Scott and White agrees I never received a Treatment or Observation room, but cites billing policy and my deductible as a reason for charging me for something I did not receive nor agree to paying.

I am rejecting this response because:This case is not resolved.  I have not received any information from Scott and White other than they said they were going to appeal the charges from Seton.  Today I received an appeal form from Scott and White with no explanation of what it is for and why I need to fill it out.  There are four pages of legal disclaimer included that do nothing to explain what the appeal form is for.  This is ridiculous.  I have never experienced an insurance company refusing to pay their bills.  I can see that this is just an endless cycle of delay.  It looks like a complaint to the Texas Department of Insurance is in order.
