The "signed" document is bogus.
It is the same document several other scavenger collection agency's tried to pass as official. Every time I dispute the debt they would pass it on to another collection agency. 
I did not go to college in texas and any loan document for a student loan would have been signed before 1999.

I disagree with NCO Consumer Affairs Team Lead [redacted] explanation. NCO called me before for this matter and I already explain them the situation. They told me that they make a mistake and will delete the account but they sent to credit bureau instead to destroy my FICO score. This is a criminal case they should be prosecuted. Remove or correct this information from my credit report.

I am rejecting this response because:
[redacted] of NCO Finnancial Systems told me via phone that the account was closed. Either [redacted] or the representative that wrote the response is lying and is falsely misrepresenting the account.  The response also does not explain why NCO waited several years to harass my family. The company's response also does not make up for the fact that I, as well as, my husband were treated very rudely by [redacted] and that she refused to answer our questions. Since [redacted] told me that the account was closed I would like for the account to be closed and documentation proving that the account is closed sent to me.

June 18, 2014Dear [redacted]: I acknowledge receipt of your May 29, 2014 complaint regarding the above reference account. Please be advised that NCO is collecting on an account that [redacted] University forwarded to NCO with a total balance due of $626.36. Please note that...

the account relates to unpaid fees in the amount of $482.30 that [redacted]. [redacted] still owes to the University. NCO does not own this account and was only retained to perform collection activity. When the account was placed for collection, the University added an additional $144.06 to the principal balance as a charge for collecting on the account. Please note that before [redacted]. [redacted] account was forwarded to NCO, the University advised [redacted]. [redacted] if the account should be placed for collection, [redacted]. [redacted] would be responsible for paying the collection fees. According to NCOs account notes, NCO has already mailed him the validation materials relating to this account. NCO is forwarding you the validation that NCO already sent that reflects that [redacted]. [redacted] owes $482.30 for unpaid fees. NCO is also forwarding you billing statements that the University sent before the account was placed for collection.
Please note that per the Universitys guidelines, NCO does not have the authority to settle his account. Nonetheless, in response to [redacted]. [redacted]s complaint, the client has agreed to waive the $144.06 in collection costs as a courtesy. NCO has updated the account to reflect that [redacted] is now only responsible for paying the $482.30 principal balance.
NCO has updated the account to prevent NCO from initiating communication with [redacted] *. [redacted] and has also transferred the account to a managers queue. Please advise [redacted]. [redacted] that he may contact [redacted] directly at ###-###-#### to discuss repayment options.
