Although Ms*** trada qualifying device, she did not purchase an Apple iPhone or iPhone Plus device, as such; it is T-Mobile’s position that her account is not eligible for the offer Nevertheless, in an effort to amicably resolve this matter, on September 23, 2017, T-Mobile issued a onetime bill credit to the account in the amount of $for the difference between the $tradcredit already provided and the $total tradfor the disputed offer. • Tax Time Switcher Offer; and
• Tax Season Smartphone Offer
As can be expected, there are several eligibility requirements for each of the above promotions; some of these requirements were met and others were not
To begin with, our records reflect that Ms*** met all of the requirements for the Tax Season Smartphone offer and as such was sent a Prepaid MasterCard with the full promotional amount
With respect to the Tax Season Smartphone Offer, Ms*** did not meet the requirementsAlthough Ms*** did activate service during the offer window and ported in a number from a different provider, the promotional offer also required that customers submit an online rebate submission form at www.t-mobile.com/promotions within days of activationUnfortunately, T-Mobile has no record of Ms*** submitting her online submission form and as such is no longer eligible for the $per line promotional offer
However, in an effort to amicably resolve this matter, T-Mobile has agreed to apply a onetime lump sum credit to Ms***’s account in the amount of $300.00, which represents the two lines she ported in to T-MobileMs***’s account reflects a credit balance of $77.59, which will be applied towards her bill due on July 15,

In reviewing the records for the above account it appears that Mr*** was able to utilize the service in several countries that were covered under Simple Global and therefore he incurred international long distance charges for calls in the initiated directly from his T-Mobile handsets for the mobile numbers ending in 7437, and in the amount of $per minute

T-Mobile regrets any concern to Ms*** in regards to her participation in T-Mobile’s Un-carrier Carrier Freedom offerWith Carrier Freedom, consumers can get reimbursed for their current carrier's early termination fees or device payment plan remaining balances (“cancellation costs”) as tracredits and a prepaid debit card when they switch to T-Mobile and tratheir devicesAs can be expected, there are several eligibility requirements for our Carrier Freedom reimbursement which are available at https://www.switch2t-mobile.comIt is important to note however that in order to qualify for the reimbursement, the T-Mobile account must be in good standing
T-Mobile records confirm that Ms*** traded in handsets and received a total trade in credit of $80.00, ported in her mobile numbers and purchased three new handsetsRegrettably, on March 9, 2017, only hours into service, Ms*** returned her handsets and ported her mobile numbers out to another carrier closing her T-Mobile accountAs the account is closed Ms*** does not qualify for any reimbursement
However, in an effort to amicably resolve this matter, on April 21, 2017, T-Mobile spoke with Ms*** and agreed to refund her the trade in credit she received of $via a prepaid credit card which she acceptedMs*** should allow seven to ten business days for delivery and T-Mobile regrets any inconvenience to her
