 3 times to the number provided and then they finally mailed it. April 24 I received an email from *** stating "I do see that your file has been prepped for the new fraud mitigation process, which means we received all the information we need. It is currently in queue for review. If you haven't heard anything in the next week, please feel free to reach out again." On May 8 I received the following "We regret to inform you that your Weight Loss Grant was not approved for the following reason(s):

"Applicant weighed out Dec 14th, 2018. Deadline Dec 7th, 2018. Applicant did not upload weigh in declaration form." Once I received this I went onto my portal and they had changed my "weigh in" from verified to declared even though I have email confirming. As per their website "If you are rejected for one or more of the reasons cited above, you will be permitted up to 10 days from the date of rejection to resolve any issues so you can be approved." I called on May 9 and was told *** would return my call within a week. On May 10, 2019 I sent an email appealing their decision and then again on May 17. To date I have not received an response.

I heard about the weight loss grant in 2017. I was advised that I had 14 months to loose the weight. I signed up with dalewood the preferred place and lost the weight. I submitted all the appropriate documentation as per the website and was advised that I was approved as I had submitted the correct documentation and would be receiving my cheque soon. After 2 months when I had not received anything I had contacted them again and *** reaches out and said I am being audited. They asked for the doctor to resubmit the weight in declaration which he has and I was told they received it. It's been over a month and I haven't heard anything they just keep saying my case is being escalated. ***. WLGO keeps telling me they don't know when my case will be reviewed. I email *** and no response. I just want my 1680 reimbursed as I have lost the weight followed the rules paid for the doctor to sign the form

I paid money in the amount of $2640.00 On April 30th 2018 and was promised to receive 80% of my money back if I lost the specified amount of weight they were supposed to give me a year. I lost the weight on October 4th and sent in everything they required. They said congratulations and they would review my file. I was going out of the country at the end of October and would not ne returning until 1st of April 2019.They said that my submission date was March 24th for my weigh in. They rejected me because I didn't weigh out with mr Dr. on that datw. Well I wasn't even here. Besides that I didn't receive any correspondence from them until May 22 which was my start up package. So how can they say that was a year.I feel I complied by sendidng in all the proper documentation and *** of my money by no fault of my own. Pretty impossible to weigh out March 24 when I wasn't even here.They didn't hold up the end of their contract which was supposed to be a year!

I want my 80% of the $2640.00 I paid. I lost the weight. I lost 36 pounds and the contract said 18% of total weight to qualify' I started out at196 lbs and lost 36 pounds thats more than 18$
