We made contact with Mr [redacted] and apologized for the incident that took place in our optical department I explained that the optician recommendations were based on Mr***'s high minus power Without said recommendations, the finished product would have resulted in a pair of glasses with increased weight and would have also affected the cosmetic appearance of his lenses In most cases, our opticians recommendation's are based on what is written by the prescribing doctor Upon answering a few more legitimate concerns Mr [redacted] had regarding the incident, we were able to resolve it To regain Mr***'s confidence with our staff and in our Optical Department, we have extended an offer to make him a free pair of glasses to show that we were truly sorry for his experience in our department

Costco had senior leadership at [redacted] review his quote & declination of coverge.We had them reach out to the member via email & phone to go over the quote and provide him coverage options whether through their program or through the in house partner agency

Dear ***, Our memberships are valid worldwide, but not all countries have online shopping As I know, only US and Canada have online shopping available I have forwarded your complaint to our Taiwan offices and have someone contact you directly to discuss I apologize for any frustrations or miscommunication you may have received

Complaint: [redacted] I am rejecting this response because: We have spent hours and hours of our time trying to find a repair solution that would not require the entire floor being changed outThey have not made any effort in more then months to pursue any alternative solutionThey have only showed by their lack of involvement they really do not care that our home is been damaged I again have had another flooring company come this week and they too state that there is not any replacement product that is availableThat sanding down the floors would lose us our lifetime warranty on the balance of the floors in our homeThe only solution is complete replacement The cost is $ 31,I have sent Beltman, Costco and Lifestyle Solutions the quotes from the flooring places for thisAll three companies are responsible.Sincerely, [redacted] ***

[redacted] , Thank you for taking care of this issue I see that you issued a $credit to the member's credit card All, I noticed that we did not deduct the $coupon when the refunds were processed so I added the two together in my response to the member I checked with Tim Min OPS and the $will be viewed as a customer service credit so as to not recharge the member for this error Rachel, The response: We apologize for the inconveniences caused when the defective items you received were not replaced promptly and correctly We are reviewing the processes that caused the mis-communications and the delays in delivering the washer and the bottom sidekick washer We have refunded $3,which is the full amount of the order as well as an additional $credit to the credit card used on the original order

Revdex.com: I saw a request from CostCo for a copy of my receipt A copy is attached to this e-mail After talking with [redacted] I learned CostCo and [redacted] are "retail" pharmacies and retail pharmacies like to be called "standard" pharmacies These retail pharmacies negotiate the price CostCo can charge the insurance company's clients The negotiated price for those two medications plus the $per prescription came to $which is better than the $per medication it would have cost me had the insurance company and CostCo not negotiated HOWEVER, this information needs to be made known clearly to the public and especially senior citizens In my situation I can get my tier one prescriptions from [redacted] or from [redacted] free because I have a policy with [redacted] *** I have gone to CostCo Pharmacy for years but that will be no more Thank you Sincerely, [redacted] ***

Complaint: [redacted] I am rejecting this response because: Costco initially offered a $gift card after the project was delayed into the second week (Keith R [redacted] ) and Carpets Galore a $gift card (Marilyn R***) - which I thought was reasonable until MsR [redacted] claimed that I was responsible for the projects delay All of this was outlined in my email to Ms R [redacted] on February 22nd Additionally I asked Costco for $for per diem for a single week for a family of four - Costco sent a contract for $ It was days before this contract was mostly finished and my home was reasonably inhabitable I made some minor changes to the contract and signed it Costco rejected the contract My offer of February 22nd stands The contract stipulates my "complete satisfaction" for acceptance - nor has any Costco employee or their representative ask me to sign that acceptance They they have my final offer I could be requesting damages to my landscaping, damaged furniture, my ruined garage flooring, and loss of wages but I'm not Nobody from Costco or Carpets Galore management ever appeared on this job site or personally addressed my concerns going back to the end of the first week when this contract was scheduled to have been completed Nor would I have EVER agreed to any contractor being paid in full until a contract was finished A substantial deposit for materials - yes The entire amount upfront - no Joe G [redacted] the salesman falsely told me that the entire amount would not be billed until the work was done That was false And that is why I filed a similar dispute with American Express I signed this agreement based on years of experience with the Costco brand and I'm displeased that they now are taking the legal position that they "are not liable for non-performance of their subcontractors" despite Costco being written all over my contract! [redacted] I signed a contract with Costco based on product reputation and performance Not some sub-contractor who Costco now claims no responsibility for failing to meet the conditions written in their own contract Sincerely, [redacted]

We understand that he would like a more detailed explanation of why coverage was not offeredWe appreciate the opportunity to address his concernsMr*** contacted [redacted] on September 14, 2017, to inquire about obtaining an auto quote with their company through their partnership with usWhile [redacted] representatives were gathering information about the ***’s driving history, it was discovered that Mrs*** was involved in an accident within the past three years Unfortunately, that precluded [redacted] from being able to provide a quote to the ***’s through our partnershipTo ensure that they were following guidelines correctly, the sales representative who was speaking with Mr***, placed Mr*** on hold while he reached out to the [redacted] Auto & Home underwriting department for verification of those guidelinesHowever, it is my understanding that before the representative had a chance to fully explain the decision and provide more insight, Mr*** ended the conversationUnfortunately, a driver with any accident is ineligible to receive an auto quote with [redacted] In those situations, [redacted] likes to offer the option of getting a quote through their in-house agencyWe apologize if this option wasn’t made more clear to Mr*** when he calledIf he would like to explore that option, we would be more than happy to help him with that

Complaint: [redacted] I am rejecting this response because: There is no reason that all lug nuts should have been loose after 5,milesThe tires were not secured initiallyI rarely drive over mph because I live on an island where this is the maximum speedI spoke with several mechanics and customers of Costco and the conclusion was the lug nuts were not properly secured in the first place when the tires were purchasedSince then the car has been repairedThe tires and wheel rims have been secured properly with a competent certified mechanicThe technicians that put the tires on at Costco need to accept their incompetency in providing a service that is trusted to their service renderedIt is unfortunate that in this situation you chose to reply in such a mannerCostco's "customer satisfaction" no longer has any validitySincerely, [redacted] ***

We have successfully refunded the member in full and have followed up with him regarding his promotional Costco Cash Card for 60$ His tires were successfully installed on Sunday at our Location in IndianapolisThe member is now happy with the resolution provided and will be receiving a Costco Cash Card for the inconvenience as well

After review of case, there are no service bulletins listed for this particular Samsung modelThis unit was purchased in and member had the opportunity to purchase Square Trade for additional coverage but declinedMember was offered a good faith repair estimate but repair costs would be at the members expense as Costco is unable to recoup from the supplier and warranty coverage is NOT applicable
