credential from [redacted] Public School is not a required document. The applicant has completed secondary school more than 10 years. - Verification of licensure from New York received on February 03, 2017 and processed on February 21, 2017. Evaluator deemed this document to be acceptable. The following document was reviewed by the PT Consultant: -The transcript(s) from [redacted] University and [redacted]. University, [redacted] were received from February 03, 2017 to August 01, 2017. The PT Consultant stated that the applicant had educational deficiencies and in order to meet the requirements of the assessment tool [redacted] needed to make up the following: Professional Education: Within the educational materials available for review, there is a lack of evidence in the following:   Clinical Education: insufficient clinical education hours   Total Professional Didactic Credits: 68 credits minimum; applicant has 116.5; applicant needs 0. Total Clinical Education Hours: (1050 hours required=22 credits) 22 credits minimum; applicant has 20; applicant needs 2.   Total Credits: 170 credits minimum; applicant has 175; applicant needs 0.   The evaluator assigned to this file will call the applicant no later than February 14, 2018 (as he is back in the office) to have a discussion regarding her file. However, the evaluator cannot give placement recommendations regarding where to make up deficiencies. He can only refer the applicant to contact an accredited PT program.   We trust that this will address the applicant’s concerns and that the matter will be closed.

We have conducted an investigation and the results show that there was a delay in the assessment review of the applicant’s credentials. Click on the Order Status link under Applicant Options, select your order and click the View [redacted] Applicant Report button. In response to the referenced complaint. [redacted] placed a CES order on June 09, 2017. Our organization is required by the applicant’s report recipient to review all documentation. The following are documents were received in the same envelope on June 08, 2017 from the Nursing Council of...

Trinidad and Tobago. Secondary school credential from Caribbean Examination Council. Deemed acceptable by the assigned evaluator.License verification from Trinidad and Tobago. Deemed acceptable by the assigned evaluator. Transcript from School of Nursing [redacted] The following documents were not accepted: Transcript from School of Nursing [redacted]. The applicant’s school has not closed and all documents must come directly from the school not the Nursing Council of Trinidad and Tobago. Once we have received all outstanding documents/missing educational components and has also met the assessment requirements then a CES Professional Report can be issued to the applicant’s report recipient. Our organization will continue to keep the applicant notified during the process. We trust that this will address the applicant’s concerns and that the matter will be closed.

I am rejecting this response because:
This is not what I asked for. Noting that CGFNS still did not issue a report as its still under 'review'. I asked for a full refund as I have taken my business elsewhere already. The university has never received any "RFA" for CGFNS. CGFNS have tried to contact the university using bogus email address. I was told by CGFNS in September 2017 that they send an RFA to UC using an email address starting with "records"  The university does not have an email address starting with 'records'. I noticed that the piece of correspondence with this in it has been removed from my online CGFNS account. This is most unprofessional to send a RFA to a bogus email address. Not only is CGFNS responsible for severely prolonging the time it will take me to get my US nursing licence because I have had to go with another agency, they have made my university send official transcripts 3 times, the nursing board of Australia send official nursing licence certificate twice, and most recently I have received correspondence from CGFNS through Revdex.com which was addressed to the wrong person ([redacted]).



I do not wish to have anything else to do with this agency and I want a full refund please.

applicant to sit for the Texas nurse licensure exam during the month of April
A brief summary of events is as
follows:

On December 14, 2015, [redacted] applied to
our Credentials Evaluation Service ProgramPrior to actually applying, the
applicant called CGFNS and was informed that she did not need a validated license
as part of the application processThis was in conflict with what the
applicant was told on November 13, 2015.·
Between January, and March, 2016, the
applicant called several times to verify the status of her applicationUnfortunately,
the applicant was given conflicting informationTexas requires that an
applicant's license be validated within one year of issuing the report ·
On March 14, 2016, we received the applicant's validated
license enabling us to proceed with completing her report for the Texas State
Board of Nursing.·
On March 24, 2016, the applicant filed a
complaint with your agency.·
On March 25, 2016, we issued the applicant's
report to the Texas State Board of Nursing
During our investigation of
this complaint, we noted a glitch in our IT system that did not properly
identify the applicant's licensure requirementsWe also noted that staff were
incorrect when relaying information to the applicant. This prolonged the customary 8-week
processing timeIn addition to issuing the applicant's report, we are working
with our IT department to rectify our internal IT processes to avoid any future re-occurrence of this nature. We will
also counsel staff involved in this incident.

September 7, 2016 Dear [redacted], I am writing as a follow-up to the Revdex.com complaint for [redacted] ID# 116713. On August 28, 2016, CGFNS submitted its response to the applicant’s initial complaint noting that we would continue to try to secure the documentation needed to complete the applicant’s report.  As a follow-up to this response, the applicant wrote that she did not find our response satisfactory.  Since this time we have continued trying to secure the necessary documentation for her report to no avail. However, we were able to complete the report using data from our existing data base. Therefore on September 6, 2016, CGFNS issued a professional report to the Texas Board of Nurse Examiners. [redacted] can view her copy on CGFNS Connect. We trust that this will address the applicant’s concerns and that the matter will be closed. any changes in the official signatory for any school and/or license registration. After three attempts on October 31, 2016, December 19, 2016 and January 05 2017 we were still waiting on a response from the nursing school.  On February 28, 2017 our Authentication and Verification department received confirmation from [redacted] that the signatures on the nursing documents submitted were authenticate. To expedite [redacted]’s order her file was assigned to a Senior Evaluator in the CES department. Based on the Senior Evaluator’s assessment review this applicant’s order will remain incomplete because the applicant has not submitted verification of nursing licensure from the Netherlands and [redacted] has not filled out (left blank) the Request for Academic Records/Transcripts form that was included with nursing transcripts. The Request for Academic Records/Transcripts form is an essential document because it gives an exact breakdown of theoretical instruction and clinical practice hours that the applicant has completed. Once we have received all outstanding required documents a CES Professional Report can be issued to the District of Columbia State Board of Nursing. We have communicated with [redacted] regarding what documents are still outstanding with her order. Our organization will continue to keep the applicant notified until we have received all the required documents to process her CES order.  We trust that this will address the applicant’s concerns and that the matter will be closed. credential from [redacted] Education Standard Authority License verification from Australia Transcript from [redacted] University Transcript from [redacted] College of Nursing We have received and verified the documents listed above.    After reviewing the documents received from [redacted] University, the evaluator had several key questions for the university that are needed to complete the report.  The evaluator has sent the list of questions to the school via email and a physical letter on February 12, 2018.  Once we receive the responses, we will be able to continue the evaluation.     We trust that this will address the applicant’s concerns and that the matter will be closed.

[redacted]. Deemed as acceptable by the evaluator. License verification from Philippines. Deemed as acceptable by the evaluator. The following documents were outstanding: Transcript from [redacted]. Deemed as unacceptable by the evaluator. After reviewing the documents received from [redacted], the evaluator needed to assess the applicant’s clinical concurrency. The school left this information blank on the Request for Academic Records/Transcripts form. CGFNS received a new form that was processed on March 26, 2018. The CES Professional Report is complete and will be available to the recipient on April 13, 2018.     We trust that this will address the applicant’s concerns and that the matter will be closed.

report with current documents that are on file. The last document to be processed was a transcript authorization form to [redacted] Center Post-Graduate Education on September 02, 2017. Our organization has not received any transcript information from [redacted]. As a result, the New York State Board of Education received a NYCVS Report based on current documents. A NYCVS Report was issued on January 18, 2018.   We trust that this will address the applicant’s concerns and that the matter will be closed.

Applicant was missing required elements to issue the report. When she completed the required elements, she believed that the report would be sent immediately; however, the evaluation process including the lacking requirements was not completed immediately. This letter was sent out yesterday, today,  October 28th was considered the 10th calendar day from when the complaint was mailed. I have contacted the customer and she is satisfied with the result.

evaluation and the outcome was that at the time of your education your school
was not approved by [redacted] and the title Licensed Practical Nurse is not a
recognized title in Jamaica by the Council. Also, in December 2014, you were

As a result, while you may have been approved for state
licensure in Florida, you do not meet the federal requirement at this time.
